Primary malleus fixation: diagnosis and treatment.

Primary malleus fixation occurs in an otherwise normal middle ear without evidence of congenital deformity and without chronic inflammatory changes. It occurs in the latter decades of life and is frequently associated with sensorineural presbycusis. We believe it is a ligament ankylosis with osteoarthritis related to the aging process. The diagnosis of malleus fixation is facilitated through the use of a modified Siegle pneumatic otoscope in conjunction with the Zeiss binocular microscope. The literature pertaining to this subject as well as the more historical reports are reviewed. Goodhill h

Effect of length variations in 3D-printed middle-ear prostheses on ossicular chain mobility.

BACKGROUND: Patient-specific 3D printing may improve ossiculoplasty, yet the mechanical tolerance to prosthesis length remains poorly quantified. We evaluated how systematic lengthening of a custom 3D-printed incus replacement affects middle-ear mobility in human temporal bones. METHODS: Five fresh-frozen human temporal bones were prepared through a posterior tympanotomy. Cone-beam CT was used to reconstruct specimen-specific ossicular geometry; the incus was digitally removed and a malleus-stapes prosthesis was designed and printed by stereolithography. For each specimen, three prostheses we

Electrocochleographic and mechanical assessment of round window stimulation with an active middle ear prosthesis.

Mechanical stimulation of the round window (RW) with an active middle ear prosthesis (AMEP) has shown functional benefit in clinical reports in patients with mixed hearing loss (MHL). Further objective physiological data on the efficacy of RW stimulation is needed, however, to demonstrate that RW stimulation with an AMEP can generate input to the inner ear comparable to acoustic input. Cochlear microphonic (CM) and mechanical (stapes velocity) responses to sinusoidal stimuli were measured by electrode and laser Doppler vibrometry in eight chinchillas in response to normal acoustic stimulation

Influence of simulated adverse events of the floating mass transducer in a mechanical middle ear model.

BACKGROUND: The Vibrant Soundbridge Implant provides multiple options for coupling the Floating Mass Transducer (FMT) to the ossicles. A parametric evaluation in clinical trials is difficult. AIMS/OBJECTIVES: We studied the vibratory output of the FMT with three couplers-short process, former long process, and current long process-in an acoustic-mechanical middle ear model (AMEM) under simulated impaired coupling conditions. MATERIALS AND METHODS: The AMEM, with life-sized ossicles, was tested via acoustic and electrical (FMT-) stimulation. Simulated impairments included accidental bone-cont

Ten years of active middle ear implantation for sensorineural hearing loss.

OBJECTIVES: To evaluate long-term benefits of a totally implantable active middle ear implant (AMEI) that has been used in a single implanting center for over 10 years. METHODS: Forty-one subjects who underwent implantation with an Esteem® AMEI during a 10-years period were evaluated on the auditory benefits, as derived from pure tone and speech audiometry tests. The analysis included a comparison with a conventional hearing aid, the problematics related to the battery duration and surgical replacement and, finally, the complication rate. RESULTS: Over 80% of the implanted subjects maintaine

Revision stapes surgery for recurrent transmissional hearing loss after stapedectomy and stapedotomy for otosclerosis.

A total of 20 stapedotomy and 24 stapedectomy cases were retrospectively reviewed to establish the causes of failure, and to evaluate hearing results after revision surgery. Our series included 23 male and 21 female patients. Mean age at revision time was 42 years, and the mean interval from primary surgery and revision stapes surgery was 27 months. The retrospective review of our data, revealed that the most common cause for revision surgery was a displaced prosthesis (47.7%). After revision surgery, the mean post-operative air-bone gap was 14.78 dB. A mean post-operative air-bone gap within

Middle ear malformations combined with cerebrospinal fluid otorrhea: A case report.

RATIONALE: Middle ear malformations (MEMs) encompass a diverse spectrum of congenital anomalies that significantly impair auditory function. They account for approximately 0.5% to 3% of conductive hearing loss cases, with etiologies involving both genetic and environmental factors. Clinically, MEMs complicated by cerebrospinal fluid (CSF) otorrhea are extremely rare. PATIENT CONCERNS: A 17-year-old male presented with right-sided hearing loss of over 7 years' duration. Otoscopic examination revealed a perforation in pars flaccid of the tympanic membrane. Pure-tone audiometry confirmed moderat

Sound transfer of active middle ear implants.

Implantable hearing aids are gaining importance for the treatment of sensorineural hearing loss and also for mixed hearing loss. The various hearing aid systems, combined with different middle ear situations, give rise to a wide range of different reconstructions. This article attempts to summarize the current knowledge concerning the mechanical interaction between active middle ear implants (AMEIs) and the normal or reconstructed middle ear. Some basic characteristics of the different AMEIs are provided in conjunction with the middle ear mechanics. The interaction of AMEIs and middle ear and

[Results after rebuilding the ossicular chain using the autogenous incus, ionomer-cement-and titanium implants (tympanoplasty type III)].

BACKGROUND: A defective ossicular chain can reliably be reconstructed with standardized techniques using e. g. modern alloplastic materials. The comparison of clinical and functional results have proved its worth. Prospective clinical trial as well as collecting and evaluating relevant intraoperative and postoperative findings may be helpful to find the appropriate bone substitute in each case when rebuilding middle ear structures. METHOD: In 354 middle ears (332 patients) the defective or destroyed ossicular chain was rebuilt with the carefully trimmed autogenous incus (n = 83), with ionomer

Ossiculoplasty in the Setting of Erosive Middle Ear Disease.

Erosive middle ear disease (EMED) involves a spectrum of otologic conditions that involve chronic inflammatory processes, including chronic otitis media with or without cholesteatoma, that contribute to osseous erosion of the middle ear and ossicular chain. Treatment of EMED involves the eradication of the disease followed by ossicular chain reconstruction to optimize hearing outcomes. There are many challenges regarding ossiculoplasty in EMED, including the potential for microscopic residual disease, variability of ossicular chain status following disease eradication, and concern for recurren

Methods and reference data for middle ear transfer functions.

Human temporal bone specimens are used in experiments measuring the sound transfer of the middle ear, which is the standard method used in the development of active and passive middle ear implants. Statistical analyses of these experiments usually require that the TB samples are representative of the population of non-pathological middle ears. Specifically, this means that the specimens must be mechanically well-characterized. We present an in-depth statistical analysis of 478 data sets of middle ear transfer functions (METFs) from different laboratories. The data sets are preprocessed and var

[The inverse procedure in stapedioplasty].

The inverse procedure in stapedoplastic with the primary stapedectomy was done in 55 patients with otosclerosis. The early cochleo-vestibular disorders (1.8-3.6%), graver sensoneural damage after 6 months (1.8%) and bone--air conduction gap were significantly better in relation to the patients operated in classical way. Many advantages of inverse procedure were analyzed in prevention of inner ear lesions and better functional outcome.
