Investigating the impact of structural racism on black birthing people - associations between racialized economic segregation, incarceration inequality, and severe maternal morbidity.

Black birthing people are twice as likely to experience severe maternal morbidity (SMM) as their white counterparts. Structural racism provides a framework for understanding root causes of perinatal health disparities. Our objective was to investigate associations between measures of structural racism and severe maternal morbidity (SMM) among Black birthing people in the US. We linked delivery hospitalizations for Black birthing people in the National Inpatient Sample (2008-2011) with data from the American Community Survey 5-year estimates and the Vera Institute of Justice Incarceration Trend

Bridging Public Health and Health Care for Systems-Level Change: The NYC Coalition to End Racism in Clinical Algorithms, 2021-2023.

The use of race-based clinical algorithms contributes to population-level racial health inequities. To address these inequities, the New York City Department of Health and Mental Hygiene convened 10 health systems in the Coalition to End Racism in Clinical Algorithms (CERCA). Our mixed-methods evaluation showed that health systems implemented algorithm change and provider education and that CERCA members perceived the coalition as a catalyst for systems-level change. CERCA is a replicable model for collaboration between public health and medicine to advance health equity and antiracist policy.

Online racism, online mental health engagement, depression, and suicide risk among Asian American and Pacific Islander and Black American emerging adults.

INTRODUCTION: Recent studies suggest that exposure to racist online interactions and content may be associated with depressive symptoms and suicide risk among racially minoritized individuals. Despite the hate and violence easily encountered online, individuals use social media platforms to engage with their mental health concerns and seek support. Hence, the current study aimed to explore whether online mental health engagement would buffer the depressive symptoms and suicide risk associated with online racism among Black and Asian American Pacific Islander (AAPI) emerging adults (18-29) who

Emotion Regulation Within Context: Understanding Buffering Effects on Chronic Pain and PTSD Among Black Women Who've Experienced Sexual Assault and Racism.

Previous research has found that both racism and sexual assault are related to posttraumatic stress disorder (PTSD) symptoms and chronic pain. Black women have unique relationships to these stressors situated within contexts of oppression, and little is known about factors that may exacerbate these associations among Black women. Among Black women experiencing both racism and sexual assault (n = 148), emotion clarity moderated the relation between PTSD and chronic pain, β = .0126, SE = 0.0059, p < .05. Our findings support the importance of considering the place that psychological constructs l

HIV-related 'conspiracy beliefs': lived experiences of racism and socio-economic exclusion among people living with HIV in New York City.

HIV-related 'conspiracy beliefs' include ideas about the genocidal origin of HIV and the nature and purpose of HIV-related medications. These ideas have been widely documented as affecting myriad health behaviours and outcomes, including birth control use and HIV testing. Most HIV-related research has quantitatively explored this phenomenon, and further qualitative research is necessary to better understand the complexity of these beliefs as articulated by those who endorse them. Moreover, public health in general has over-emphasised the role of the Tuskegee Syphilis Study in explaining mistru

A conditional indirect effect model of racism and depressive and anxiety symptoms by ethnicity among Asian Americans during the COVID-19 pandemic.

The COVID-19 pandemic contributed to the rise in anti-Asian hate crimes, leading to adverse mental and physical health outcomes in Asian American communities and a growing need to better understand the detrimental effects of racism. This study aims to understand whether the indirect relationship between facing discrimination and depressive and anxiety symptoms through perceived danger is conditional on Asian ethnicity. We used data from the Asian American & Native Hawaiian/Pacific Islander COVID-19 Needs Assessment Project (n = 3071), employing multivariable linear regression models and path a

How systemic racism results in poorer outcomes for First Nations, and what First Nations are doing about it: the example of kidney health.

