Generalization-a key challenge for responsible AI in patient-facing clinical applications.

Generalization – the ability of AI systems to apply and/or extrapolate their knowledge to new data which might differ from the original training data – is a major challenge for the effective and responsible implementation of human-centric AI applications. Current debate in bioethics proposes selective prediction as a solution. Here we explore data-based reasons for generalization challenges and look at how selective predictions might be implemented technically, focusing on clinical AI applications in real-world healthcare settings.

Position statement of the Brazilian Society of Nephrology on the refusal and discontinuation of dialysis.

Renal failure is considered a life-limiting CONDITION that often requires Renal Replacement Therapy, such as dialysis or kidney transplantation. Dialysis can effectively relieve symptoms and prolong life, but its withdrawal results in severe complications and death. The decision to discontinue or refuse to dialysis must be made collaboratively by the patient, family, and healthcare team, considering the clinical condition, life expectancy, symptom burden, and individual preferences. This decision, involving clinical, bioethical, and legal aspects, is complex and requires a collective understan

Getting nano tattoos right - a checklist of legal and ethical hurdles for an emerging nanomedical technology.

UNLABELLED: The nano tattoo represents a nascent technology designed to be implanted in the skin to provide continuous and reliable glucose detection for diabetics. Its potential benefits are compelling not only for its ability to prevent diabetic complications and decrease related social costs, but also for its ease of use and relative patient-user comfort. This Note aims to articulate a checklist of fundamental intellectual property, bioethical and system design issues that are appropriately considered in the pre-clinical, pre-commercialization phase of nano tattoo development. Prenatal diagnosis as a tool and support for eugenics: myth or reality in contemporary French society?

Today, French public debate and bioethics research reflect an ongoing controversy about eugenics. The field of reproductive medicine is often targeted as pre-implantation genetic diagnosis (PGD), prenatal diagnosis, and prenatal detection are accused of drifting towards eugenics or being driven by eugenics considerations. This article aims at understanding why the charge against eugenics came at the forefront of the ethical debate. Above all, it aims at showing that the charge against prenatal diagnosis is groundless. The point of view presented in this article has been elaborated jointly by a

Caring for People with Disabilities: An Ethics of Respect.

Eva Feder Kittay's Learning from My Daughter: The Value and Care of Disabled Minds is poised to make a major contribution to the disability literature and is likely to spark controversy among disability scholars. The book's central contribution is the articulation of an ethics of care for meeting the "genuine needs" and "legitimate wants" of people with disabilities or chronic illnesses. We applaud Kittay, who is the mother of a woman with cerebral palsy who has multiple physical and intellectual impairments, for sharing her story in such an eloquent, accessible, and personal manner. The quest

[Ethical reflections on xenotransplantation].

The success of transplantation and the increasing demand for donor organs have led to an increasingly acute shortage of human organs and tissue for transplantation. In this situation xenotransplantation (cross-species transplantation from animals to humans) is discussed as a possible alternative to allotransplantation. Therefore the animal plays an important role in the discussions on xenotransplantation. Currently, a lot of research is being done on xenotransplantation. Cross-species transplantation involves many biological and medical problems and risks which may affect not only the individu

Critically Exploring Vulnerability in Frail Older Adults Following a Fall.

Vulnerability is widely invoked in healthcare, bioethics, and nursing, yet it remains poorly defined because universal human susceptibility, situational exposure to harm, and clinical frailty are frequently treated as interchangeable. This conceptual ambiguity is particularly significant following a fall, when older adults may be understood predominantly through physical risk, functional decline, and the need for protection. Through an interpretative analysis drawing on nursing philosophy, feminist bioethics, phenomenological accounts of embodiment, and nursing ethics, this paper critically ex

Moral conflicts among patients-caregivers dyads in oncological care pathway: a systematic review of the ethical literature.

PURPOSE: Cancer patients are most often accompanied by at least one caregiver along the oncological care path. Since cancer has been defined as a "family disease", patients may rely on caregivers to take medical decisions. In some cases, they arrive at shared decisions, and in other cases, they experience some conflict, with negative implications on the care process. No systematic collection of "moral conflicts", i.e., conflicts pertaining to ethically related issues, occurring among patients and their caregivers in cancer care path is available in current bioethics literature. METHODS: Using

The care of the actively dying in an academic medical center: a survey of registered nurses' professional capability and comfort.

BACKGROUND: Care of the dying is a significant component of nursing practice particularly in hospitals. Nurses who work in certain areas like oncology, intensive care unit (ICU) face the care of the dying, more so than other units. OBJECTIVES: The survey was conducted to assess nurses' self-perception of their professional capability and comfort in the care of the actively dying. Determine if professional capability and comfort was associated with any of the six demographics characteristics (age, gender, clinical experience, education level, nursing unit, continuing education). Unresolved legal and ethical issues in research of adults with severe traumatic brain injury: analysis of an ongoing protocol.

