Transition from child- to adult-orientated care for children with long-term health conditions: a process, not an event.

Long-term health conditions in childhood include both congenital conditions and acquired diseases. Children with long-term health conditions face issues and potential secondary problems that are different from those of adults with chronic diseases. Transition to adult-orientated care for such children and adolescents is a major challenge. Transition needs to be prepared for and planned. A variety of possible transition models exists, depending on circumstances.

Transition to a smoke-free culture within mental health and drug and alcohol services: A survey of key stakeholders.

Tobacco smoking is common among people with mental illnesses, and they carry a higher burden of smoking-related illnesses. Despite this, smoke-free policies and systems for supporting cessation have proved difficult to introduce in mental health and drug and alcohol services (MHDAS). This paper examines the barriers to becoming smoke free within New Zealand services. Key informants, including staff, smoke-free coordinators, and cessation specialists were interviewed. Of the 142 invited informants 61 agreed (42%) to participate in a telephone interview, and 56 provided useable data. Organizatio

Latent profile transitions in bullying role behaviors and associated mental health problems among Chinese adolescents.

BACKGROUND: Existing research indicates that bullying roles often co-occur in different profiles. However, evidence on how these profiles transition over time and how such transitions relate to later mental health remains limited, especially in Chinese adolescents. This study examined latent profiles and transition patterns of bullying role behaviors and their associations with subsequent psychological outcomes. METHODS: This study involved a total of 5477 adolescents in China, surveyed on two occasions. Participants completed questionnaires assessing bullying role behaviors at both T1 and T2

Ageing, health system implications, and a changing epidemic: organizational transitions and stigma dynamics in Philippine HIV care.

BACKGROUND: The global HIV epidemic is undergoing a demographic transition as expanded access to antiretroviral therapy (ART) has increased life expectancy among people living with HIV (PLWH). As a result, the population of older adults living with HIV (OPLWH) is rapidly growing, introducing new clinical, psychosocial, and public health challenges. In the Philippines, one of the fastest-growing HIV epidemics in the Asia-Pacific region, this shift is occurring alongside rising incidence, creating a dual burden of ongoing transmission and emerging survivorship needs. METHODS: This study employe

Impact of Transition from Compassionate to Scheduled Dialysis on Quality of Life for Patients at a Federally Qualified Health Center.

BACKGROUND: In the United States, uninsured patients with kidney failure often rely on emergency (compassionate) hemodialysis in acute care settings to manage life-threatening complications, as scheduled hemodialysis is often unattainable due to cost. This reactive approach has higher mortality rates, health care utilization, and cost than scheduled hemodialysis. Quality of life (QoL) outcomes are underexplored. This study evaluates changes in QoL after transitioning uninsured patients from emergency to scheduled hemodialysis. METHODS: Kidney Dialysis Quality of Life (KDQOL) surveys were admi

Mental health and gender transitioning among hijra individuals: a qualitative approach using the minority stress model.

This study used the minority stress model to explore experiences of gender transitioning among hijra (trans woman) individuals in Mumbai, India. It used a qualitative life history calendar approach to examine transitioning as a critical life experience in a chronological manner. Twenty hijra-identified persons were recruited using purposive snowball sampling and interviewed at a community-based organisation. Data were analysed using thematic analysis. Participants reported diverse experiences living as hijra and shared perspectives on transitioning. Most participants identified to undergo tran

Do transitions in and out of homelessness relate to mental health episodes? A longitudinal analysis in an extremely disadvantaged population.

This paper studies the association between homelessness and mental health episodes focusing on episodes of depression, anxiety and other conditions (bipolar affective disorder, schizophrenia, and post-traumatic stress disorder). Using panel data from the Australian Journeys Home survey, we investigate the extent to which this association is due to common determinants of homelessness and mental health episodes, or whether there is a causal link between them. The results indicate that episodes of depression increase the probability that a person becomes homeless. This is not the case for other m

The differential impacts of PEPFAR transition on private for-profit, private not-for-profit and publicly owned health facilities in Uganda.

While transition of donor programs to national control is increasingly common, there is a lack of evidence about the consequences of transition for private health care providers. In 2015, President's Emergency Plan for AIDS Relief (PEPFAR) identified 734 facilities in Uganda for transition from PEPFAR support, including 137 private not-for-profits (PNFP) and 140 private for-profits (PFPs). We sought to understand the differential impacts of transition on facilities with differing ownership statuses. We used a survey conducted in mid-2017 among 145 public, 29 PNFP and 32 PFP facilities reportin

Mental health and quality of life of migrants in transit through the Darién gap: A cross-sectional assessment in Panama.

