The Influence of Piezoelectric Transducer Stimulating Sites on the Performance of Implantable Middle Ear Hearing Devices: A Numerical Analysis.

To overcome the inherent deficiencies of hearing aids, implantable middle ear hearing devices (IMEHDs) have emerged as a new treatment for hearing loss. However, clinical results show that the IMEHD performance varies with its transducer's stimulating site. To numerically analyze the influence of the piezoelectric transducer's stimulating sites on its hearing compensation performance, we constructed a human ear finite element model and confirmed its validity. Based on this finite element model, the displacement stimulation, which simulates the piezoelectric transducer's stimulation, was applie

A novel multipurpose modular mini-endoscope for otology.

Transtympanic access to the round window membrane (RWM) for drug delivery is in the focus of otology and has stimulated development of various endoscopes. These endoscopes are tasked to enable best visualization at a low diameter and to offer a working channel for various instruments. The specific aspect of sterilization is a major issue especially in regard to the diameter of the endoscope with its integrated working channel. We evaluated a new multi-purpose modular semi-rigid optical fiber endoscope (10,000 pixel resolution) for minimal invasive middle ear endoscopy focusing on access to the

The role of the window-damping test in preoperative decision-making for semicircular canal dehiscence syndrome.

PURPOSE: To evaluate the utility of a novel window-damping test (WDT), which involves a simple intratympanic injection of fibrin to dampen the movement of the round window membrane and stapedial footplate, in predicting surgical outcomes for round window reinforcement (RWR) in patients with superior semicircular canal dehiscence syndrome (SSCDS). MATERIALS AND METHODS: This retrospective cohort study included 8 patients with clinically suspected SSCDS. Patients underwent pure-tone audiometry, tympanometry, vestibular-evoked myogenic potential (VEMP) testing, and high-resolution CT scans with

A self-mixing laser-diode interferometer for measuring basilar membrane vibrations without opening the cochlea.

A laser-diode forms the basis of a displacement sensitive homodyne interferometer suitable for measurements from poorly reflective surfaces. The compact and cost-effective interferometer utilizes the self-mixing effect when laser light reflected from a moving target re-enters the laser cavity and causes phase dependent changes of the lasing intensity. A piezo positioner was used to displace the interferometer with known frequency and amplitude as a basis for real-time calibration of the interferometer's sensitivity. The signal-processing algorithm is described that allows measurements in prese

Steerable Robot-assisted Micromanipulation in the Middle Ear: Preliminary Feasibility Evaluation.

HYPOTHESIS: The use of a robotic manipulator with a dexterously orientable gripper will expand the ability of middle ear surgeons to perform precise tasks and access otherwise challenging anatomic regions. BACKGROUND: Middle ear surgery presents unique challenges because of the constrained operative space and limited access to certain anatomic regions. METHODS: A custom-designed robot with a sideways-reaching gripper was used to evaluate feasibility of manipulation tasks in different middle-ear anatomical zones. Reachable workspace within the middle ear, accuracy of free-space path following

Micro CT visualization of silver nanoparticles in the middle and inner ear of rat and transportation pathway after transtympanic injection.

BACKGROUND: Silver nanoparticles (Ag NPs) displayed strong activities in anti-bacterial, anti-viral, and anti-fungal studies and were reportedly efficient in treating otitis media. Information on distribution of AgNPs in different compartments of the ear is lacking. OBJECTIVE: To detect distribution of Ag NPs in the middle and inner ear and transportation pathways after transtympanic injection. METHODS: Contrast effect of Ag NPs in the micro CT imaging was assessed in a phantom. AgNPs at various concentrations (1.85 mM, 37.1 mM, and 370.7 mM) were administered to rat middle ear using transty

Experimental herpes simplex virus and cytomegalovirus labyrinthitis.

Herpes simplex virus (HSV) and guinea pig cytomegalovirus (GPCMV) were inoculated into the middle ear of guinea pigs. The routes of passage of middle ear infection to the inner ear are hematogenous and/or via the cochlear windows. The presence of GPCMV inclusion body-bearing cells within the cochlea strongly suggests the hematogenous route, as in some animals the inner ear does not show cell infiltration and precipitate. When viral infection approaches the cochlear windows, the first morphological changes observed are the presence of precipitate in the inner ear fluids and the appearance of ma

The transport of radioactive labelled sodium ions at the round window. Corroboration of the mechanism of "Arslan's operation" for morbus Menière.

Radioactive-labelled sodium does not penetrate via the round window in the inner ear of guinea-pigs. The findings reported confirm the mechanism assumed by Arslan for his "osmotic induction" surgery in the treatment of morbus Menière.

Feasibility of Round Window Stimulation by a Novel Electromagnetic Microactuator.

