Chronic venous insufficiency in heart failure: exploring a reciprocal influence on cardiovascular health.

Chronic venous insufficiency (CVI) is a prevalent disorder, arising from venous valve incompetence and vein wall weakness, which impairs blood return and leads to venous stasis and hypertension in the lower extremities. This condition, affecting up to 40% of older adults, has been primarily considered a peripheral issue. However, growing evidence indicates its systemic impacts, notably its contribution to cardiovascular dysfunction and heart failure (HF). CVI exacerbates central venous pressure and cardiac preload, placing strain on the right heart and predisposing at-risk patients to HF. More

Plethysmography: A game changer for chronic venous insufficiency diagnosis, treatment, and follow-up.

OBJECTIVE: Chronic Venous Insufficiency (CVI) presents a various clinical symptoms and treatment options. Although it is generally known which treatment option is more appropriate in which situations in CVI, it is even more difficult to find the right option in some uncertain situations. In this study, we aimed to evaluate the potential contribution of Digital Photoplethysmography (D-PPG) in the diagnosis of CVI and especially in the selection of treatment options. PATIENTS AND METHOD: This retrospective study was conducted at Bursa Heart and Arrhythmia Hospital, involving 721 consecutive pat

[Peculiarities of anticoagulant therapy for venous thrombosis and chronic renal insufficiency (literature review)].

The choice of antithrombotic therapy for treatment of patients with venous thromboses and renal dysfunction supposes a special approach due to increased risk of both hemorrhagic and thrombotic events. Therapeutic decision-making is difficult because of concomitant renal dysfunction. Improved quality of life and prognosis in elderly patients with chronic renal insufficiency largely depend on adequate anticoagulant therapy with modern drugs. Presented herein are the findings of several studies in patients with deep vein thrombosis/pulmonary embolism, receiving therapy including that with rivarox

A nutraceutical formulation combined with sclerofoam-assisted laser treatment ameliorates chronic venous insufficiency.

Chronic venous insufficiency has a high impact on the healthcare system due to its high incidence worldwide. We performed a study in 30 women with thigh and leg varices due to major saphenous vein valve incontinence with saphenous trunk reflux causing phlebo-lymphoedema to assess the efficacy of sclerofoam-assisted laser treatment combined with nutraceutical administration. The patients underwent endovascular combination sealing of the saphenous trunk with sclerofoam-assisted laser treatment technique into the major saphenous veins under low-volume tumescent anesthesia followed by intraoperati

Early Results of Popliteal Vein External Banding for Treatment of Chronic Venous Insufficiency.

BACKGROUND: Chronic venous insufficiency (CVI) encompasses a myriad of clinical manifestations including lower extremity swelling and pain, ulcerations and chronic skin changes such as stasis dermatitis, and lipodermatosclerosis. CVI effects greater than 25 million Americans and has a significant socioeconomic and psychosocial impact. Treatment of CVI varies depending on the etiology. For those patients with deep venous reflux, restoration of the deep venous valvular system is critical. Popliteal vein external banding is a novel technique to treat deep venous reflux. Our study aims to retrospe

Imaging findings of pelvic venous insufficiency in patients with postural orthostatic tachycardia syndrome.

OBJECTIVES: Some patients with postural orthostatic tachycardia syndrome (POTS) demonstrate improved dysautonomic symptoms following treatment for pelvic venous insufficiency (PVI). This study assessed the prevalence of significant left common iliac vein (LCIV) compression in POTS patients. METHODS: Radiologists retrospectively reviewed CT images of pelvic veins for 216 women (191 with POTS and 25 age-comparable controls).Quantitative vascular analysis identified percent-diameter compression of the LCIV by the right common iliac artery. Significant LCIV compression was defined as >50%. RESUL

Human trial using the novel bioprosthetic VenoValve in patients with chronic venous insufficiency.

BACKGROUND: Chronic venous insufficiency (CVI) is a disease characterized by failure of the venous valves that allow blood from the lower limbs to return to the heart. The more severe types of CVI (C4b-C6 disease) significantly influence patients' quality of life (QOL). The treatment options for CVI of the deep system are limited. The present study is the first-in-human study using a prosthetic venous valve, VenoValve, to initially evaluate patient outcomes. We evaluated the feasibility, initial safety, and performance outcomes of the VenoValve. METHODS: Ten patients with C5-C6 CVI of the dee

Extraluminal lipoma with common femoral vein obstruction: a cause of chronic venous insufficiency.

