OBJECTIVE: Cisplatin is a chemotherapeutic agent with the undesirable side effect of ototoxicity. Transtympanic injections of antioxidant formulations may provide local otoprotection. We tested a novel antioxidant-eluting microparticle for its otoprotective capability from systemic cisplatin as measured by cochlear electrophysiology.
STUDY DESIGN: Basic science.
SETTING: Translational research laboratory.
METHODS: Eighteen mice were assigned to three groups. All mice underwent baseline click-evoked auditory brainstem response (ABR) audiometry and right ear microparticle injections before be

Therapeutic drugs for central nervous system (CNS) diseases need to reach CNS tissues. However, the blood-brain barrier often limits their therapeutic effects. To address this issue, highly invasive drug administration routes, such as intracerebroventricular or intrathecal administration, can be used. In addition, intranasal (i.n.) administration is increasingly being recognized as a non-invasive route, although its application in humans is limited. Hence, we explored intratympanic (i.t.) administration as a novel, minimally invasive route for direct drug delivery to the CNS. The aim of this s

A novel technique for the diagnosis and management of middle ear myoclonus is described. A patient with middle ear myoclonus underwent a trans-canal microscopic middle ear exploration with injection of botulinum toxin into the stapedius and tensor tympani muscles. Postoperatively, the patient noted complete resolution of symptoms. This is the first report of the use of botulinum toxin directly applied to the middle ear musculature via a trans-canal approach for the management of middle ear myoclonus. This approach is both a useful diagnostic and therapeutic tool that allows for temporary muscl

Intratympanic gentamicin therapy is widely used clinically to treat the debilitating symptoms of Ménière's disease. Cochleotoxicity is an undesirable potential side effect of the treatment and the risk of hearing loss increases proportionately with gentamicin concentration in the cochlea. It has recently been shown that gentamicin is readily absorbed through the oval window in guinea pigs. The present study uses quantitative functional measures of vestibular and cochlea function to investigate the efficacy of treating the vestibule by applying a small volume of gentamicin onto the stapes footp

OBJECTIVE: Systemic dexamethasone has demonstrated conclusive benefits in reversing sudden sensorineural hearing loss (SSNHL) despite considerable number of potential side effects. In contrast, the intratympanic route of steroid administration averts several possible complications. This study aims to examine the literature to delineate the efficacy and side effect of intratympanic dexamethasone (ITD) injection for the treatment of SSNHL.

HYPOTHESIS: OTO-201 can provide sustained release to the middle ear and effectively treat otitis media, when compared with FDA-approved ciprofloxacin otic drop formulations.
BACKGROUND: There is an unmet medical need for antibiotic therapy that can provide a full course of treatment from a single administration by an otolaryngologist at the time of tympanostomy tube placement, obviating the need for twice daily multiday treatment with short-acting otic drops.
METHODS: Studies in guinea pigs and chinchillas were conducted. OTO-201 was administered as a single intratympanic injection and compa

We present results at 2 years follow-up of a group of 15 patients with Ménière's disease treated with a low concentration of intratympanic gentamicin (IG group), compared with a group of 15 patients who refused any surgical treatment (NH group). IG was administered according to a predetermined and fixed schedule consisting of 2 doses of 0.5 ml gentamicin solution, pH 7.8, injected once a week, with a drug concentration of 20 mg/ml; the total dose of gentamicin was < or = 20 mg. Additional doses of IG were administered only in patients who had recurrence of vertigo. The results were evaluated f

OBJECTIVES: Cisplatin is a widely used chemotherapy agent. However, one of the side effects associated with cisplatin use is ototoxicity, causing hearing loss, tinnitus, and balance disorders. Furthermore, no preventive treatment for these conditions is currently available. Intratympanic administration of dexamethasone has reduced cisplatin-induced ototoxicity in vivo. However, clinical trials still need to be conducted to assess its efficacy in humans. The aim of this study was to assess the efficacy of intratympanic administration of dexamethasone in preventing tinnitus in patients with canc

BACKGROUND: Hearing impairment is a growing social and economic issue. New technical or biological approaches aiming hearing rehabilitation or regeneration require animal testing. Therefore, a reproducible and safe model for hearing-impaired animals is essential.
NEW METHOD: Intratympanic injection of kanamycin and furosemide was administered for BFA bunt pigmented guinea pigs for either 1 or 2 h. Hearing loss was regularly measured with compound action potential response to click and tone burst stimuli for up to 26 weeks. Hair cell loss and the density of spiral ganglion neurons were histolo

UNLABELLED: <p>Introduction: Intratympanic corticosteroids (ITCs) are used to treat patients with idiopathic sudden sensorineural hearing loss (ISSNHL) when systemic therapy has been unsuccessful. However, the efficacy of this treatment remains uncertain.
METHODS: We retrospectively analysed ISSNHL patients at a tertiary reference centre, who received ITC injections following unsuccessful systemic corticosteroid therapy administered either orally or intravenously. Patients were eligible for ITC if they showed insufficient improvement after systemic treatment, defined as a remaining hearing lo

Twenty-four hours after intratympanic administration of gadolinium contrast material (Gd), the Gd was distributed mainly in the perilymphatic space. Three-dimensional FLAIR can differentiate endolymphatic space from perilymphatic space, but not from surrounding bone. The purpose of this study was to evaluate whether 3D inversion-recovery turbo spin echo (3D-IR TSE) with real reconstruction could separate the signals of perilymphatic space (positive value), endolymphatic space (negative value) and bone (near zero) by setting the inversion time between the null point of Gd-containing perilymph f

BACKGROUND: Gentamicin is used as a therapeutic agent for Ménière's disease because of its vestibulotoxicity causing chemo-ablation of the vestibular sensory epithelia. Its use has increased in recent years. However, there is still no consensus about the dose regimen of gentamicin in the treatment of Ménière's disease. In this study two different dose regimen treatment protocols are compared in a placebo controlled study design. The primary objective is to quantify the treatment effect on dizziness, the secondary objective is hearing evaluation.
METHODS: We performed a randomized, double-blin

The aim of this study was to verify the efficacy of a modified Odkvist titration protocol of intratympanic gentamicin application in the control of vertigo attacks and the effects on the auditory and vestibular function in a group of 71 patients affected by monolateral MD resistant to medical therapy. All the patients underwent an intratympanic administration of a 1-ml solution containing 26.6 mg of gentamicin sulfate. The treatment protocol provided one to three injections for a total amount of gentamicin varying from 26.6 to 80 mg. Five days after the first gentamicin administration, cochlea

We report 3 rare cases of Ménière's disease in children. In Case 1 and 3, vertigo, hearing loss and tinnitus recovered soon after medical therapy. In Case 2, however, vertigo recurred and the hearing level on the right side markedly deteriorated. The equal-loudness contours on three-dimensional audiogram showed that right-sided aggravated hearing loss fluctuated for 4 years at middle-and low-frequencies despite medication. Finally intratympanic injection of gentamicin sulfate was performed. The patient has had no definitive spell of vertigo after gentamicin therapy. At our department, the inci

A review of the recent literature concerning hearing loss and Meniere's disease is presented. A compilation of the change in hearing resulting from endolymphatic sac surgery, vestibular nerve section, and intratympanic gentamicin injection and a summary of the results of pharmacologic interventions is tabulated for 1986 to 1996. Although vertigo control is variable with the above treatment strategies, hearing loss does not appear altered by any of them when compared with the natural history of hearing loss associated with Meniere's disease. Further investigation is necessary to understand the
