Placental steroid sulfatase deficiency: biochemical diagnosis and clinical review.

Twenty-three cases of placental steroid sulfatase deficiency are reported. All children were boys who later acquired ichthyosis of the recessive X-linked type. The steroid sulfatase deficiency was present in placental tissue, umbilical cord leucocytes, and cultured skin fibroblasts of affected boys. An antepartum diagnosis can be obtained either by detecting the enzyme deficiency in cultured amniotic fluid cells or by finding an elevated total excretion of androstenetriol , 16 alpha-hydroxy-dehydroepiandrosterone, and 16 alpha-hydroxy-pregnenolone in maternal third-trimester urine. How to Manage Low Estriol Levels in Pregnancies, One Center Experience.

OBJECTIVE: Low estriol (uE3) levels in the second-trimester screening for Down syndrome may be the result of fetal demise, congenital abnormalities, or some genetic hormonal disorders of the fetus. Although X-linked ichthyosis, a microdeletion syndrome with mild ichthyosis, which causes steroid sulfatase (STS) deficiency, is the most common genetic cause, second-trimester screening tests calculate the risk for a less common and severe disorder known as the Smith Lemli Opitz syndrome (SLOS). We aimed to investigate the outcomes of pregnancies with low uE3 levels in Down syndrome screening and e

Compound heterozygous mutations in SNAP29 is associated with Pelizaeus-Merzbacher-like disorder (PMLD).

Pelizaeus-Merzbacher-like disease (PMLD) is an autosomal recessive hypomyelinating leukodystrophy, which is clinically and radiologically similar to X-linked Pelizaeus-Merzbacher disease (PMD). PMLD is characterized by early-onset nystagmus, delayed development (motor delay, speech delay and dysarthria), dystonia, hypotonia typically evolving into spasticity, ataxia, seizures, optic atrophy, and diffuse leukodystrophy on magnetic resonance imaging (MRI). We identified a 12-year-old Caucasian/Hispanic male with the classical clinical characteristics of PMLD with lack of myelination of the subco

Hypohidrotic ectodermal dysplasia syndrome in a Sudanese patient: a case report.

Hypohidrotic ectodermal dysplasia (HED) is a rare genodermatosis characterised by defects in the development of ectodermal tissues, primarily affecting hair, teeth, and sweat glands. It can be inherited in autosomal dominant, autosomal recessive, or X-linked patterns. Mutations in the EDA, EDAR, or EDARADD genes, which encode Ectodysplasin-A, the EDA receptor, and a death domain, respectively, are responsible for HED. A 36-year-old Sudanese male presented with hypohidrosis, hypodontia, hypotrichosis, and Ichthyosis without an obvious family history of the HED syndrome. Six members of his exten

Keratosis follicularis spinulosa decalvans in a female.

Keratosis follicularis spinulosa decalvans (KFSD), is a rare follicular syndrome associated with widespread keratosis pilaris and progressive scarring alopecia. This genodermatoses often starts at infancy or early childhood with an X-linked mode of inheritance. Males are predominantly affected and females frequently show no disease or only a mild form. We describe this not so common entity of KFSD in a nine year old female child.

Prenatal diagnosis and molecular cytogenetic analysis of Xp22.31 microdeletions and microduplications in Chinese populations.

BACKGROUND: Chromosome rearrangements are frequently observed in the Xp22.31 region. Xp22.31 microdeletion is linked to X-linked ichthyosis, whereas the pathogenicity of Xp22.31 microduplication remains a subject of controversy. In the present study, 52 additional foetuses with Xp22.31 microdeletions/microduplications were presented, with the aim of providing further insights into the prenatal diagnosis and genetic counselling of Xp22.31 microdeletions/microduplications. METHODS: A retrospective analysis was conducted involving a total of 4,251 pregnant women who received amniocentesis of pren

A novel mutation in MBTPS2 causes ichthyosis follicularis, alopecia, and photophobia syndrome.

BACKGROUND: The ichthyosis follicularis, alopecia, and photophobia (IFAP) syndrome is a rare X-linked genodermatosis characterized by noninflammatory spiny follicular hyperkeratosis, severe photophobia, and non-scarring alopecia with variable severities. IFAP syndrome results from mutations in the gene encoding the membrane-bound transcription factor peptidase, site 2 (MBTPS2). METHODS: We present an 11-year-old male with typical clinical features of IFAP syndrome, including diffuse follicular hyperkeratosis, alopecia, photophobia, psoriasiform plaques, short statue, nail dystrophy, mental re

X-linked dominant chondrodysplasia punctata due to novel mutation in EBP gene: A case report.

