Leukocoria in infants is always a danger signal as retinoblastoma, a malignant retinal tumor, is responsible for half of the cases in this age group. More common signs should also be considered suspicious until proved otherwise, such as strabismus, the second most frequent sign of retinoblastoma. Less frequent manifestations are inflammatory conditions resistant to treatment, hypopyon, orbital cellulitis, hyphema or heterochromia. Other causal pathologies, including persistent hyperplastic primary vitreous (PHPV), Coats' disease, ocular toxocariasis or retinopathy of prematurity, may also mani

Until now, ultrasound examination of the fetal eyes has not played an important role in prenatal diagnosis. National and international guidelines are generally confined to documentation of the presence of the orbits and the lenses. However, in recent years, with the advent of high-resolution ultrasound technology and increasing knowledge of prenatal medicine and genetics, careful examination of the fetal eye has enabled the detection of many ocular malformations before birth. This article provides an overview of the anatomy related to the development of the fetal eye and covers the following c

We report the fourth example of a patient with germline partial trisomy of 2p21-pter and congenital neuroblastoma. The male infant had a dysmorphic facial expression and presented with congenital heart disease, supernumerary nipples, hypospadias, shawl scrotum, hemilateral persistent hyperplastic primary vitreous, and neuroblastoma. His germline karyotype of 46,XY,der(8)t(2;8)(p21;p23.2) was inherited from a maternal-balanced translocation, which indicates that the proto-oncogene MYCN region of 2p24.3 is tripicated in germline cells. A cytogenetic study of the biopsied tumor cells did not show

Congenital microphthalmia is a rare anomaly of the fetal orbit resulting from developmental defects of the primary optic vesicle. Chromosomal anomalies, genetic defect, infection, and prenatal drug exposure are the most common causes. Congenital microphthalmia is usually associated with other abnormalities, and cases of isolated microphthalmia are rarely reported. Congenital microphthalmia can be diagnosed by prenatal ultrasound by measuring the axial diameter of the eye ball, but the accuracy depends on fetal position and associated anomalies. We report a case of an isolated unilateral microp

BACKGROUND: Familial exudative vitreoretinopathy is a hereditary, bilaterally progressive formation of a vitreoretinal membrane. It usually occurs in full-term newborns without previous treatment with hyperbaric oxygen. In this report, we present six cases of this disease with various abnormalities of the posterior segment diagnosed in two Syrian families related by first degree of consanguinity.
DESIGN: A retrospective family analysis and presentation of cases.
CASE PRESENTATION: Six members of the two consanguineous families, aged between 3.5 and 13 years, who were systemically healthy, pr

The vertebrate basic helix-loop-helix (bHLH) transcription factor ATOH7 (Math5) is specifically expressed in the embryonic neural retina and is required for the genesis of retinal ganglion cells (RGCs) and optic nerves. In Atoh7 mutant mice, the absence of trophic factors secreted by RGCs prevents the development of the intrinsic retinal vasculature and the regression of fetal blood vessels, causing persistent hyperplasia of the primary vitreous (PHPV). We therefore screened patients with hereditary PHPV, as well as bilateral optic nerve aplasia (ONA) or hypoplasia (ONH), for mutations in ATOH

PURPOSE: To study the relationship between retinal and tunica vasculosa lentis (TVL) disease in retinopathy of prematurity (ROP). Although the clinical hallmark of ROP is abnormal retinal blood vessels, the vessels of the anterior segment, including the TVL, are also altered.
METHODS: ROP was induced in Long-Evans pigmented and Sprague Dawley albino rats; room-air-reared (RAR) rats served as controls. Then, fluorescein angiographic images of the TVL and retinal vessels were serially obtained with a scanning laser ophthalmoscope near the height of retinal vascular disease, ~20 days of age, and

We reviewed the records of 32 consecutive children (43 eyes) who underwent lensectomy/vitrectomy between January 1988 and August 1990 at the Medical Center of the University of California at San Francisco, to study the incidence and characteristics of clinically significant postoperative inflammation (CSPI). No eyes of patients 18 months old or younger (22 eyes) developed CSPI; nine of 21 eyes of older patients did develop CSPI. Other ocular abnormalities (microphthalmos, persistent hyperplastic primary vitreous, retinopathy of prematurity, and Axenfeld's syndrome) and systemic syndromes did n

