Non-alcoholic fatty liver disease (NAFLD) comprises a heterogeneous group of metabolic liver diseases and is characterized by the presence of steatosis in at least 5% of hepatocytes. The aim of our study was to assess the effect of the combination therapy of empagliflozin + metformin vs. metformin monotherapy on NAFLD progression in type 2 diabetic (T2DM) patients. Sixty-three metformin-treated T2DM patients who were SGLT2i-naïve and had an ultrasound diagnosis of NAFLD (aged 60.95 ± 11.14 years; males, 57.1%) were included in the present analysis. Thirty-three started the combination therapy.

AIMS: Cardiovascular (CV) disease is a major cause of reduced life expectancy in type 1 diabetes (T1D). Intensive insulin therapy prevents CV complications but is constrained by hypoglycaemia and weight gain. Adjunct metformin reduces insulin dose requirement and stabilizes weight but there are no data on its cardiovascular effects. We have therefore initiated an international double-blind, randomized, placebo-controlled trial (REMOVAL: REducing with MetfOrmin Vascular Adverse Lesions in type 1 diabetes) to examine whether metformin reduces progression of atherosclerosis in adults with T1D. AIM: The aim of the study is to compare the effects of metformin and insulin treatment for gestational diabetes mellitus (GDM) on vitamin B12 and homocysteine (Hcy) status.
METHODS: Women with GDM, who met criteria for insulin treatment, were randomly assigned to metformin (n = 89) or insulin (n = 91) in the Adelaide cohort of the metformin in gestational diabetes (MiG) trial. Fasting serum total vitamin B12 (TB12), holotranscobalamin (HoloTC), a marker of functional B12 status and plasma Hcy concentrations were measured at 20-34 weeks (at randomization) and 36 weeks gestation, then at 6-8 we

INTRODUCTION: Insulin resistance plays the central role in the pathophysiology of polycystic ovary syndrome (PCOS). While metformin is widely used, its metabolic and androgen-lowering benefits are modest and often limited by tolerability, highlighting the need to evaluate newer therapeutic agents targeting insulin resistance in PCOS. Dapagliflozin complements metformin's effects through an insulin-independent mechanism of action and may be beneficial in improving metabolic outcomes in PCOS.
METHODS: Women diagnosed with PCOS based on Rotterdam criteria were randomly allocated to receive eithe

Metformin exerts a good efficacy for gestational diabetes mellitus (GDM) treatment by regulating gluconeogenesis and insulin resistance, while no consensus about its preventive effect on GDM is reached yet. Thus, this meta-analysis aimed to comprehensively investigate the prophylactic administration of metformin in pregnant women at high risk of GDM. Databases (EMBASE, PubMed, Cochrane, CNKI, Wanfang, CQVIP) were searched to screen papers concerning the GDM prevention using metformin in women at high risk of GDM (polycystic ovary syndrome (PCOS), obese, and pregestational insulin resistance pa

Durable glycemic control remains challenging in patients with type 2 diabetes mellitus inadequately controlled with metformin alone. Liraglutide, a glucagon-like peptide-1 receptor agonist, has been proposed as a beneficial add-on therapeutic option. This retrospective study included adults with type 2 diabetes mellitus treated between March 2021 and March 2025. Patients were divided into a combination group receiving liraglutide plus metformin and a control group receiving metformin alone. Glycemic parameters and safety outcomes were assessed at baseline and after 6 months of treatment. Stati

BACKGROUND: Type 2 diabetes mellitus (T2DM), being a progressive metabolic disorder, often needs combination therapy for optimal glycaemic control. Metformin is the standard first-line agent, but its efficacy may diminish over time. Ranolazine, an antianginal drug with emerging antihyperglycaemic properties, has shown promise in improving glucose regulation. This study evaluated the efficacy of ranolazine as an add-on therapy to metformin compared with metformin monotherapy.
METHODS: A quasi-experimental comparative study was conducted in a tertiary care hospital in eastern India. Adults aged