BACKGROUND: End-stage kidney disease continues to disproportionally impact the lives of First Nations peoples. Systemic racism is a key determinant, and manifests as differential access to determinants of health (housing, employment, access to care) and differential care. This paper discusses how different models of primary healthcare operating in rural and remote Manitoba communities results in different outcomes for patients identified as being at risk of kidney disease. METHODS: This study is a partnership between researchers from the First Nations Health and Social Secretariat of Manitoba

"This Work Belongs to Everyone": Understanding Health Equity Practitioners' Approaches to Addressing Structural Racism in the Context of Racial Health Equity Practice.

Structural racism-which encompasses discriminatory laws/policies, institutional practices, and social norms-is a driver of racialized health inequities in the U.S. Notably, laws/policies at the federal, state, and local levels perpetuate structural racism across all societal domains and are key structural determinants of racial health equity. Health equity practitioners play a critical role in mitigating the impacts of structural racism-related laws/policies through their legal, policy, and community-based work at state and local health departments, community-based organizations, and health po

On the compounding manifestations of racism shaping the US HIV/AIDS epidemic: why ending the HIV epidemic must address these factors for success.

OBJECTIVE: Growing racial/ethnic inequities in healthcare access and racially segregated sexual mixing contribute to persistent disparities in HIV incidence in the US. We aim to examine the extent to which eliminating racial/ethnic inequities in healthcare access could reduce disparities in HIV incidence and its interaction with assortative sexual mixing. DESIGN: A mathematical model. METHODS: We used two independently developed HIV transmission models to estimate HIV incidence among Black, Hispanic/Latino, and White/Other MSM and the corresponding incidence rate ratios (IRRs) comparing Blac

Ending the HIV epidemic for all, not just some: structural racism as a fundamental but overlooked social-structural determinant of the US HIV epidemic.

PURPOSE OF REVIEW: We review the recent theoretical and empirical literature on structural racism, social determinants of health frameworks within the context of HIV prevention and treatment, and criticism of the national responses to the US epidemic. RECENT FINDINGS: In line with growing mainstream attention to the role of structural racism and health inequities, recent editorials and studies cite ending structural racism as an essential step to ending the US HIV epidemic. Recent studies demonstrate that barriers rooted in structural racism such as incarceration, housing instability, police

Understanding the link between racial discrimination and traumatic stress symptoms among Black youth: The role of co-rumination and racism-related vigilance.

OBJECTIVE: Racial discrimination represents a pernicious source of trauma for Black youth that can critically hinder mental health. This cross-sectional study examined whether racism-related co-rumination and racism-related vigilance function as distinct and sequential pathways linking racial discrimination to traumatic stress symptoms (TSS) among a sample of Black adolescents. METHOD: Data were drawn from a national sample of 195 Black adolescents (Mage = 14.52; 49% female) who completed questionnaires assessing racial discrimination, racism-related co-rumination, racism-related vigilance, a

Health equity follows racial equity: learning the impact of historic racism through a summer reading assignment in a graduate public health course.

Prior to a fall semester graduate course in public health, students are encouraged to complete a summer reading assignment based on a book chosen from a curated list of popular literature relevant to public health concepts. From 2020 to 2024, this assignment enabled students to explore the history of structural and systemic racism in the United States and its impacts on current public health policy and programs. Themes from these books are woven into class discussions and further explored in subsequent course assignments. This paper describes the assignment and shares responses from the 1,141

Parental Racism-Related Stress and Child Depression: Disparities Across Income and Race/Ethnicity in a National Sample of Racial/Ethnic Minority Families.

BACKGROUND: Vicariously experienced racism-related stress is a key social determinant of child depression, yet national data on racial/ethnic and income disparities in this relationship are limited. OBJECTIVES: This study tested whether non-Hispanic Black (NHB) children and children from lower income households experience more frequent depression linked to parent-experienced racism-related stress than other groups. METHODS: In the 2023 National Health Interview Survey (NHIS), 1712 parents of children from minoritized racial/ethnic groups aged 5-17 years (weighted N = 37.7 million) reported t

Online Racial/Ethnic Discrimination, Suicidal Ideation, and Alcohol Misuse Among Ethnoracially Minoritized College Students: The Roles of Internalized Racism and Ethnic Identity.

