New pharmacotherapies for alcohol dependence.

Two pharmacotherapies recently introduced in Australia, acamprosate and naltrexone, provide a major advance in the treatment of severe alcohol dependence, a common condition leading to a considerable burden of illness and major costs to the community. Acamprosate and naltrexone reduce alcohol intake, and increase the likelihood and prolong the duration of abstinence (Level I evidence). Compared with naltrexone, the benefits of acamprosate have been confirmed in a larger number of studies involving larger numbers of patients with longer durations of follow-up. Unlike naltrexone, acamprosate app

[Evaluation of drug treatment of primary alcoholism].

Clinical improvement (abstinence and sobriety) can be obtained in 30% of alcohol dependent (primary) patients at one year when therapy consists of meetings and talking with physicians. The association of drugs led to 40% clinical improvement in controlled studies. These drugs are acamprosate (Aotal), which is well tolerated and shown to be beneficial at one year, naltrexone (Revia), also well tolerated with a follow-up of 3 months, and lithium (Téralithe), to be used more cautiously, with a follow-up of one year. Disulfirame (Espéral), is poorly effective in its usual oral prescription, but is

Effects of postpartum psychiatric illnesses on family planning.

OBJECTIVE: We investigated the relationship between postpartum psychiatric episodes and subsequent family planning. Our hypothesis was that women who had a postpartum illness would plan to have fewer children. METHOD: We conducted a mail survey of members of the self-help group Depression After Delivery (DAD). The membership was asked about changes in family planning after a postpartum illness. Two groups were defined: women who took action to prevent further pregnancies after the illness (CHANGE) and women who did not take action to prevent future pregnancies (NO CHANGE). RESULTS: Among res

Providing psychological support for patients after critical illness.

The majority of patients have little or no memory of their stay in ICU or remember only pain, suctioning or lack of sleep. Dreams and nightmares while in the intensive care unit (ICU) and after discharge home have also been reported. The few studies investigating the longer-term psychological problems of critical illness point to a picture of social isolation with patients avoiding company and showing less affection to their partners. Our own experience, running a special outpatient clinic and following up patients by post, showed a picture of depression, anxiety, irritability and social isola

Pathological gambling.

According to DSM-III criteria, pathological gambling is now recognised as a mental illness. Epidemiological data suggest that the incidence of this disorder in the general population varies from 0.5% to 1%. However, until recently, psychiatrists and clinical psychologists have tended to neglect the problem because of a lack of understanding of its aetiology and management. This paper describes pathological gambling and presents an overview of current psychiatric treatments. It concludes that behavioural interventions are the most effective treatments for compulsive gamblers.

Strategic Withdrawal as Empowerment for Individuals With Depression in Internet Support Groups: Qualitative Study.

BACKGROUND: Internet support groups (ISGs) provide accessible peer support for individuals with depression; yet, their withdrawal rates are substantially higher than those observed in general online groups. Existing research has focused primarily on retention as a marker of intervention success. The significance and mechanisms of withdrawal, therefore, remain underexplored. OBJECTIVE: This study aimed to examine how individuals with depression use withdrawal from ISGs as a strategy for empowerment. It also sought to clarify how autonomy, courage, and responsibility are enacted through the pro

Fostering self-advocacy development in service-based self-advocacy groups.

BACKGROUND: While the importance of self-advocacy for those with an intellectual disability is well recognised, little is known about how organisations can support self-advocacy development by implementing self-advocacy groups. METHOD: This inclusive research study collected empirical data from fourteen self-advocates, two managers, three group supporters, and one advocacy coordinator, all experienced and involved in an established not-for-profit service-based self-advocacy group. RESULTS: Our findings identified several benefits of being involved in a self-advocacy group, including confiden

Exploring Unmet Needs from an Online Metastatic Breast Cancer Support Group: A Qualitative Study.

Background and Objective: Despite the increasing treatment options for patients with metastatic breast cancer (MBC), unmet needs remain common, especially in low and middle-income countries where resources are limited and MBC patients face many challenges. They often join support groups to cope with their unmet needs. Currently, many MBC patients connect with each other via online support group in view of the constant availability of support and rapid information exchange. The objective of this study is to determine the unmet needs of women with MBC from an online support group. Material and M

Considering the value of online support groups for colorectal conditions: perspectives from caregivers and adult patients.

PURPOSE: To evaluate the benefits of participating in an online support group for caregivers of children with a colorectal condition or adult patients with a similar condition. METHODS: An electronic survey was administered to members of an international online support group (18 items for caregivers; 15 for patients). Items included demographics, medical diagnosis, potential benefits, and overall experiences in the group. Quantitative results were summarized as descriptive trends, while qualitative responses were summarized thematically. RESULTS: Respondents (102 caregivers, 6 patients) were

Modern technology to support carers of care recipients with dementia or functional mental illness: promising progress, but a long road ahead.