BACKGROUND: Polycystic ovary syndrome (PCOS) is a common endocrine disorder among reproductive-age women, often accompanied by insulin resistance, obesity, and increased metabolic risk. While Metformin (MET) is commonly used to improve insulin sensitivity, its limited effect on postprandial glucose has led to interest in combination therapies. Exenatide (EX), a glucagon-like peptide-1 receptor agonist, may offer complementary benefits.
OBJECTIVE: To assess the effectiveness of EX combined with MET (EX + MET) versus MET alone (MET) in improving insulin resistance and metabolic outcomes in over

Polycystic ovary syndrome (PCOS) is one of the most prevalent metabolic diseases during female reproductive life, often associated with insulin resistance and hyperprolactinemia. The efficacy of metformin and cabergoline for managing PCOS remains debated in the literature. This three-arm interventional study in Iraq assessed the effects of these drugs on body mass index (BMI), hormonal balance, and uterine artery blood flow in 75 women with PCOS and hyperprolactinemia. Participants were randomized into three groups: metformin (500 mg twice daily), cabergoline (0.5 mg weekly), and a combination

BACKGROUND: Intense statin therapy was found to impair testosterone production in men. Metformin administered to subjects with hypergonadotropic hypogonadism decreased gonadotropin production. The current study was aimed at investigating whether metformin treatment modulates the impact of high-dose rosuvastatin therapy on hypothalamic-pituitary-testicular axis activity in men.
METHODS: The study included 43 very high cardiovascular risk men with late-onset hypogonadism, 20 of whom had been treated with metformin (1.7-3 g daily) for at least 6 months. In all subjects, unsuccessful initial stat

Recent studies suggest that metformin, which is a commonly used oral anti-hyperglycemic agent of the biguanide family, may reduce cancer risk and improve prognosis, yet the detailed mechanisms by which metformin affects various types of cancers, including pancreatic cancer, remain unknown. The aim of the present study was to evaluate the effects of metformin on human pancreatic cancer cell proliferation in vitro and in vivo, and to study microRNAs (miRNAs) associated with the antitumor effect of metformin. We used the human pancreatic cancer cell lines Panc1, PK1 and PK9 to study the effects o

PURPOSE: Metformin was the first medication targeting insulin resistance in PCOS, and it has been extensively studied as a metabolic treatment option. In recent years, inositols have emerged as potential treatment options for PCOS, but confidence in the available evidence supporting their use is limited.
METHODS: We comprehensively searched PubMed, Embase, and Cochrane databases for RCTs comparing the use of combined metformin and inositol versus metformin alone in women with PCOS. A random-effects model was used to calculate the risk ratios (RRs) and mean differences (MDs) with 95% confidenc

Metformin, the most widely prescribed oral antihyperglycemic agent, is established as the first-line therapy for type 2 diabetes mellitus (T2DM) owing to its efficacy, affordability, and safety. Increasing evidence indicates that its benefits extend beyond glycemic control, encompassing cardiovascular protection and diabetes prevention in individuals at elevated cardiometabolic risk. Mechanistic studies demonstrate that metformin exerts pleiotropic effects through activation of AMP-activated protein kinase, modulation of the gut microbiota, inhibition of pro-inflammatory and oxidative stress p

Blastic plasmacytoid dendritic cell neoplasm (BPDCN) is a rare and aggressive hematologic malignancy with limited therapeutic options. Metformin, a commonly prescribed antidiabetic drug, has recently gained attention for its anticancer potential, but its effects on BPDCN remain unknown. Here, we show that metformin reduces cell viability and induces caspase-dependent apoptosis in both established (CAL-1, GEN2.2) and primary BPDCN cells, partly through activation of the intrinsic apoptotic pathway. Mechanistically, metformin activates AMPK and disrupts mitochondrial respiration and glycolysis,

BACKGROUND: Sodium-glucose co-transporter 2 inhibitors (SGLT2i), when combined with metformin (COMBI), offer multi-organ protective effects in patients with type 2 diabetes (T2D), particularly those at high risk of cardiovascular or renal complications. However, the underlying molecular mechanisms remain poorly understood.
METHODS: We profiled 303 targeted serum metabolites in 1494 participants of the KORA study, including T2D patients treated with COMBI therapy, metformin monotherapy, or no glucose-lowering medication. Additionally, metabolomic profiling was quantified on seven tissues (plas
