Association of Preserved Ratio Impaired Spirometry with Arterial Stiffness.

Rationale: Preserved ratio impaired spirometry (PRISm) is a recently recognized spirometric pattern defined by a ratio of forced expiratory volume in 1 second to forced vital capacity of at least 0.70 and a forced expiratory volume in 1 second  <80% of reference. For unclear reasons, PRISm is associated with increased cardiovascular (CV) morbidity and mortality. Arterial stiffness is a major mechanism of CV disease, which can be measured by carotid-femoral pulse-wave velocity (cfPWV). Objectives: We explored the hypothesis that cfPWV would be increased in individuals with PRISm and airflow lim

Arterial Stiffness Is Associated With Cytomegalovirus-Specific Senescent CD8+ T Cells.

BACKGROUND: Arterial stiffness is a well-known predictor of future cardiovascular events. Search for the underlying mechanism of arterial stiffening is still under way. We investigated the relationship between arterial stiffness and cytomegalovirus infection in terms of T-cell senescence. METHODS AND RESULTS: Arterial stiffness was evaluated using pulse wave velocity measurements in 415 Koreans (age 59±12 years). We also investigated the frequency of CD57+ or CD28null senescent T cells in peripheral blood lymphocytes and analyzed which immune parameters were correlated with pulse wave velocit

Average Real Variability Is an Independent Predictor of Increased Arterial Stiffness Assessed by Pulse Wave Velocity.

Blood pressure variability (BPV) is an established cardiovascular risk factor associated with target organ damage beyond mean blood pressure levels. Arterial stiffness, assessed by carotid-femoral pulse wave velocity (cfPWV), represents an early manifestation of hypertension and an independent predictor of cardiovascular events. Among short-term BPV indices, Average Real Variability (ARV) is considered a robust marker of blood pressure fluctuations. This study evaluated the relationship between ARV and arterial stiffness and identified clinically relevant ARV thresholds associated with increas

Blood Pressure Mediates the Association Between Carotid-Femoral Pulse Wave Velocity and Basal Ganglia Enlarged Perivascular Spaces: A Community-Based Cross-Sectional Study.

Although carotid-femoral pulse wave velocity (cf-PWV) is the non-invasive gold standard for assessing aortic stiffness, evidence linking cf-PWV to basal ganglia enlarged perivascular spaces (BG-EPVS) in the general population is limited. We aimed to evaluate this association and quantify the mediating role of blood pressure (BP) in community-based adults. This cross-sectional study included 485 participants from the Kailuan cohort. Cf-PWV was measured by ultrasound, and BG-EPVS were graded on 3.0-Tesla MRI. Logistic regression, restricted cubic spline, and mediation analyses were applied to qu

Stronger Association Between Blood Pressure and Arterial Stiffness in Older Women than Men: The Hordaland Health Study.

INTRODUCTION: Sex differences in the life course development of brachial blood pressure (Bi et al. N Engl J Med, 2024. https://doi.org/10.1056/NEJMoa2412006 ) and the association between brachial and central BP are well documented. AIM: To explore if the associations of brachial and central BP with the presence of increased arterial stiffness differ between older women and men. METHODS: Data from 1135 women and 943 men aged 68 in the Hordaland Health Study were used. Brachial BP was categorised as normal (BP< 130/85 mmHg), high-normal (BP 130-139/85-89 mmHg) or hypertension (BP≥140/90 mmHg).

The Impact of Aortic Pulsatile Hemodynamics on Renal Function Trajectories in Acute Heart Failure.

BACKGROUND: Perturbation of aortic pulsatile hemodynamics and the presence of impaired renal function (IRF) may increase postdischarge events in patients hospitalized for acute heart failure (AHF). The present study investigated whether on-admission aortic pulsatile hemodynamics is associated with IRF and in-hospital changes in renal function, and modulates its impact on postdischarge adverse events in patients with AHF. METHODS: A total of 240 AHF patients (69.3 ± 15.6 years, 76.7% men) were enrolled and followed for outcomes of heart failure hospitalization and death up to 3 years after dis

Risk Factors for Plaque Vulnerability in a Community Population: Endothelial Function and Arterial Stiffness Indexes.