There is no doubt that family carers who look after a family member with dementia or with a functional mental illness fulfill an important role, not only for their loved one, but also for the health and aged care systems of the countries they live in. Due to increasing life expectancy, but also improved healthcare the number of family carers supporting older care recipients with functional mental illness or dementia is on the rise. While the carer role often can offer rewarding experiences caregivers are at increased risk of stress, depression, sleep problems, and often experience poor health

Grief, traumatic stress, and posttraumatic growth in women who have experienced pregnancy loss.

OBJECTIVE: Pregnancy loss is common and can be devastating for those who experience it. However, a historical focus on negative outcomes, and grief in particular, has rendered an incomplete portrait of both the gravity of the loss, and the potential for growth in its wake. Consistent with contemporary models of growth following bereavement, this study explored the occurrence of posttraumatic growth following pregnancy loss and further assessed the role of core belief disruptions and common loss context factors across perinatal grief, posttraumatic stress symptoms, and posttraumatic growth. Development and evaluation of a telehealth videoconferenced support group for rural spouses of individuals diagnosed with atypical early-onset dementias.

Atypical and early-onset dementias can be particularly problematic for family caregivers, and support groups aimed at memory loss and Alzheimer's disease are not always helpful. Unfortunately, little has been developed specifically for caregivers of individuals with atypical dementias such as the frontotemporal dementias. Compounding the lack of access to interventions targeted specifically at caregivers of individuals with atypical and early-onset dementias are the unique needs of rural caregivers. Due to the relative infrequency of these particular dementias and the large geographical distan

Considerations for affirming gender nonconforming boys and their families: new approaches, new challenges.

The media and the public's reaction have created the impression that gender variant behaviors of many children are indicative of later transsexual identities. This has suggested to some parents that the best way to manage their sons' gender variance and perhaps gender dysphoria is to allow them to dress as girls in increasingly more situations. For some this becomes a transition to living full-time as a girl. However, the author believes that most of these boys would be better served by helping them develop an awareness of the many ways of being a boy, stigma-reducing social strategies, and fa

Does your older adult client have a gambling problem?

Pathological gambling is becoming an increasing problem in today's culture, particularly because opportunities and inducements to gamble abound. This article describes symptoms, consequences, and comorbidities experienced by pathological gamblers. Gambling pathology is often invisible because pathological gamblers seldom disclose gambling problems to their primary care provider. Thus, nurses will benefit from knowledge of the physical and emotional signs of the disorder. A two-question assessment tool, the Lie/Bet Questionnaire, is useful in helping nurses identify pathological gamblers. Inter

Concreteness training reduces dysphoria: proof-of-principle for repeated cognitive bias modification in depression.

A tendency toward abstract and overgeneral processing is a cognitive bias hypothesized to causally contribute to symptoms of depression. This hypothesis predicts that training dysphoric individuals to become more concrete and specific in their thinking would reduce depressive symptoms. To test this prediction, 60 participants with dysphoria were randomly allocated either to (a) concreteness training; (b) bogus concreteness training, matched with concreteness training for treatment rationale, experimenter contact, and treatment duration but without active engagement in concrete thinking; (c) a

Cancer survivorship: advocacy organizations and support systems.

When oncology evolved into a specialized field of medicine more than four decades ago, the primary goals of most cancer treatment included the extension of patients' life expectancies and the occasional hope for cure. Physicians were seen as the principal and solitary advocate for patients, and information regarding cancer diagnosis, treatment, and side effects was delivered or screened by a doctor. Patient education materials were scarce, formalized support systems were nonexistent, and the future was often difficult to define. Patient advocacy has since expanded to models of self, organizati

Faecal incontinence in adults. 2: Treatment and management.

The first article in this two-part series (Vol 5(22): 1366-74) discussed the prevalence, aetiology and investigation of faecal incontinence in adults. The second article summarizes the main treatment options available for faecal incontinence. Surgery is the most widely available intervention, and offers a reasonable chance of success for appropriately selected individuals. Drug therapy is currently limited to constipating agents, which can be very useful for people with internal anal sphincter disruption. The potential role of biofeedback is promising and warrants further research. Management

Helping them cope.

1. Self-help groups for individuals with sarcoidosis (a noncontagious multisystem disorder characterized by granulomas in many organs) are rare despite the physical and emotional problems associated with this disease. 2. The stimulus for the development of the group arose when staff in a pulmonary clinic identified that sarcoidosis patients were helping each other cope with problems and concerns related to their disease and feelings of isolation. 3. Nurses play a significant role in creating vehicles of help and service to facilitate and support patients experiencing fear and anxiety about the
