Diagnostic performance of T-SPOT.TB for extrapulmonary tuberculosis according to the site of infection.

BACKGROUND: The clinical manifestations of extrapulmonary tuberculosis (E-TB) vary according to site of disease, so we tested the hypothesis that IFN-γ producing T-cell responses also vary in parallel. Therefore we conducted a prospective, blinded, observational study to evaluate the diagnostic performance of blood T-SPOT.TB according to the various sites of E-TB. METHODS: From April 2008 to August 2010, all patients with suspected E-TB were enrolled at a tertiary hospital in an intermediate TB-burden country. Final diagnosis in patients with suspected E-TB was classified by clinical category

HIV-associated extrapulmonary tuberculosis in Thailand: epidemiology and risk factors for death.

BACKGROUND: We conducted a prospective, multicenter observational cohort study in Thailand to characterize the epidemiology of extrapulmonary tuberculosis (TB) in HIV-infected persons and to identify risk factors for death. METHODS: From May 2005 to September 2006, we enrolled, interviewed, examined, and performed laboratory tests on HIV-infected adult TB patients and followed them from TB treatment initiation until the end of TB treatment. We conducted multivariate proportional hazards analysis to identify factors associated with death. RESULTS: Of the 769 patients, pulmonary TB only was di

[Specific features of DNA isolation for polymerase chain reaction in extrapulmonary tuberculosis].

While testing samples by the polymerase chain reaction (PCR), a procedure for DNA isolation is advisable adapted for different types of materials; thorough DNA purification is of the greatest importance in testing blood-containing samples, deep sample lysis is of particular importance for osseous tissue samples. A differential approach to isolating DNA from clinical samples allows the efficiency of PCR to be enhanced.

Extrapulmonary tuberculosis in an immigrant society: clinical and demographic aspects of 92 cases.

Extrapulmonary tuberculosis accounted for 33% of all new cases of tuberculosis identified at the Soroka Medical Center in Beer Sheva, Israel, during a 10-year period. The most common types of extrapulmonary infection diagnosed were genitourinary tuberculosis (54% of patients), lymphadenitis (13%), pleural tuberculosis (9%), and tuberculosis of bones and joints (8%). Of 92 patients, 51% were Jews of Ethiopian origin, 29% were Jews of non-Ethiopian origin, and 20% were Bedouins. Thus, extrapulmonary tuberculosis remains a significant problem for Israel's heterogeneous population.

Quadriplegia Following Prevertebral/Retropharyngeal Collection Secondary to Extrapulmonary Spinal Tuberculosis (Pott's Disease).

Tuberculous retropharyngeal abscess, though rare, poses significant diagnostic and therapeutic challenges due to its atypical presentation. We present the case of a 51-year-old male with a history of drug abuse and inhalational burn injury presented with generalized weakness, sensory deficits, and neurological symptoms. Despite initial negative investigations, subsequent MRI and microbiological studies confirmed a rare case of tuberculous retropharyngeal abscess. The patient underwent urgent drainage and anti-tubercular therapy, experiencing complications such as Candida infection that require

[Extrapulmonary tuberculosis: case of a 70-year-old woman suffering from cutaneous lymph node, renal and bone tuberculosis ].

We have described a case of a 70-year-old woman, treated for a pancreas cancer, which was diagnosed for many months because of atypical skin lesions, lymphadenopathy and progressing cachexia. Despite many autoimmunological, serological, histopathological, bacteriological and mycological examinations there was no diagnosis. Finally the case of extrapulmonary tuberculosis involving skin, lymph nodes, kidneys and bones was diagnosed. After the introduction of antituberculotic therapy we are observing some clinical improvement and regression of symptoms.

Antiretroviral treatment does not prevent extrapulmonary tuberculosis during SIV/Mtb coinfection in macaques.

