Revision of the surgically overshortened nose.

The short nose deformity may arise from multiple etiologies including iatrogenic following rhinoplasty. The weakening of the cartilaginous support and the contractile forces imposed by the healing soft tissue envelope may result in the short nose deformity. The focus of this article is to present the senior author's approach to this nasal deformity.

Revision of the cleft lip nose.

Secondary or revision rhinoplasty for the cleft nasal deformity represents one of the most challenging problems in rhinoplasty surgery. The secondary nasal deformity of the unilateral cleft lip involves a retrodisplaced dome of the ipsilateral nasal tip, hooding of the alar rim, a secondary alar-columellar web, and other deficiencies. This article discusses techniques to achieve the best possible outcome for patients with cleft nasal deformities. We emphasize the importance of early intervention by way of primary cleft rhinoplasty and highlight the typical challenges presented in delayed (seco

Primary localized amyloidosis of the nose.

Two rare cases of primary localized amyloidosis of the nose are described. A 14-year-old woman presented with anosmia, facial pain, and recurrent episodes of bilateral epistaxis persisting for 2 years. Endoscopic sinus surgery was performed and amyloidosis was diagnosed. Amyloid was also found in the trachea. A 27-year-old woman presented with decreased bilateral hearing and recurrent episodes of bilateral epistaxis. Tympanostomy tube placement was performed and observation continued. The optimum treatment of amyloidosis of the nose has not yet established. We consider that the treatment shoul

Structure rhinoplasty of the long nose.

Rhinoplasty modifies the aesthetic appearance and functional properties of the nose with operative manipulation of the skin, underlying cartilage, bone, and linings. A long nose is an aesthetically undesired feature disturbing the harmony of the face. The underlying pathology of the long nose may be due to either a long septum that invades the lip or dislocation of the alar cartilages downward from the aponeurotic attachments to the septal angle. The increase in the nasal height due to a dorsal nasal hump may give an illusion of a long nose. An absent or shallow frontonasal angle also gives th

A nodulo-ulcerative lesion on the nose.

Lupus vulgaris (LV) is a chronic, progressive, and potentially destructive form of cutaneous tuberculosis commonly seen in previously sensitized individuals with moderate to high immunity. We present a case of LV located on the nose of an 84-year-old female patient, discuss the diagnosis and treatment modalities, and emphasize the importance of having a high index of suspicion for this condition.

The surgical repair of half-nose.

Absence of half-nose is an extremely rare congenital malformation, which has a devastating impact on the patient and the family. A review of indexed English-language literature found 91 cases of half-nose, including 50 patients with proboscis lateralis. Pathogenesis is not clear, and the reported cases have sporadically occurred. Many aspects must be considered when reconstructing a congenital half-nose, such as timing of surgery, type of tissue to be used and the need to reconstruct nasal airway. The aim of this article is to present personal experience in seven cases of half-nose reconstruct

Nasal trauma and the deviated nose.

As the most prominent facial feature, the nose carries an increased risk of traumatic injury. Nasal fracture is the most common bone injury of the adult face and frequently results from motor vehicle accidents, sports-related injuries, and altercations. Although often initially considered minor, nasal fracture may eventually result in significant cosmetic or functional defects. Optimal management of nasal trauma in the acute setting is critical in minimizing secondary nasal deformities. In recent years, numerous guidelines have been described to refine and optimize acute nasal trauma managemen

[Ultrasonography of the nose and paranasal sinuses].

UNLABELLED: Pathological changes of the sinuses and the nasal bones are rarely examined sonographically. However, both acute and chronic sinusitis in maxillary and frontal sinuses and fractures of the nose could all be diagnosed correctly by diagnostic A-mode ultrasound, which is of clinical importance if children or pregnant patients are examined. The aim of the study was to analyse the reliability of A-mode ultrasonography in diagnosing sinusitis and nasal bone fractures, compared to results of CT, X-ray and clinical symptoms. MATERIAL AND METHODS: 86 patients were included in the study: 20

Olfactory and sensory attributes of the nose.

The human nasal cavity contains multiple sensory and olfactory structures. The nasal mucosa with its complex innervation detects the danger substances in the air and stimulates the protective reflexes. Healthy olfactory mucosa allows for appreciation of pleasant aromas and food flavors. The olfactory nerve, in concert with the trigeminal nerve, serves as a main interpreter and modulator of chemosensory information. The anatomy of the olfactory neuroepithelium, which occupies only a small portion of the nasal mucosa, is generally well understood, while the presence and distribution of the senso

Extraskeletal Ewing's sarcoma of the nose.

A 20-year-old woman presented with nasal obstruction and slight epistaxis. The obstructing lesion was excised and microscopy showed a neoplasm composed of comparatively uniform undifferentiated cells forming solid nests. The cytoplasm of the cells was clear but poorly demarcated, partly vacuolated and contained much glycogen. Although widespread in the nasal mucosa, the cells did not penetrate into the underlying bone. The cells expressed the MIC2 gene (using the CD99 marker). Electron microscopy showed simple cells with a small number of mitochondria, many glycogen particles; there were no ne

Microsurgical replantation of the amputated nose.

A case of successful replantation of the nose is presented. Two arteries and one vein were anastomosed, providing a stable framework for direct revascularization of the amputated nasal segment. This resulted in complete survival of the nose, with an excellent aesthetic result. However, despite successful microsurgical arterial and venous repair, significant postoperative blood loss still occurred as a result of anticoagulation. In cases of the amputation of specialized structures, the improved functional and cosmetic result obtained with replantation must be weighed against the risk of blood-b

Mucormycosis of the nose and paranasal sinuses.

Rhinocerebral mucormycosis is an invasive fungal infection initiated in the paranasal sinuses that frequently progresses to orbital and brain involvement. If recognized early, involvement is limited to the nasal cavity and paranasal sinuses. Diabetics in poor control are at greatest risk, however, any immunocompromised individual may be infected. The mainstays of therapy are reversal of immunosuppression, systemic amphortericin B, and surgical débridement. Survival has improved dramatically, yet deaths still occur if the infection is not recognized and not treated early in its course or if the

[A method for true nose lengthening].

There are various methods of lengthening the nose after trauma or for congenitally short noses. Forehead flaps and bone and cartilage grafts to the nasal dorsum are routinely performed. Bone grafts to the dorsum are onlay grafts that can show resorption. In craniofacial procedures, e.g., after Le Fort II/III osteotomies, the nose is brought forward and downward as necessary. This principle for nasal lengthening has been applied to 11 patients with congenitally short noses or after trauma. A coronal incision was made, an extensive subperiosteal skin undermining above the entire nasal bones and

[The classification of combined deformities of the nose].

A classification of deformations of the nose is proposed. All deformations are classified into 5 degrees: I) deformations involving one compartment of the nose, II) two compartments, III) three compartments, IV and V) combined deformations involving more than 4 compartments. This classification helps the physician to be better oriented in the great variety of combined deformations of the nose and permits a more accurate planning of the steps of surgery. This classification is not a final or exhausting, it is merely an attempt to present a scheme of the great variety of combined deformations of

Histamine receptors in the human nose.

The aim of this study was to determine the role of histamine receptors in the nose. The effects of intranasal histamine challenge were compared with those of a specific H1-receptor agonist, betahistine and a specific H2-receptor agonist, impromidine, in 11 normal individuals and four with rhinitis. Sneezing, nasal irritation and hypersecretion were induced by histamine and the H1-receptor agonist, betahistine only. Nasal airway resistance (Rna) was measured by passive anterior rhinomanometry. Histamine, betahistine and impromidine all induced rises in Rna in both normal individuals and those w