A 49-year-old man, with a misdiagnosis of common femoral vein deep vein thrombosis presented with advanced chronic venous insufficiency. Further imaging revealed a patent common femoral vein with augmentation that was compressed by an extrinsic mass. Exploration identified a lipoma that was extravascular and was resulting in venous outflow obstruction. Excision of the lipoma resulted in clinical improvement and ulcer healing.

Leg ulceration in chronic venous insufficiency caused by an absent inferior vena cava.

We report here the case of a 55-year-old Japanese man with a one-year history of multiple ulcers on the left crural region. He had had pronounced varicose veins on both legs and the abdominal region for 35 years. Computed tomography images of the thoracic and abdominal regions showed the absence of an inferior vena cava, with pronounced dilatation of paravertebral venous plexus, cutaneous and azygous veins. Conservative topical treatments led to complete healing of the ulcers in one month. An absent inferior vena cava is an uncommon abnormality, often complicated by cardiac and other visceral

[Remote outcomes of endoscopic dissection of crural perforating veins in chronic venous insufficiency].

We studied the long-term results following endoscopic dissection of crural perforating veins (EDCPV) in chronic venous insufficiency. The study comprised a total of 36 patients (41 lower limbs) who had endured the intervention concerned. The term having passed from the time of the operation till the moment of assessing the obtained outcomes varied from I to 10 years. Ultrasonographic angioscanning within the zone of the previous intervention revealed 38 perforating veins on 3.1 of the postoperative lower limbs (75.6 %). Amongst the veins revealed, most frequently encountered were Cockett's per

Micronized purified flavonoid fraction and the treatment of chronic venous insufficiency: microcirculatory mechanisms.

The standard treatments for venous diseases of the lower limb include compression bandaging and stockings as well as surgical removal of varicose veins. There is a number of conditions in which these conventional treatments are ineffective, particularly in the management of leg ulceration. Drug treatments for healing venous leg ulcers have yet to be developed to the stage of good clinical efficacy, but may assist in the management of patients. Flavonoid drugs have been widely used in the management of the symptoms of venous disease for many years and have recently been studied in some detail t

Investigation of chronic venous insufficiency: A consensus statement (France, March 5-9, 1997).

This consensus document provides an up-to-date account of the various methods available for the investigation of chronic venous insufficiency of the lower limbs (CVI), with an outline of their history, usefulness, and limitations. CVI is characterized by symptoms or signs produced by venous hypertension as a result of structural or functional abnormalities of veins. The most frequent causes of CVI are primary abnormalities of the venous wall and the valves and secondary changes due to previous venous thrombosis that can lead to reflux, obstruction, or both. Because the history and clinical exa

From Tissue to Simulation: A Review of Venous Valve Biomechanics, Extracellular Matrix Architecture, and Constitutive Modeling in Chronic Venous Insufficiency.

Chronic venous insufficiency (CVI) affects an estimated 25-40% of the adult population worldwide, yet the tissue-level mechanics driving venous valve failure remain largely uncharacterized. This review synthesizes experimental and computational literature on venous valve biomechanics as an integrated pipeline spanning biaxial mechanical testing, extracellular matrix (ECM) microstructural characterization, constitutive model development, and fluid-structure interaction (FSI) simulation. Our primary experimental source is biaxial data from bovine jugular and saphenous venous tissues, representin

Early Endovenous Thermal Ablation With Concomitant Anticoagulation in Chronic Venous Insufficiency Complicated by Superficial Venous Thrombosis: A Retrospective Observational Study.

INTRODUCTION: Whether endovenous thermal ablation (EVTA) can be performed soon after the diagnosis of superficial venous thrombosis (SVT) in patients with chronic venous insufficiency (CVI) remains uncertain. This study assessed the safety and clinical benefit of early EVTA performed under anticoagulation. METHOD: Consecutive CVI patients with acute SVT (symptom onset ≤14 days) who underwent endovenous laser ablation (EVLA), radiofrequency ablation (RFA), or endovenous microwave ablation (EMA) between January 2022 and March 2025 were identified. All received peri-procedural anticoagulation. C

Heel-Rise Test for functional assessment in chronic venous insufficiency: a validity study.

BACKGROUND: Chronic venous insufficiency (CVI) affects the superficial and/or deep venous system and is often associated with valve abnormalities and calf pump dysfunction, including reduced lower limb strength. Assessment of calf muscle strength should be part of the clinical routine for these patients. The Heel-Rise Test (HRT) has been proposed for this purpose, but its correlation with other functional instruments remains unclear. OBJECTIVES: To verify the validity of the HRT for assessment of calf muscle strength and endurance in patients with CVI by comparing it with other functional tes