Coinfection with both HIV and M. tuberculosis (Mtb) results in disseminated tuberculosis (TB) and accelerated HIV progression. Despite greater access to antiretroviral treatment (ART), it remains unclear whether suppression of HIV replication protects against severe Mtb infection. Here, using a macaque model of SIV/Mtb coinfection, we investigated whether treatment of SIV infection with ART influenced control of a subsequent Mtb challenge compared with SIV-infected macaques that were not treated with ART. Macaques were first infected with SIVB670, SIVB670 with ART, or saline followed by a low-

18F-FDG PET/CT Incidental Finding of Rare Extrapulmonary Presentation of Mycobacterium tuberculosis.

A 41-year-old woman presented with a left breast mass for which mammography was performed. Mammogram revealed a suspicious BI-RADS 4, palpable left breast mass with associated nipple retraction. As malignancy was suspected, the patient underwent breast MRI and 18F-FDG PET/CT for staging. Multiple ultrasound guided biopsies were then performed of the left breast, left axillary lymph nodes, and left cervical lymph nodes. Final pathology was negative for malignancy, but final cultures grew Mycobacterium tuberculosis (Tb). The patient was initially intolerant to frontline medications, but eventual

Disseminated Extrapulmonary Tuberculosis in an Immunocompetent Individual Without Pulmonary Involvement: A Case Report.

Tuberculosis (TB) is a major global health burden, particularly in developing countries like India. While the most common presentation is pulmonary TB, extrapulmonary TB involving other body systems can also occur, posing diagnostic challenges. We present the case of a 24-year-old immunocompetent man from India who exhibited an uncommon and complex presentation of disseminated extrapulmonary TB. The patient had an asymptomatic brain cavitated lesion, likely tuberculoma, cervical lymphadenopathy, a small subcutaneous collection in the neck, a destructive lytic lesion in the sacrum, and a subcut

Evaluation of treatment outcome in extrapulmonary tuberculosis: A mixed method study from Puducherry.

BACKGROUND: Extrapulmonary tuberculosis (EPTB) is also a public health problem and involves other parts of the body other than lung. During the quarterly review meeting of State Task Force (STF), Puducherry the members raised concern about the outcome of patients referred from the medical colleges. Even though we receive 60 % feedback of initiation of treatment from the PHI but the treatment outcome of EPTB is not clear. Hence primary agenda of this research was to identify the gap in adherence to diagnostic criteria and treatment. AIM: To assess the methods adopted for diagnosis of EPTB and

Beyond the norm: Primary multidrug-resistant extrapulmonary tuberculosis unveiled in a case series.

Tuberculosis is a preventable and generally curable infectious disease caused by Mycobacterium tuberculosis. It mostly affects the lungs causing pulmonary tuberculosis; however, it may also involve non-pulmonary organs resulting in extrapulmonary tuberculosis (EPTB). Diagnosis of tuberculosis was based on the constitutional symptoms, organ-specific radiographs, and biological specimen examination. However, diagnosis of extrapulmonary tuberculosis can be difficult when the lungs are not affected and constitutional signs and symptoms of tuberculosis that can help to identify the disease are abse

Ultrasononography in Managing Extrapulmonary Tuberculosis: A Randomized, Controlled, Parallel, Superiority, Open-Label Trial.

BACKGROUND: Patients with suspected extrapulmonary tuberculosis are often treated empirically. We hypothesized that extended focused assessment with sonography for human immunodeficiency virus (HIV) and tuberculosis (eFASH), in combination with other tests, would increase the proportion of correctly managed patients with suspected extrapulmonary tuberculosis. METHODS: This trial in adults with suspected extrapulmonary tuberculosis was performed in a rural and an urban hospital in Tanzania. Participants were randomized 1:1 to intervention or routine care, stratified by site and HIV status. All

Leukotriene A4 hydrolase (LTA4H rs17525495) gene polymorphisms and paradoxical reactions in extrapulmonary tuberculosis.

