External mandibular fixation for gunshot fractures: report of 2 cases.

High-energy ballistic injuries may cause comminuted facial fractures. Treatment of such fractures might be challenging because of in-fection and soft- and hard-tissue loss. These cases may not be amenable to open reduction and internal fixation. We present 2 cases of gunshot fractures, for which external fixation was used as a surgical step before definitive treatment. With the use of external fixation, existing infection had been controlled and soft tissues had been restored, which allowed oral rehabilitation with reconstruction plates and autogenous bone grafting, if needed.

Mandibular Ramus Fractures: A Case Series of Diversity in Rarity.

Mandibular ramus fracture is usually minimally displaced as it is surrounded by the medial pterygoid medially, masseter laterally, and the pterygomasseteric sling inferiorly. They are commonly caused either by road traffic accidents or interpersonal violence. Ramus fracture is usually seen in conjunction with other mandibular fractures and is seldom found alone. The ramus is located at the congregation of the dentate and the non-dentate parts of the mandible. Ramus fractures are generally managed by closed reduction when minimally displaced but this technique has its disadvantages like poor ma

Short-Segment Drilling Guides for the Management Comminuted Mandibular Fractures.

Treatment of a severely comminuted mandibular fracture is challenging. This technical note describes a novel office-based workflow, combining virtual surgery planning with short-segment drilling guides. The authors reduced the comminuted mandibular fractures via virtual surgery planning. Then, the reconstructed mandible model was printed using an in-house 3D printer. Next, the reconstruction plate was preformed according to the shape of the mandibular model surface, and the position of the screw hole in the mandibular surface was determined. Finally, hand-made short-segment drilling guides for

Minimally Invasive "Trocar-Free" Approach for Rear Mandibular Fractures Management.

The request of minimally invasive surgery is progressively expanding the indications of endoscopically assisted intraoral access for mandibular traumas. The aim of our study is to assess how much the use of the angled drill may affect the outcome of patients treated for rear mandibular fracture. In our retrospective case-control trial we enrolled 36 patients with mandibular rear fractures treated through endoscopically assisted intraoral access. Eighteen patients were treated by using an angled drill ''trocar free,'' and 18 treated by linear drills placed through trocars. Surgical times, hospi

Treatment and complications of mandibular fractures: a 10-year analysis.

The surgical treatment and complications of patients with mandibular fractures in Amsterdam over a period of 10 years are analysed. Between January 2000 and January 2009 225 patients were surgically treated for a mandibular fracture (mean age of 32.6 (SD±14.6) years). A total of 426 fracture lines were identified. Of 213 dentate patients 29 patients were treated primarily with intermaxillary fixation (IMF). IMF combined with osteosynthesis was performed on 99 patients. Seventy-nine patients received IMF only per-operatively to make open reduction and internal fixation (ORIF) possible. Of 12 ed

Combined surgical approach retromandibular and intraoral to subcondylar mandibular fractures.

BACKGROUND: Condylar and subcondylar fractures are very common and account for at least one third of all mandibular fractures. In literature, little agreement exists about the management of extracapsular condylar and subcondylar fractures in adults. Some studies provide better results of occlusion, masticatory function, mouth opening, and bone morphology with surgical treatment. Conversely, other studies report excellent results with conservative treatment avoiding complications such as facial nerve injury and unsightly scar. METHODS: In this study, we report our experience of 25 condylar/sub

Reconstruction of old orbital wall fractures with mandibular outer cortex.

BACKGROUND: This study focused on using the mandibular outer cortex in the reconstruction of old orbital wall fractures. METHODS: Old orbital wall fractures of 46 patients were reconstructed with the mandibular outer cortex. The surgical results were evaluated by performing clinical follow-up and by measuring the changes in the orbital volume. Donor-site complications were also evaluated. RESULTS: All patients had good aesthetic and functional results, with an accurate repair of orbital wall defects and the restoration of orbital volume. There were no obvious major donor-site complications.

Functional outcomes following surgical treatment of bilateral mandibular condylar fractures.

Debate continues regarding unilateral or bilateral treatment for mandibular condylar fractures. This retrospective study evaluates the functional outcomes of bilateral condylar process fractures after surgical intervention. From May 1994 to December 2004, 51 adult patients with bilateral mandibular condylar process fractures were studied. There were 33 cases of bilateral condylar fractures (type I); 12 cases of condylar-subcondylar fractures (type II); and six cases of bilateral subcondylar fractures (type III). All patients underwent open reduction and internal fixation. Four patients had chi