UNLABELLED: Persistent fetal vasculature (PFV) is a human disease that results from failure of the fetal vasculature to regress normally. The regulatory mechanisms responsible for fetal vascular regression remain obscure, as does the underlying cause of regression failure. However, there are a few animal models that mimic the clinical manifestations of human PFV, which can be used to study different aspects of the disease. One such model is the Nuc1 rat model that arose from a spontaneous mutation in the Cryba1 (crystallin, beta 1) gene and exhibits complete failure of the hyaloid vasculature

Primary angle closure (PAC) is rare in pediatric patients, typically associated with persistent hyperplastic vitreous, congenital cataracts, ocular surgery, retinopathy of prematurity, topical or oral medications, and ocular albinism. This case report describes the youngest documented case of unilateral subacute PAC in a seven-year-old female without these underlying conditions. The patient underwent evaluation using A-scan ultrasound and ultrasound biomicroscopy (UBM), revealing a shallow anterior chamber and a short axial length (20.45 ± 0.02 mm in the left eye). The angle closure was caused

Children with fetal alcohol syndrome (FAS) may have impaired vision and various ocular abnormalities. All parts of the eye may be affected and anomalies such as microphthalmus, microcornea, Peters' anomaly, cataract, persistent hyperplastic primary hyaloid vitreous body, coloboma of the iris and choroid, retinal dysplasia and, most commonly, optic nerve hypoplasia and tortuosity of the retinal vessels, have been reported. The periocular facial features consist of short palpebral fissures (included in the criteria for diagnosis of FAS), telecanthus, epicanthus and blepharoptosis. Visual functio

Anophthalmia and microphthalmia are etiologically and clinically heterogeneous. We present a 13-year-old boy with microphthalmia and multiple anomalies who was evaluated as part of our research into the etiology of microphthalmia. His clinical features included left microphthalmia, persistent hyperplastic primary vitreous and posterior coloboma, right posterior pole coloboma, pectus excavatum, mild hypotonia, mild delays in speech and motor development, and an anxiety disorder with social difficulties. Investigations with a chromosome microarray revealed a de novo deletion of chromosome 16p11.

Diseases that can clinically simulate an uveitis are summarised under the name Masquerade syndromes. They constitute a heterogenous group of malignant and non-malignant entities that must be excluded in the differential diagnosis. Of the malignant diseases lymphomas are only rarely observed as the cause of a pseudo-uveitis, whereas especially leukaemic diseases and, in childhood, retinoblastomas and the extremely rare intraocular medulloepithelioma must be excluded. Of the non-malignant diseases in childhood, besides Coats' disease, a juvenile xanthogranuloma as well as persisting hyperplastic

A now-17-year-old girl was reportedly born with no pupil formed in her right eye. In her first week of life, she underwent attempted pupilloplasty at a foreign institution. Surgery was reported to have been complicated and followed by vitreous hemorrhage. Nine months later, the eye was reevaluated at another institution and the presence of primary persistent hyperplastic vitreous noted. Secondary widening of the artificial pupil was performed. She now presents with a slit-shaped pupil and a very dense nuclear cataract (). She now desires removal of the cataract to regain the temporal visual fi

We report five cases of complex microphthalmia with other ocular malformations in infants or children, which were evaluated to investigate the relationship between the corneal diameters and total axial length. The size of the globe was measured by using computerized tomographic scans (CT scan), A-scan ultrasonography, or magnetic resonance imaging (MRI). There is a limited range of well-described malformation, including anterior or posterior segment dysgenesis or combined pathology such as corneal opacity, small cornea, iris hypoplasia, lens dislocation, cataract, chorioretinal coloboma, persi

We describe six cases from three unrelated consanguineous Egyptian families with a novel characteristic brain malformation at the level of the diencephalic-mesencephalic junction. Brain magnetic resonance imaging demonstrated a dysplasia of the diencephalic-mesencephalic junction with a characteristic 'butterfly'-like contour of the midbrain on axial sections. Additional imaging features included variable degrees of supratentorial ventricular dilatation and hypoplasia to complete agenesis of the corpus callosum. Diffusion tensor imaging showed diffuse hypomyelination and lack of an identifiabl