Paradoxical reactions (PRs) are poorly studied complex immunological phenomena, among patients with tuberculosis (TB). When PRs involves critical structures like the central nervous system (CNS), immunomodulatory therapy is often required. Predictors for PRs in TB to pre-empt appropriate treatment strategies in high-risk groups are lacking. TT genotype of Leukotriene A4 hydrolase (LTA4H) promoter region rs17525495 polymorphisms are associated with exaggerated immune responses in Tuberculous meningitis (TBM), the most severe form of extrapulmonary tuberculosis (EPTB). The association of these p

Complex Case Management of Mycobacterium Tuberculosis Extrapulmonary Manifestation to the Right Sacroiliac Joint.

While Mycobacterium tuberculosis is a common bacterial pathogen that infects the respiratory system, especially in endemic regions, it may uncommonly manifest in other organ systems, such as the nervous, gastrointestinal, or musculoskeletal systems. Sacroiliac joint infections are rare, and only 1%-5% of all infections are tuberculous in nature. Given nonspecific inflammatory signs in both laboratory and radiologic examinations, early identification of the causative agent can be difficult. In this report, we present the case of a 29-year-old Eritrean woman who presented with an uncommon extrap

Examination of respiratory specimens improves microbiological diagnosis of patients with presumptive extrapulmonary tuberculosis.

OBJECTIVES: Bacteriological confirmation of extrapulmonary tuberculosis (EPTB) is challenging for several reasons: the paucibacillary nature of the sample; scarce resources, mainly in middle and low-income countries; the need for hospitalization; and unfavorable outcomes. We evaluated the diagnostic role of respiratory specimen examination prospectively in a cohort of patients with presumptive EPTB. METHODS: From July 2018 to January 2019, in a tuberculosis (TB)/HIV reference hospital, a cohort of 157 patients with presumed EPTB was evaluated. Xpert® MTB/RIF Ultra or a culture-positive result

Immune Reconstitution Inflammatory Syndrome Occurring in a Kidney Transplant Patient with Extrapulmonary Tuberculosis.

Tuberculosis (TB) occurring in solid organ transplantation (SOT) is associated with significant morbidity and mortality usually due to delays in diagnosis, drug toxicity encountered with antimycobacterial therapy, and drug-drug interactions. TB in SOT patients may mimic other infectious and noninfectious posttransplant complications such as posttransplant lymphoproliferative disorder (PTLD) and systemic cytomegalovirus infection. Immune reconstitution inflammatory syndrome (IRIS) is a host response resulting in paradoxical worsening of an infectious disease which occurs after the employment of

Tuberculosis in Turkish patients with systemic lupus erythematosus: increased frequency of extrapulmonary localization.

The objective was to investigate the frequency and characteristics of tuberculosis (TB) in patients with systemic lupus erythematosus (SLE). We reviewed the charts of 556 patients with SLE who were followed up between 1978 and 2001 in our lupus clinic. Patients who developed TB after the diagnosis of SLE were identified (SLE/TB+). Ninety-six consecutive patients with SLE who did not develop TB during the follow-up were evaluated as a control group (SLE/TB-). Clinical, laboratory and management characteristics of these two groups of patients were recorded according to a predefined protocol, and

Extrapulmonary tuberculosis--a simple diagnosis? A retrospective study at Dar es Salaam, Tanzania.

Data from the national tuberculosis programme show that extrapulmonary tuberculosis is increasing rapidly in Tanzania, most likely caused by the human immunodeficiency virus (HIV) epidemic. Retrospective data from 271 patients admitted to Muhimbili Medical Centre (MMC), Dar es Salaam between January 1, 1987 and December 31, 1988 with the diagnosis of extrapulmonary tuberculosis reveal that in only 18% of the cases a bacteriological or histological confirmation of the diagnosis had been made. This figure is only 3% when the tuberculous lymphadenitis cases are excluded. The most common diagnosis
