Assessing the Implementation of Shared Decision-Making in Primary Care: A Study With Unannounced Standardized Patients.

RATIONALE, AIMS, AND OBJECTIVES: Shared decision making (SDM) is widely endorsed in clinical guidelines, yet clinicians may not use it consistently across different clinical problems. This study explores how SDM is influenced by the clinical topic under discussion. METHODS: Using unannounced standardized patients (USPs), we observed how resident physicians engaged in SDM during new patient visits involving both gastroesophageal reflux disease (GERD) and smoking cessation. Audio recordings were evaluated using the validated Observer OPTION-5 tool. RESULTS: SDM scores were significantly higher

Engaging Dialysis Teams in Shared Decision-Making Conversations With Patients to Improve Rates of Kidney Transplantation.

RATIONALE & OBJECTIVE: Increasing access to kidney transplantation requires patients to be active on the waiting list and empowered to consider donor offers that are consistent with their preferences. We sought to develop and refine patient-centered communication tools for shared decision-making conversations within dialysis facilities about active status on the waiting list, consent for hepatitis C and high-kidney donor profile index organ offers, and living donation. Conversations may help patients improve their probability of kidney transplantation. STUDY DESIGN: A qualitative study conven

Establishing care goals and shared decision-making in hospitalized oncology patients: a best practice implementation project.

INTRODUCTION: Recognizing the right moment for discussions and defining a patient's care goals is essential. However, aligning these goals across the entire health care team involved in the patient's care can be challenging. AIM: This project aimed to implement best practices to support a shared understanding and care goal concordance among health care professionals caring for hospitalized oncology patients. METHOD: This project was guided by the JBI Evidence Implementation Framework, which is based on an audit and feedback process. The project was conducted in a Brazilian tertiary hospital

Shared Decision-Making-Guided Prehabilitation for High-Risk Anomalous Aortic Origin of the Left Coronary Artery.

OBJECTIVE: To describe shared decision-making-guided prehabilitation as a bridge to repair in unrepaired high-risk anomalous aortic origin of the left main coronary artery. KEY STEPS: A young woman developed cardiogenic shock after hysteroscopic hemorrhage, required extracorporeal membrane oxygenation, and was found to have severe ostial compression of an anomalous left main coronary artery. After initial weaning, oral feeding was followed by recurrent shock requiring renewed support. Because repair was deferred for ventricular dysfunction, malnutrition, frailty, and emotional distress, she u

Association Between the Implementation of Shared Decision-Making and Clinical Characteristics of Living-Donor Kidney Transplantation.

Background/Objectives: Shared decision-making (SDM), which incorporates patients' values and preferences, may support patient-centered selection of renal replacement therapy, but its association with the clinical characteristics of living-donor kidney transplantation (LDKT) remains unclear. This study aimed to evaluate the association between SDM implementation and the clinical characteristics of LDKT. Methods: A specialized SDM outpatient clinic was established at our institution in 2019. We retrospectively analyzed 65 patients who underwent LDKT at a single center between 2010 and 2025, comp

The economic and health consequences of shared clinical decision-making for hepatitis B birth dose vaccination.

AIMS: In December 2025, the Advisory Committee on Immunization Practices recommended individual-based decision-making-termed shared clinical decision-making (SCDM) on CDC schedules-for hepatitis B (HepB) birth-dose vaccination in infants of mothers documented as HBsAg-negative at delivery. We evaluated projected economic, health, and distributional consequences for the 2026 US birth cohort. MATERIALS AND METHODS: A hybrid decision tree-Markov cohort model took the societal perspective over a lifetime horizon for 3.6 million 2026 US births, under three coverage-decline scenarios (10-, 20-, and

Shared clinical decision-making for vaccination: A view through the lens of mid-adult HPV vaccination.

In the summer of 2019, the U.S. Advisory Committee on Immunization Practices (ACIP) updated its recommendations for HPV vaccination. The new recommendation regarding adults ages 27-45y (i.e., mid-adults) indicated that a "shared clinical decision-making" (SCDM) approach, rather than a routine recommendation, should be used. The Morbidity and Mortality Weekly Report (MMWR) publication that described this SCDM recommendation included specific conditional guidance and considerations for SCDM. However, some aspects of the guidance are vague, and others are inconsistent with SCDM, which could lead

Purposeful Shared Decision-Making in Caring for and with Patients with Chronic Pain Receiving Opioid Therapy.

