Testing the vestibular-ocular reflexes: abnormalities of the otolith contribution in patients with neuro-otological disease.

Conventional vestibular rotation testing with the head centered on the axis stimulates the semicircular canals evoking compensatory eye movements. If the head is placed forwards of the axis in an eccentric position the otoliths are also stimulated by a tangential linear acceleration acting laterally to the skull. In normal subjects the additional otolithic stimulus evokes compensatory eye movements with a higher gain than with head centred, particularly for high frequency (greater than 0.1 Hz) stimuli. The responses with head centred and eccentric in various patients with known/suspected neuro

Development of a vestibular implant for otolith stimulation: A mouse model study.

OBJECTIVE: Worldwide, bilateral vestibulopathy (BVH) affects ≈1.8 million adults. Its symptoms include oscillopsia, postural instability, and gait disturbances, which significantly impair the patient's quality of life. Despite advancements in vestibular rehabilitation, these approaches remain supportive and cannot restore vestibular function. We aimed to evaluate the efficacy of a vestibular implant targeting the utricle and saccule in a mouse model of BVH. METHODS: A total of 30 CBA/J mice underwent bilateral vestibular destruction, followed by implantation of a vestibular implant in the lef

Spontaneous Canalith Jam and Apogeotropic Horizontal Canal Benign Paroxysmal Positional Vertigo: Considerations on a Particular Case Mimicking an Acute Vestibular Deficit.

OBJECTIVE: Canalith jam refers to a condition caused by an otolithic clump blocked inside a semicircular canal, generally provoked by canalith repositioning procedure. We describe the first case of spontaneous canalith jam mimicking an acute vestibular deficit. PATIENT: We report the case of an 82-year-old woman who suffered a sudden episode of persistent rotational vertigo with nausea and vomiting, not provoked by head movements. INTERVENTIONS: Videonystagmography revealed a horizontal right-beating spontaneous nystagmus, inhibited by visual fixation. Surprisingly, the positional test showe

Changes in Listing plane thickness caused by vestibular schwannoma: a parameter for evaluating the accuracy of the gravity-oriented internal model.

OBJECTIVE: Three-dimensional analysis of video-oculograms can be used to calculate Listing plane for patients and experimental subjects. Listing plane reflects the head's orientation with respect to gravity, which suggests that the plane is derived from otolithic vestibular input, itself, or from a gravity-oriented internal model constructed through integration of visual, vestibular, and proprioceptive sensory inputs. The goal of this study was to determine whether the Listing plane can serve as a parameter for evaluating static (peripheral or central) vestibular function. STUDY DESIGN: Prosp

Motion sickness induced by otolith stimulation is correlated with otolith-induced eye movements.

This article addresses the relationships between motion sickness (MS) and three-dimensional (3D) ocular responses during otolith stimulation. A group of 19 healthy subjects was tested for motion sickness during a 16 min otolith stimulation induced by off-vertical axis rotation (OVAR) (constant velocity 60 degrees /s, frequency 0.16 Hz). For each subject, the MS induced during the session was quantified, and based on this quantification, the subjects were divided into two groups of less susceptible (MS-), and more susceptible (MS+) subjects. The angular eye velocity induced by the otolith stimu

Otolith-ocular responses in Meniere's patients before and after endolymphatic shunt operation.

Caloric testing in prone/supine position and constant velocity off-vertical axis rotation (OVAR) in yaw axis (rotate-then-tilt paradigm) can evaluate labyrinth function and vestibular-ocular reflex (VOR) behaviour before and after endolymphatic shunt surgery (ESS). Preoperative and postoperative otolith dysfunction can be documented by constant velocity OVAR, before the VOR is modulated by the vestibular compensation. Vestibulo-ocular responses in prone/supine position and linear VOR (lVOR) OVAR responses were observed in 10 patients before and after ESS. Ipsilateral caloric reaction in prone/

Histology of the utricle in kinetotically swimming fish: a parabolic aircraft flight study.

OBJECTIVES: Humans taking part in parabolic aircraft flights (PAFs) may suffer from space motion sickness, which is a form of kinetosis. As it has been repeatedly shown that some fish in a given batch also reveal kinetotic behaviour (especially so-called spinning movements and looping responses) during PAFs, and as a result of the homology of the vestibular apparatus of all vertebrates, fish can be used as model systems to investigate the origin of susceptibility to motion sickness. Therefore. we were prompted to examine the utricular maculae, which are responsible for the internalization of g

Galvanic-evoked myogenic responses in patients with an absence of click-evoked vestibulo-collic reflexes.