BACKGROUND: Rupture of atherosclerotic plaques and subsequent acute cardiovascular complications remain a major cause of morbidity and mortality throughout the world. Despite recent advances in treatment, we have not yet identified the means to prevent atherosclerotic vascular disease has not been met. It remains a challenge to determine at an early stage whether an atherosclerotic plaque will become unstable and vulnerable. Therefore, this study aim to identify key risk factors associated with the stability and vulnerability of carotid artery plaques, which may offer valuable insights for the

A cross-sectional study of non-invasive markers of vascular health in children and adolescents.

BACKGROUND: Non-invasive methods assessing subclinical atherosclerosis, such as carotid intima-media thickness (IMT) and carotid-femoral pulse wave velocity (cfPWV), are well-described in adults but less so in the paediatric population. Other techniques, such as aortic IMT, may be more appropriate in children. This study aimed to characterise age-related changes to arterial vasculature using best practice methodology for these measures. METHODS: This cross-sectional study involved 97 healthy volunteers aged 2 to 20 (mean age 11.2 years [SD 5.1]). Carotid and aortic IMT were measured using hig

The major determinants of arterial stiffness in Korean patients with rheumatoid arthritis are age and systolic blood pressure, not disease-related factors.

Patients with rheumatoid arthritis are at increased risk of cardiovascular morbidity and mortality. This study was undertaken to investigate the prevalence of peripheral arterial disease, and to identify factors, especially those related to rheumatoid arthritis, influencing arterial stiffness in Korean patients with rheumatoid arthritis. A total of 262 patients with rheumatoid arthritis managed in a tertiary clinic were included. Ankle-brachial index and brachial-ankle pulse wave velocity were measured. Rheumatoid arthritis-related factors were determined, as well as the traditional cardiovasc

[Effects of local vibration exposure on the finger blood flow (author's transl)].

The purpose of the present study was to clarify the physiological response of peripheral blood vessels when some vibrational stimuli were given to five healthy young men (21-22) continuously and intermittently. The vasoreaction was observed on each subjects after the exposure to vibration at frequencies 18, 32, 64 and 125 Hz with acceleration level at 5 g (peak value). In the pulse wave height of left and right arms, a specified tendency was not found while a larger variation was observed. After continuous exposures, pulse wave propagation velocity at the left arm showed a decreasing tendency

Reduced fractional anisotropy in patients with major depressive disorder and associations with vascular stiffness.

Previous studies revealed several alterations of the cerebral white matter in patients with major depressive disorder. However, it is unknown if these alterations are associated with vascular changes in the brain and other body parts. We compared diffusion tensor imaging derived fractional anisotropy in a well characterized sample of middle-aged patients with major depressive disorder (n = 290) and never-depressed controls (n = 346) by the method of tract-based spatial statistics. Subsequently, the potential role of pulse wave velocity as a mediator of depression- and age-related changes in ex

Comprehensive assessment of vascular health in patients; towards endothelium-guided therapy.

Endothelial function has diagnostic, prognostic and therapeutic significance. A number of non-invasive techniques were introduced for its assessment, including flow-mediated dilation (FMD), finger plethysmography (RH-PAT) and digital thermal monitoring (DTM). All these methods can be performed simultaneously. In addition, various methods for measuring arterial wall stiffness are available such as: pulse wave analysis (PWA), pulse wave velocity (PWV), pulse contour analysis (PCA) and carotid wall distensibility coefficient (DC). Finally, carotid intima-media thickness (cIMT) and ankle brachial

Association of obstructive sleep apnea with arterial stiffness and nondipping blood pressure in patients with hypertension.

