Intrathecal anaesthesia for the elderly patient: the influence of the induction position on perioperative haemodynamic stability and patient comfort.

Ninety elderly (>65 y) patients were studied to assess the influence of patient position during induction of spinal anaesthesia on the incidence of perioperative hypotension and haemodynamic stability. Prior to induction of anaesthesia, Lactated Ringer's solution (8-10 ml/kg) was administered. In the Sitting Group, intrathecal anaesthesia was performed with the patient in the sitting position. In the Lateral Group, patients assumed the lateral decubitus position. In all cases hyperbaric bupivacaine (10 mg) was administered using a 25 gauge Quincke spinal needle. Patients were placed in the sup

Stand up urgency: is this symptom related to a urethral mechanism?

OBJECTIVE: To study the role of women's position as a stimulus of urgency, and specifically the change of position, i.e. to stand up form a lying or sitting position. Thus, we compared clinical and urodynamics characteristics among women with overactive bladder syndrome (OAB) depending of the position which could trigger urgency. METHODS: Thirty-eight females with complaints of urgency, without urinary infection, neurological or urological diseases, were prospectively enrolled. Patients completed a study-specific questionnaire asking about urgency and urge incontinence when in three different

What is the variability in urodynamic parameters with position change in children? Analysis of a prospectively enrolled cohort.

PURPOSE: Insufficient data are available regarding the effect of patient position on urodynamic parameters and diagnoses in children. We report on a cohort of pediatric patients, comparing the results of serial urodynamic studies to determine the variability between the supine and sitting positions. MATERIALS AND METHODS: Children referred for urodynamic evaluation were prospectively enrolled based on the ability to tolerate position change. Consecutive studies in supine and sitting positions were performed during a single session by the same nursing team. Volume infused and infusion rate did

Position dependent changes of the cerebral venous drainage--implications for the imaging of the cervical spine.

PURPOSE: This study demonstrates the physiological changes of the cerebral venous outflow routes in healthy humans in the recumbent and the sitting position employing positional MRI. METHODS: In five volunteers, the internal jugular veins and the cervical vertebral plexus were analyzed in the supine and sitting position using an open MR system. Axial T2-weighted scans and axial T1-weighted flow sensitive gradient echo sequences were acquired. The findings were compared to previously published anatomic descriptions from cadaver preparations. RESULTS: In the supine position, the internal jugul

Positional changes of oropharyngeal structures due to gravity in the upright and supine positions.

OBJECTIVES: To compare the responses of oropharyngeal structures to gravity while sitting upright or lying down in a supine position. METHODS: Seven subjects were evaluated by cone beam CT (CBCT) while in the upright position and by a four-row multidetector helical CT (MDCT) while in the supine position. All of the voxel sizes were adjusted to be 0.3x0.3x0.3 mm3 in the x-y-z axis. The posterior nasal spine, basion and fourth cervical bone were used as references to measure positional changes in the oropharyngeal structures between the upright and supine positions. The smallest areas in the or

Surgical repair of platypnea-orthodeoxia syndrome caused by patent foramen ovale, ascending aortic dilation, and pectus excavatum: a case report.

BACKGROUND: Platypnea-orthodeoxia syndrome (POS) is a relatively uncommon clinical syndrome characterized by dyspnea and deoxygenation accompanying postural changes, such as sitting or standing from a recumbent position. CASE PRESENTATION: We describe the case of a 76-year-old woman who was hospitalized with dyspnea in the sitting position, which was relieved in the right lateral decubitus position. The patient was diagnosed with POS caused by a patent foramen ovale (PFO), dilation and elongation of the ascending aorta, and pectus excavatum. We performed PFO closure and replacement of the asc

Intracardiac platypnea-orthodeoxia syndrome diagnosed by forward-bending and abdominal compression.

Intracardiac platypnea-orthodeoxia syndrome (POS) is a pathological condition that leads to a reduction in oxygenation in the sitting position. The detection rate of patent foramen ovale that causes POS increases with Valsalva manoeuvre and cough. We report the case of a 92-year-old woman with POS, which could not be diagnosed in the sitting position. The Valsalva manoeuvre could not be performed because of dementia, and POS was diagnosed on the forward-bending position and abdominal compression. It was assumed that forward-bending increases the right atrial pressure by compressing the right v

Does Degenerative Lumbar Spine Disease Influence Femoroacetabular Flexion in Patients Undergoing Total Hip Arthroplasty?

BACKGROUND: Sitting pelvic tilt dictates the proximity of the rim of the acetabulum to the proximal femur and, therefore, the risk of impingement in patients undergoing total hip arthroplasty (THA). Sitting position is achieved through a combination of lumbar spine segmental motions and/or femoroacetabular articular motion in the lumbar-pelvic-femoral complex. Multilevel degenerative disc disease (DDD) may limit spine flexion and therefore increase femoroacetabular flexion in patients having THAs, but this has not been well characterized. Therefore, we measured standing and sitting lumbar-pelv

Dynamic Lateral Semisitting Position for Supracerebellar Approaches: Technical Note and Case Series.