This paper synthesizes federal and state laws and bioethics literature with observations from an ongoing research protocol to identify, define, and clarify the unresolved legal and ethical issues regarding research involving adults with traumatic brain injury (TBI). Solutions that protect rights and minimize unnecessary impediments to valuable clinical and scientific inquiry are also illustrated using the same protocol. Research was performed at intensive care, inpatient rehabilitation, and long-term acute chronic hospitals. Our research protocol identified five areas of law impacting adults w

A Path Not Taken: Beecher, Brain Death, and the Aims of Medicine.

It has been fifty years since a report by an ad hoc committee of Harvard Medical School ushered in the widespread adoption of brain death as a definition of death. Yet brain death remains disputed as an acceptable definition within bioethics. The continuous debate among bioethicists has had three key recurring features: first and foremost, argument over alleged flaws in the conceptual logic and consistency of the "whole-brain" approach as a description of the meaning of death; second, efforts to fix perceived limitations of brain death-based practices to optimize transplantation, especially gi

Locked in, Left Behind: Clinical, Psychosocial, and Ethical Dimensions of Prison Health-A Narrative Review.

The global prison population has expanded over recent decades, straining correctional systems and positioning detention facilities as critical, yet frequently overlooked, sites of health inequity. With an estimated 11.5 million people currently incarcerated worldwide, prisons have become pivotal settings for examining the convergence of clinical vulnerability, psychosocial adversity, and human rights concerns. The aim of this narrative review was to examine how clinical, psychosocial, and bioethical factors interact to shape the health of incarcerated populations and to address four questions:

"Go Ask Alice": The Case for Researching Schedule I Drugs.

The available treatments for disorders affecting large segments of the population are often costly, complex, and only marginally effective, and many have numerous side effects. These disorders include dementias, debilitating neurological disorders, the multiple types of drug addiction, and the spectrum of mental health disorders.Preliminary studies have shown that a variety of psychedelic and similar U.S. Drug Enforcement Administration Schedule I drugs may offer better treatment options than those that currently exist and pose potentially the same or even less risk than do legal psychoactive

Percutaneous endoscopic gastrostomy (PEG), in elderly patients with dementia and anorexia. Medical and ethical issues regarding placement.

BACKGROUND AND AIMS: Ethical and medico-legal issues reviews of Percutaneous Endoscopic Gastrostomy (PEG) placement in elderly patients is an important topic of international medical literature. PEG is often inappropriately placed in patients with not spontaneous oral feeding intake, cause of unrealistic expectations. We performed a critical review of the literature for placement of PEG in geriatric patients. METHODS: A literature review was performed about the positioning of the PEG in geriatric patients with dementia and severe anorexia. This assessment has served to develop an algorithm th

Anonymous or known donors? A brief discussion of the psychosocial issues raised by removing anonymity from sperm donors.

In spite of a trend towards open identity, the ethical issues raised by the removal of anonymity from sperm donors are still contentious and raise controversy about the significance of genetic inheritance and what are often seen as competing rights claims for the children born, for donors and for the parents of donor insemination (DI) offspring. This review provides a short discussion of the arguments for and against the disclosure of donor insemination status to offspring, which are at the heart of the controversy over anonymity as they relate to the stakeholders in the process.

Evidence-based neuroethics for neurodevelopmental disorders.

Many neurodevelopmental disorders affect early brain development in ways that are still poorly understood; yet, these disorders can place an enormous toll on patients, families, and society as a whole and affect all aspects of daily living for patients and their families. We describe a pragmatic, evidence-based framework for engaging in empiric ethics inquiry for a large consortium of researchers in neurodevelopmental disorders and provide relevant case studies of pragmatic neuroethics. The 3 neurodevelopmental disorders that are at the focus of our research, cerebral palsy (CP), autism spectr

[Dignity, ageing and artificial intelligence].

Dignity is a concept frequently invoked in bioethics, both in the literature and in practice. However, those who use the term rarely describe or define it, despite the word's multiple meanings. This article explores its various meanings, highlighting the relational context in which it is situated, whether it is attributed or intrinsic. The discussion focuses on the impact of AI-powered assistive robots, designed to combat isolation and loneliness, as well as on relationality and dignity in the broadest sense.

Turning the Tables.

In bioethics, the concept of vulnerability is applied almost exclusively to research participants and patients. We turn the tables and apply the concept to nurses caring for anorexia nervosa (AN) sufferers. In doing so, and using results from a qualitative research study undertaken in the UK, we show that AN nurses face a significant probability of incurring identifiable harms (inauthentic relationships and nonreciprocal relationships). Some recommendations on how these harms can be avoided or mitigated are given, but further research is needed.

When a Surgical Colleague Makes an Error.

Professionalism requires that doctors acknowledge their errors and figure out how to avoid making similar ones in the future. Over the last few decades, doctors have gotten better at acknowledging mistakes and apologizing to patients when a mistake happens. Such disclosure is especially complicated when one becomes aware of an error made by a colleague. We present a case in which consultant surgeons became aware that a colleague seemed to have made a serious error. Experts in surgery and bioethics comment on appropriate responses to this situation.