INTRODUCTION: Most implantable hearing aids currently available were developed to compensate the sensorineural hearing loss by driving middle ear structures (e.g., the ossicles). These devices are successfully used in round window (RW) stimulation clinically, although this was initially not the intended use. Here, a novel microactuator, specifically designed for RW stimulation, was tested in human temporal bones to determine actuator performance and applicability. METHODS: Stapes footplate response to RW stimulation was determined experimentally in human temporal bones and the obtained sound

Obliteration of the ductus reuniens.

The ductus reuniens was successfully obliterated in 52 guinea pig ears. Histopathological study showed that a majority of these specimens demonstrated cochlear hydrops, saccular collapse and normal utricle. These results support the theory of longitudinal flow of endolymph from the cochlea toward the endolymphatic sac via the ductus reuniens and saccule. A major source of endolymph in the saccule appears to be the scala media. In another set of 11 animals in which the ductus reuniens was first obstructed and two months later the endolymphatic duct was blocked, endolymphatic hydrops was shown i

Streptomycin perfusion of the labyrinth through the round window plus intravenous streptomycin.

Streptomycin perfusion of the labyrinth through the round window membrane in the middle ear is a simple, safe, and effective way to stop the dizzy spells, fullness, and tinnitus of Ménière's disease without making the hearing worse. My results with 24 patients in the last 8 months are presented. Although the long-term effect of this operation remains to be seen, my experience with streptomycin perfusion of the labyrinth through the lateral semicircular canal leads me to believe these good results will be maintained in the future. Although the caloric response is not usually eliminated complete

Transcanal endoscopic ear surgery for perilymphatic fistula after electric acoustic stimulation.

Transcanal endoscopic ear surgery (TEES) will become a very useful therapeutic option. A perilymphatic fistula (PLF) is defined as sudden sensorineural hearing loss and/or vertigo caused by leakage of the perilymph through a fistula from the oval window and/or round window. We report a case of PLF after electric acoustic stimulation (EAS), a kind of cochlear implant, successfully treated by TEES. A 38-year-old man presented to our hospital with vertigo and hearing loss (HL). His vertigo was induced by Valsalva maneuvers. Eight months ago, he underwent EAS for his right ear for congenital senso

Inner ear hearing loss in acute and chronic otitis media.

Temporary or permanent threshold shift of bone conduction occurs frequently in acute or chronic otitis media. The sensorineural hearing loss is dependent on the age of the patient and the duration of the illness. Fluid in the middle ear may impede sound transmission and oxygen transport to the inner ear. In middle ear inflammation noxious substances may pass across the round window membrane leading to serous labyrinthitis. In therapy ototoxic drugs and operations (tympanoplasty) can cause sensorineural deafness.

[Round window membrane pathology in sudden deafness (author's transl)].

Exploration of the middle ear was performed on 15 patients with sudden deafness that had failed to improve after intravenous infusion of Eupaverin and Rheomacrodex. Three patients had abnormalities of the round window membrane, in 2 there was a perilymphatic fistula and in 1 an abnormal bulging of the membrane with no transmission of perilymphatic fluid movement. These 3 patients presented with a history of exertion or stress prior to the onset of symptoms. Surgical closure ofthe 2 fistulae led to improvement of hearing, and perilymphatic decompression in the third patient improved the tinnitu

Histopathological Patterns of Otosclerosis Progression: Exploring Otic Capsule and Round Window Involvement.

OBJECTIVES: Obliteration of the round window (RW) in cases of otosclerosis presents a significant clinical challenge due to its association with more severe hearing loss and a poorer prognosis for functional recovery after stapes surgery. The objective is to assess and characterize the occurrence of RW involvement in otosclerosis cases and to identify patterns of disease progression that may indicate a potential for RW obliteration. METHODS: We selected archival temporal bones from donors with otosclerosis. We evaluated the degree of RW obliteration using a semi-quantitative scale and the loc

Three-dimensional quantification of fibrosis and ossification after cochlear implantation via virtual re-sectioning: Potential implications for residual hearing.

Hearing preservation may be achieved initially in the majority of patients after cochlear implantation, however, a significant proportion of these patients experience delayed hearing loss months or years later. A prior histological report in a case of delayed hearing loss suggested a potential cochlear mechanical origin of this hearing loss due to tissue fibrosis, and older case series highlight the frequent findings of post-implantation fibrosis and neoosteogenesis though without a focus on the impact on residual hearing. Here we present the largest series (N = 20) of 3-dimensionally reconstr

The effect of systemic lipoic acid on hearing preservation after cochlear implantation via the round window approach: A guinea pig model.

The present study aimed to evaluate the effects of systemic lipoic acid on hearing preservation after cochlear implantation. Twelve Dunkin-Hartley guinea pigs were randomly divided into two groups: the control group and the lipoic acid group. Animals in the lipoic acid group received lipoic acid intraperitoneally for 4 weeks. A sterilised silicone electrode-dummy was inserted through the round window to a depth of approximately 5 mm. The hearing level was measured using auditory brainstem responses (ABRs) prior to electrode-dummy insertion, and at 4 days and 1, 2, 3 and 4 weeks after electrode