RATIONALE: Chondrodysplasia punctata is a rare hereditary disorder that affects bone development. The disease is primarily caused by mutations in the emopamil binding protein (EBP) gene, leading to X-linked dominant metabolic enzyme abnormalities. This study aims to report a novel EBP mutation in X-linked dominant chondrodysplasia punctata type 2 (CDPX2), expanding the genetic spectrum of the disease and highlighting the critical role of combined clinical and genetic diagnosis in guiding prenatal counseling and postnatal management. PATIENT CONCERNS: Multiple prenatal ultrasounds revealed mis

[X-chromosome dominant chondrodysplasia punctata (Happle syndrome). Lyonization of the eyelashes?].

X-linked dominant chondrodysplasia punctata or Happle syndrome is a rare hereditary disorder. The clinical features include retardation of growth, cataract, and temporary, ichthyosiform erythroderma. Atrophic lesions and hyperkeratotic papules of the skin and disturbances of hair growth follow the lines of Blaschko. A 5-year-old girl with X-linked dominant chondrodysplasia punctata was found to be mentally retarded and to have abnormalities of the eyelashes (growth and pigmentation), which had not previously been described in this condition.

Retrotransposition disrupting EBP in a girl and her mother with X-linked dominant chondrodysplasia punctata.

X-linked dominant chondrodysplasia punctata (CDPX2) is a rare congenital disorder caused by pathogenic variants in EBP on Xp11.23. We encountered a girl and her mother with CDPX2-compatible phenotypes including punctiform calcification in the neonatal period of the girl, and asymmetric limb shortening and ichthyosis following the Blaschko lines in both subjects. Although Sanger direct sequencing failed to reveal a disease-causing variant in EBP, whole genome sequencing (WGS) followed by Manta analysis identified a ~ 4.5 kb insertion at EBP exon 2 of both subjects. The insertion was associated

Genetic heterogeneity of steroid sulfatase deficiency revealed with cDNA for human steroid sulfatase.

Three cDNA clones with inserts of 1.2-1.6 kb that reacted both with antibodies and oligonucleotides specific for steroid sulfatase were isolated from a human placental library in lambda gt11. The 5'-end of one of the inserts, STS-3, was sequenced and colinearity with the amino acid sequence of 3 peptides of steroid sulfatase encompassing 64 amino acids was demonstrated. STS-3 hybridized with 2.5, 4.6 and 6.3 kb species in poly(A)+RNA and with 2.5, 4 and 9 kb fragments of EcoRI digested human DNA. The frequency of the EcoRI fragments in DNA from females was approximately twice that in DNA from

Genotype-phenotype correlations emerging from the identification of missense mutations in MBTPS2.

Missense mutations affecting membrane-bound transcription factor protease site 2 (MBTPS2) have been associated with Ichthyosis Follicularis with Atrichia and Photophobia (IFAP) syndrome with or without BRESHECK syndrome, with keratosis follicularis spinulosa decalvans, and Olmsted syndrome. This metalloprotease activates, by intramembranous trimming in conjunction with the protease MBTPS1, regulatory factors involved in sterol control of transcription and in cellular stress response. In this study, 11 different MBTPS2 missense mutations detected in patients from 13 unrelated families were corr

Adrenoleucodystrophy: dermatological findings and skin surface lipid study.

X-linked adrenoleucodystrophy (ALD) is a rare disorder of the very-long-chain fatty acid (VLCFA) metabolism. Cutaneous findings observed in a 29-year-old man included patchy non-cicatricial alopecia of the scalp, intense oily seborrhoea of the head, mild dry-scaling ichthyosis-like appearance of the trunk and legs and pseudo-acanthosis nigricans of the folds. Scanning electron-microscopic examination of the scalp hair showed trichorrhexis-nodosa-like fractures and several structural anomalies of the hair shaft. The skin surface lipid profile showed a marked increase in lignoceric acid (C24:0)

[Biochemical study on steroid sulfatase and its clinical application to the obstetrics and gynecology].

Steroid sulfatase desulfates a number of 3 beta-hydroxysteroid sulfates, converting inactive steroid hormone to the active form. I have developed an enzyme-linked immunosorbent assay (ELISA) using polyclonal antibody against the sulfatase which was purified from human placenta to measure an amount of the enzyme protein in sera of gynecologic cancer patients. By this method, it was found that the serum steroid sulfatase level is significantly elevated in patients with endometrial carcinoma (p less than 0.05) and ovarian carcinoma (p less than 0.01) as compared to that of normal women. Steroid s

The Conradi-Hünermann-Happle syndrome (CDPX2) and emopamil binding protein: novel mutations, and somatic and gonadal mosaicism.

The Conradi-Hünermann-Happle (CHH) syndrome (X-chromosomal dominant chondrodysplasia punctata type II; MIM 302960) is an X-linked dominant disorder that is characterized by ichthyosis, chondrodysplasia punctata, cataracts and short stature. The disease occurs almost exclusively in females and shows increased disease expression in successive generations (anticipation). Recently, causative mutations in the emopamil binding protein (EBP) have been identified. To better appreciate the genetics of this syndrome we analyzed the EBP gene in seven independent families using PCR, conformation-sensitive