BACKGROUND: Migration through the Darién Gap en route to the US peaked in 2023, with hundreds of thousands of people in transit exposed to physical and psychological health hazards. However, systematic data on mental health conditions for this population remain limited. This study explored the prevalence of psychological symptoms, mental disorders, exposure to psychological trauma, and health-related quality of life among migrants in transit through Darién and described age- and sex-based differences. METHODS: In January 2022, we conducted a cross-sectional mental health assessment at a gover

Urban-Rural Health Transitions in India: A Comprehensive Review of Non-communicable Disease Trends and Risk Landscapes.

India is experiencing a dual burden of non-communicable disease (NCD) risk factors, with distinct patterns emerging across rural and urban geographies. While behavioral risks such as tobacco and alcohol use remain entrenched in rural settings, urban populations are disproportionately affected by metabolic risks such as hypertension, diabetes, and obesity. This review aims to synthesize national data to understand the geographic divergence of NCD risk factors and their implications for public health policy. A comprehensive literature review was conducted using data from the National Family Heal

Mapping developmental transitions in mental health from mid- to late-adolescence: Concurrent and longitudinal links to cognition.

BACKGROUND: Developmental changes in mental health are mostly mapped between childhood and adolescence or childhood and adulthood. This study maps developmental transitions in mental health profiles from mid- to late-adolescence, exploring how these transitions relate to cognitive function in mid-adolescence. METHOD: Participants from the IMAGEN cohort (N = 1304) were followed from mid- (14 years) to late (22 years) adolescence. K-means clustering was applied to data from those with elevated mental health problems to identify common profiles of mental health symptoms at each timepoint (n = 78

Frailty transitions and risk of chronic kidney disease: insights from the China Health and Retirement Longitudinal Study.

BACKGROUND: Frailty is increasingly recognized as a critical factor in the risk of chronic kidney disease (CKD), and it is also a condition that can undergo transitions. However, the relationship between frailty transitions and CKD risk in aging populations remains underexplored. This study aims to investigate the association between frailty transitions and CKD risk in middle-aged and older adults using data from the China Health and Retirement Longitudinal Study. METHODS: Frailty was assessed using a 40-item Frailty Index (FI), with participants categorized into three groups: robust (FI ≤ 0.

Impact of a transitional care program for heart failure patients at Baylor Scott & White Health: a pilot study.

Heart failure is a chronic health condition characterized by complex symptom management and costly hospitalizations. Hospitalization for the treatment of heart failure symptoms is common; however, many hospitalizations are thought to be preventable with effective self-management. This study describes the small, pilot implementation of a new, interventional, self-management heart failure program, "Engagement in Heart Failure Care" (EHFC), developed to assist heart failure patients with the management of disease symptoms following discharge from an inpatient hospital stay. EHFC was designed to e

Transitioning from paediatric to adult services with cystic fibrosis or bronchiectasis: What is the impact on engagement and health outcomes?

AIM: To determine whether the transfer of young people with cystic fibrosis (CF) or bronchiectasis from paediatric to adult services is associated with changes in service engagement and/or health outcomes. METHODS: Young people aged ≥15 years of age with CF or bronchiectasis who transferred from the Auckland-based paediatric service (Starship Children's Hospital) to one of three Auckland-based District Health Boards between 2005 and 2012 were identified and included if they had 3 years care both pre-transfer and post-transfer care. Use of Social Media for Health-Related Tasks by Adolescents With Inflammatory Bowel Disease: A Step in the Pathway of Transition.

BACKGROUND: Social media is commonly used among the adolescent and young adult population, including those with chronic diseases. For adults, these platforms have been shown to be a major source of health information. Our aims were to explore how youth with inflammatory bowel disease (IBD) use social media for (1) disease information gathering, (2) provider communication, (3) sense of belonging to the IBD community, (4) self-expression around IBD, and (5) disease management/monitoring. METHODS: An anonymous and voluntary survey was administered to IBD patients age 12 to 25 years at a single c

Health risk factors and differences in outcomes between younger and older veterans using VA transitional housing.

Radiotherapy remains a cornerstone of cancer treatment; however, radioresistance and normal tissue toxicity continue to limit its therapeutic efficacy. Ferroptosis, an iron-dependent form of regulated cell death driven by lipid peroxidation, has emerged as an important determinant of radiation response and a promising target for radiosensitization. Among the regulatory mechanisms involved, NCOA4-mediated ferritinophagy has attracted increasing attention because it controls intracellular iron mobilization through selective ferritin degradation. Nevertheless, its role should be considered within