OBJECTIVES: To show that galvanic-evoked responses on the sternocleidomastoid muscle (SCM) are useful for differentiating labyrinthine lesions from retro-labyrinthine lesions in patients with an absence of click-evoked vestibulo-collic reflexes. METHODS: We studied the average responses in the unrectified electromyographic (EMG) activities of the SCM to galvanic stimulation (3mA, 1ms). The cathodal electrode was on the mastoid, and the anodal electrode was on the forehead. Twenty-two healthy subjects and 28 patients with vestibular disorders were studied. All of the 28 patients showed the uni

Human otolith-ocular reflexes during off-vertical axis rotation: effect of frequency on tilt-translation ambiguity and motion sickness.

The purpose of this study was to examine how the modulation of tilt and translation otolith-ocular responses during constant velocity off-vertical axis rotation varies as a function of stimulus frequency. Eighteen human subjects were rotated in darkness about their longitudinal axis 30 degrees off-vertical at stimulus frequencies between 0.05 and 0.8 Hz. The modulation of torsion decreased while the modulation of horizontal slow phase velocity (SPV) increased with increasing frequency. It is inferred that the ambiguity of otolith afferent information is greatest in the frequency region where t

Carp experiment in space microgravity--a visual-vestibular sensory conflict model.

In the 8-d flight mission of Spacelab-J (STS-47) conducted in 1992, behavior of the dorsal light response (DLR) and EEG activity of the cerebellum were intermittently examined for two carp, normal and otolith-removed. The latter carp had immobilization trouble caused by twisting of the EEG cable on day 2 inflight. The problem continued for the remainder of the experiment. Analyses made on the normal carp provided additional evidence in fish for sensory-motor disorder and readjustment during early phase of microgravity, thus supporting the sensory conflict hypothesis for space motion sickness.

Prenatal dinocap exposure alters swimming behavior in mice due to complete otolith agenesis in the inner ear.

Exposure to the fungicide dinocap during gestation produces behavioral abnormalities in the house mouse that are not apparent at birth but become obvious at weaning. Pregnant mice (CD-1) were exposed on Days 7 to 16 of gestation to dinocap at 0, 6, 12, or 25 mg/kg/day and the postnatal behavioral development of the offspring was assessed. Torticollis (head-tilting) appears in the treated offspring at 3 weeks of age (4.4% at 12 and 25.3% at 25 mg/kg/day) and, during a test of swimming ability, many of the mice (6.8% at 12 and 47.2% at 25 mg/kg/day) sink below the surface or are unstable and swi

Bone conducted cervical vestibular evoked myogenic potentials: comparison of tone burst stimulus vs chirp stimulus.

PURPOSE: Vestibular evoked myogenic potential (VEMP) is a test used to evaluate the function of otolith organs. In healthy individuals, air-conducted (AC) responses are obtained, whereas, in conductive hearing loss, the inability to transmit the signal well enough with AC stimuli has led to the need for bone-conducted (BC) stimuli. This study aimed to compare 500 Hz Chirp and Tone Burst stimuli in terms of latency and amplitude in BC cVEMP. METHODS: This prospective observational case control study included 35 healthy participants (70 ears) between the age of 20-50. Participants underwent VEM

Contributions of mirror-image hair cell orientation to mouse otolith organ and zebrafish neuromast function.

Otolith organs in the inner ear and neuromasts in the fish lateral-line harbor two populations of hair cells oriented to detect stimuli in opposing directions. The underlying mechanism is highly conserved: the transcription factor EMX2 is regionally expressed in just one hair cell population and acts through the receptor GPR156 to reverse cell orientation relative to the other population. In mouse and zebrafish, loss of Emx2 results in sensory organs that harbor only one hair cell orientation and are not innervated properly. In zebrafish, Emx2 also confers hair cells with reduced mechanosensor

Management of 210 patients with benign paroxysmal positional vertigo: AMC protocol and outcomes.

CONCLUSION: Benign paroxysmal positional vertigo (BPPV) could be treated with specific maneuvers for the involved canals. Management on a daily basis gave earlier relief than weekly treatment, especially in apogeotropic BPPV. Apogeotropic and multi-canal BPPVs were related to the increased numbers of follow-ups and trauma was related to the increased recurrence. OBJECTIVE: We aimed to report the short-term outcomes of our protocol for BPPV and evaluate the factors influencing the results and recurrence. METHODS: In 210 BPPV patients, posterior semicircular canal (PSCC), geotropic and apogeot

Ultrastructural changes in otoconia of osteoporotic rats.

The etiology of benign paroxysmal positional vertigo (BPPV) remains obscure in many cases and women are affected more often than men. A recent prospective study, performed in women >50 years of age suffering from recurrent BPPV, showed associated osteopenia or osteoporosis in a large percentage of these patients. These results suggested the possible relationship between recurrent BPPV and a decreased fixation of calcium in bone in women >50 years. To test this hypothesis, an experimental study was performed in adult female rats. Utricular otoconia of female rats in which osteopenia/osteoporosi