BACKGROUND: It has always been a matter of debate which position is ideal for the supracerebellar approach. The risk of venous air embolism (VAE) is the major deterrent for surgeons and anesthesiologists, despite the fact that sitting and semisitting positions are commonly used in these operations. OBJECTIVE: To demonstrate a reduction on the risk of VAE and tension pneumocephalus throughout the operation period while taking advantages of the semisitting position. METHODS: In this study, 11 patients with various diagnoses were operated in our department using the supracerebellar approach in

Evaluating the biomechanics of an in-between posture to create a multi-posture office environment.

BACKGROUND: Prolonged sitting during work is common and has been shown to cause health issues. However, changing working postures has been reported to reduce musculoskeletal issues and impact other health issues; thus, there is a need for an office environment with multiple choices of working postures. OBJECTIVE: The purpose of this study was to evaluate changes in body position, body loading, and blood perfusion while in a seated, standing, and new office seating position, termed the in-between position. METHODS: Ground reaction forces, joint angles, pelvic tilt, openness angle (angle betwe

Does spinopelvic motion change after total hip arthroplasty?

PURPOSE: Spinopelvic motion plays an important role in functional acetabular cup position after total hip arthroplasty (THA). Sacral slope (SS) has been a useful surrogate for spinopelvic motion. The present study aimed to investigate statistical characteristics of spinopelvic motion before and after THA using changes in SS in supine, standing, and sitting positions. METHODS: A total of 76 patients (88 hips) were assessed. To classify spinopelvic mobility, defined as a change in SS from standing to sitting position (ΔSSstand/sit), 10° ≤ ΔSSstand/sit ≤ 30°, ΔSSstand/sit < 10°, and ΔSSstand/sit

Effects of postural change on oxygen saturation and respiration in patients after the Fontan operation: platypnea and orthodeoxia.

The aim of this study was to assess whether platypnea and orthodeoxia occur in Fontan patients. We divided 14 Fontan patients into 2 groups: 8 patients who had pulmonary arteriovenous fistulas and/or intra-atrial shunts (group A) and 6 patients who had neither pulmonary arteriovenous fistulas nor intra-atrial shunts (group B). They were compared with 9 controls (group C). Arterial oxygen saturation, minute ventilation per body weight and ventilatory equivalent for carbon dioxide were measured in the supine and then sitting positions. In group A, 1 patient had platypnea and 3 patients had ortho

Effects of posture on right-to-left shunt detection by contrast transcranial doppler.

BACKGROUND AND PURPOSE: There is controversy about the optimal patient position for the detection of right-to-left shunt (RLS). The study was performed to investigate which patient position best detects RLS during contrast-enhanced transcranial Doppler. METHODS: We prospectively evaluated consecutive patients with ischemic stroke or TIA referred to our Noninvasive Cerebrovascular Laboratory for suspected paradoxical embolism. The standard protocol for RLS detection recommended by the International Consensus Criteria was followed. Each patient was examined at rest and after Valsalva maneuver i

Evolving base of support associated multicontact stability analysis reveals distinct sit-to-stand motor control pattern in stroke survivors.

Sit-to-stand (STS) transition is a prerequisite for independent mobility and a primary benchmark for post-stroke functional recovery. However, conventional event-based measures fail to continuously resolve weight transfer, and the classical Extrapolated center of mass (XcoM) and margin of stability (MoS) framework cannot represent evolving multicontact support, vertical acceleration dynamics, or whole-body angular momentum, thereby obscuring phase-specific motor control strategies. To address this problem, we proposed a continuous biomechanical framework for analyzing multicontact STS dynamics

Estimation of human spine orientation with inertial measurement units (IMU) at low sampling rate: How low can we go?

Studying people in their daily life is important for understanding conditions with multi-faceted aetiology such as chronic low back pain. Inertial measurement units can be used to reconstruct the posture and motion of the body outside of laboratories to enable this research. The battery life of these sensors strongly affects the usability of the system, since recharging them frequently is inconvenient and can lead to additional errors. A major determinant of the battery life for these sensors is sampling rate, but the relationship between sampling rate and accuracy in motion reconstruction is

Evaluation of rest tremor in different positions in Parkinson's disease and essential tremor plus.

BACKGROUND: Rest tremor (RT) can be observed in several positions (seated, standing, lying down) but it is unknown whether the tremor features may vary across them. This study aimed to compare the RT electrophysiological features across different positions in tremor-dominant Parkinson's disease (PD) and essential tremor plus (ET with RT, rET). METHODS: We consecutively enrolled 90 tremor-dominant PD and 24 rET patients. The RT presence was evaluated in three positions: with the patient seated, the arm flexed at 90°, the forearm supported against gravity, and the hand hanging down from the cha