CDC guidelines for prescribing opioids for chronic pain recommend that patients and clinicians engage in shared decision-making (SDM), a practice often described as clinicians working with patients to find treatment options that match patient preferences. Some experts have argued otherwise given limited efficacy of opioid use for chronic pain, the potential effects of long-term opioid therapy on patient's decision-making capacity, and the societal consequences of opioid diversion. Chronic pain care involves reaching a shared understanding of how the patient's pain affects living and how to cha

Ileostomy or Ileal Pouch Anal Anastomosis: Shared Decision-Making in the Surgical Management of Ulcerative Colitis.

Up to 30% of patients with ulcerative colitis will ultimately require surgery. Generally, surgery for ulcerative colitis consists of total proctocolectomy with either end ileostomy or ileal pouch anal anastomosis, which may be performed in 1 to 3 stages. There are risks and benefits specific to undergoing permanent ileostomy creation or ileal pouch anal anastomosis, with each procedure resulting in different and dramatic changes in a patient's day-to-day life. Because most patients are eligible for either procedure, patient preferences are an important consideration when creating a treatment p

Systematic review of barriers, facilitators, and tools to promote shared decision making in the emergency department.

OBJECTIVE: The objective was to systematically review all studies focusing on barriers, facilitators, and tools currently available for shared decision making (SDM) in emergency departments (EDs). BACKGROUND: Implementing SDM in EDs seems particularly challenging, considering the fast-paced environment and sometimes life-threatening situations. Over 10 years ago, a previous review revealed only a few patient decision aids (PtDAs) available for EDs. METHODS: Literature searches were conducted in MEDLINE, Embase, and Cochrane library, up to November 2023. Observational and interventional studi

Shared Decision-Making and Collaborative Care Models for Pain Management: A Scoping Review of Existing Evidence.

This article aims to summarize the existing evidence on shared decision-making and collaborative care models for acute and chronic pain management. We searched the PubMed database for articles published between 1980 and 2023 and scanned included articles' references to identify additional sources. Two reviewers independently screened titles and abstracts. Studies met inclusion criteria if they implemented or evaluated shared decision-making or collaborative care interventions in patients with acute or chronic pain. A total of 690 articles were reviewed, with 32 full text articles meeting inclu

Can We Enhance Shared Decision-making for Periacetabular Osteotomy Surgery? A Qualitative Study of Patient Experiences.

BACKGROUND: Periacetabular osteotomy (PAO) surgery presents an opportunity for shared decision-making (SDM) and may be facilitated by decision-making tools. Currently, no diagnosis or treatment-specific decision-making tools exist for this patient population. Understanding patient PAO surgery decision-making experiences and processes would enable development of a treatment-specific decision-making tool and would help hip preservation surgeons with SDM practices. QUESTIONS/PURPOSES: Qualitative methodology was used to address the following questions: (1) What were the information support needs

Shared Decision Making in Acute Pain Management in Patients with Opioid Use Disorder: A Scoping Review.

The treatment of acute pain over the years has changed with increasing alternative therapies and increased scrutiny of opioid prescriptions. Shared Decision Making (SDM) has become a vital tool in increasing patient engagement and satisfaction in treatment decisions. SDM has been successfully implemented in the management of pain in a variety of settings; however, information regarding the use of SDM for treating acute pain in patients with a history of opioid use disorder (OUD) remains scarce. Following the Preferred Reporting Items for Systematic Reviews and Meta-analysis Extension for Scopi

Shared decision-making for postoperative opioid prescribing and preoperative pain management education decreases excess opioid burden.

INTRODUCTION: Managing postoperative pain requires an individualized approach in order to balance adequate pain control with risk of persistent opioid use and narcotic abuse associated with inappropriately outsized narcotic prescriptions. Shared decision-making has been proposed to address individual pain management needs. We report here the results of a quality improvement initiative instituting prescribing guidelines using shared decision-making and preoperative pain expectation and management education to decrease excess opioid pills after surgery and improve patient satisfaction. METHODS:

Challenges to Employing Shared Decision Making With Adults Under Community Supervision Who Have a Mental Illness.

Adults under community corrections supervision and who have a mental illness (MI) are expected to comply with conditions of release which often include involvement with supportive social services. The rates of technical violation, arrest, and incarceration that result from failure to comply with these mandates are exceedingly high. Shared decision making among officer-supervisors and client-supervisees is a promising approach to promote engagement in community corrections services among supervisees who have MI. This paper reviews recent research on shared decision making and identifies three b