Whether sex influences the association of obstructive sleep apnea (OSA) with markers of cardiovascular risk in patients with hypertension is unknown. In this study, 95 hypertensive participants underwent carotid-femoral pulse wave velocity, 24-hour ambulatory blood pressure monitoring, echocardiogram, and polysomnography after a 30-day standardized treatment with hydrochlorothiazide plus enalapril or losartan. OSA was present in 52 patients. Compared with non-OSA patients, pulse wave velocity values were higher in the OSA group (men: 11.1±2.2 vs 12.7±2.4 m/s, P=.04; women: 11.8±2.4 vs 13.2±2.2

Intrinsic PPG-ECG Coupling for Accurate and Low-Power Blood Pressure Monitoring.

Long-term, continuous blood pressure (BP) monitoring is of great clinical importance for the early diagnosis and dynamic assessment of cardiovascular diseases. Conventional BP monitoring methods based on pulse transit time (PTT) typically rely on at least two independent signal channels. However, multi-signal processing introduces synchronization errors and inaccuracies in fiducial point detection, thereby limiting the accuracy and efficiency of BP estimation. In addition, recording and processing multiple signals increases circuit complexity and elevates system power consumption. To overcome

Commentary on the Youth Vascular Consortium's 'Expert consensus documents'.

Two major articles in the Journal review childhood arterial function, one the literature, the other methodology complemented by another elsewhere, with a different method focus and presents reference values. Studies of pulse wave velocity (PWV) measured in young children followed into adulthood are just emerging. Like blood pressure (BP), PWV, the index of stiffening, tracks as it rises over time. The carotid arteries are useful indicators, but the vital measurement pathway is from the heart to the descending, distensible aorta's bifurcation. Doppler methods do that well but are too slow to de

Effect of metformin on central arterial stiffness in individuals with type 2 diabetes mellitus: a systematic review and meta-analysis.

Type 2 diabetes mellitus (T2DM) is a global health problem linked to hyperglycemia and cardiovascular complications, including central arterial stiffness. Metformin, the first-line therapy for T2DM, has been suggested to provide vascular protection, although evidence on arterial stiffness is inconsistent. This study investigated the effect of metformin on central arterial stiffness through systematic searches of PubMed/MEDLINE, ScienceDirect, Cochrane, and Google Scholar. Three reviewers independently performed data extraction and quality assessment, and pooled analyses were conducted in RevMa

Arterial stiffness in gestational diabetes: Latest insights.

Increased arterial stiffness is a known cardiovascular risk factor, associated with hypertension and acute coronary events. Gestational diabetes (GDM) is associated with the development of placental-mediated disorders and future cardiovascular morbidity, raising the possibility of an association with increased arterial stiffness (AS). Several studies have now investigated this association through the assessment of pulse wave velocity and augmentation index. In the current review, we present the latest evidence regarding the changes in arterial stiffness in pregnancies complicated by GDM, befor

An algorithm to detect dicrotic notch in arterial blood pressure and photoplethysmography waveforms using the iterative envelope mean method.

BACKGROUND AND OBJECTIVE: Detection of the dicrotic notch (DN) within a cardiac cycle is essential for assessment of cardiac output, calculation of pulse wave velocity, estimation of left ventricular ejection time, and supporting feature-based machine learning models for noninvasive blood pressure estimation, and hypotension, or hypertension prediction. In this study, we present a new algorithm based on the iterative envelope mean (IEM) method to detect automatically the DN in arterial blood pressure (ABP) and photoplethysmography (PPG) waveforms. METHODS: The algorithm was evaluated on both

Age-associated increases in middle cerebral artery pulsatility differ between men and women.

Mechanisms underlying sex differences in brain aging remain unclear but may relate to changes in cerebral pulsatile blood flow. Sex differences in the stiffening of the large arteries and expansion of pulse pressure with age may accelerate changes in pulsatile (i.e., discontinuous) blood flow in the brain that contribute to brain health. The purpose of this cross-sectional, secondary analysis was to examine sex differences in age-associated changes in large artery (aorta and carotid) stiffness, carotid pulse pressure, and cerebral pulsatility in 206 men and 217 women between 18 and 72 yr of ag