Over the past decade, mental health problems have disproportionately increased among U.S. ethnoracially minoritized college students, though explanations for this disparity remain understudied. In the present study, we examined the direct and indirect associations between online racial/ethnic discrimination, suicidal ideation (SI), and alcohol misuse. We also investigated whether internalized racism helps explain the association between online racial/ethnic discrimination (i.e., individual or vicarious exposures) and SI and alcohol misuse. Lastly, we examined the moderating role of ethnic iden

Impact of online and offline racism on cigarette smoking, marijuana use, and vaping via depressive/anxiety symptoms among racially minoritized emerging adults.

Studies have found that offline and online racism are linked to mental health issues including depressive and anxiety symptoms among racially minoritized emerging adults. To cope with these symptoms, racially minoritized emerging adults may resort to substance use. Thus, we examined the relationship between offline and online racism and recent smoking behaviors (past 30 days), and whether depressive and anxiety symptoms (DAS) mediate this relationship. We hypothesized that offline and online racism would be associated with a greater probability of recent smoking through depressive/anxiety symp

Preliminary Evidence for the Sequentially Mediated Effect of Racism-Related Stress on Pain Sensitivity Through Sleep Disturbance and Corticolimbic Opioid Receptor Function.

Sleep disturbance predicts worse pain outcomes. Because sleep disturbance inequitably impacts Black adults - with racism as the upstream cause - understanding how racism-related stress impacts pain through sleep might help minimize racialized pain inequities. This preliminary study examined sequential mediation of the effect of racism-related stress on experimental pain through sleep disturbance and corticolimbic μOR function in pain-free non-Hispanic Black (NHB) and White (NHW) adults. Participants completed questionnaires, actigraphy, positron emission tomography, and sensory testing. We rep

"I Think It's Institutional Racism:" Pediatric Social Workers' Explanations of Racial Disparities in Kidney Transplant Access for Black Children in the United States.

BACKGROUND: Black children in the US with ESKD experience higher mortality, lower transplant rates, and longer time on dialysis than other children. Prior research on health disparities in adult kidney patients often emphasizes patient-level psychosocial factors, yet little is known about how providers understand these inequities for children. This study explored pediatric dialysis and transplant social workers' explanations for racial disparities affecting US black children in kidney transplant access. METHODS: Semi-structured interviews were conducted with 16 pediatric dialysis and transpla

An Ethical Framework for Addressing the Differential Impact of Systemic Racism and Inequities on Type 1 and Type 2 Diabetes Mellitus in Youth in the United States.

BACKGROUND: Inequities in the clinical care and health outcomes of youth and young adults (YYAs) with type 1 diabetes mellitus (T1D) and type 2 diabetes mellitus (T2D) are well-established. Systemic and institutional racism and barriers, as well as implicit biases underlie these inequities in the USA. SUMMARY: This article offers a broad overview and analysis of disparities in clinical care and outcomes among YYAs with T1D and T2D, framed within an ethical context. We argue that achieving ethical care requires centering assessments of patient and family needs within the realities of their liv

Development and Integration of Community Advisory Boards to Inform Research on the Impact of Structural Racism on Racial/Ethnic Disparities in End-Stage Kidney Disease.

INTRODUCTION: Community Advisory Boards (CABs) represent a critical methodological approach to center research on the experiences of patients and families and minimise power inequities between 'researcher' and 'subject'. This approach can be especially important to inform work related to structural racism and racial and ethnic disparities in healthcare. Herein, we report the value and challenges of integrating CABs into ongoing research on structural racism and racial and ethnic disparities in care for adults with end-stage kidney disease as well as strategies to maximise impact. METHODOLOGY:
