Wearable kinesthetic system for joint knee flexion-extension monitoring in gait analysis.

In this paper we investigated on the possibility of using a wearable kinesthetic system for monitoring flexion-extension of the knee joint during deambulation. The final goal was to provide an unobtrusive technique to identify the clinical stage of the recovery process of patients affected by venous ulcer. These patients have great difficulty in walking depending on the severity level of disease and after a surgical operation they usually progressively improve up to recover their normal walking capability. Typically, they are monitored by means of invasive instrumentation. On the contrary, our

Illusory movements of the contralesional hand in patients with body image disorders.

OBJECTIVES: In the present study, we assess whether illusory sensations of movement can be elicited in patients with right brain damage (RBD). METHODS: Ten RBD patients (three with disorders of bodily representations) were asked to report whether movements of their right hand induced any illusory somatic or motor sensations. Inquiries on anomalous sensation of movement of the left hand were carried out while subjects: 1) observed the moving hand in a mirror propped vertically along the parasagittal plane; 2) looked directly at the moving hand; 3) looked at the still hand; 4) kept their eyes c

Formation and lateralization of internal representations underlying motor commands during precision grip.

The capability to store and retrieve weight-related information from a lift to scale the force output during a subsequent lift was examined in 10 healthy adults and 50 children (age 2-10 years), as well as a 22-year-old patient with corpus callosum agenesis. Subjects lifted a test object between the thumb and index finger while the isometric fingertip forces were measured. The results suggest that both healthy children and adults can transfer weight-related information between the right and left hand, although a lateralization was found. Also, the storage and retrieval of weight-related inform

Neuromagnetic activity in medial parietooccipital cortex reflects the perception of visual motion during eye movements.

We usually perceive a stationary, stable world despite coherent visual motion induced by eye movements. This astonishing example of perceptual invariance results from a comparison of visual information with internal reference signals (nonretinal signals) predicting the visual consequences of an eye movement. The important consequence of this concept is that our subjective percept of visual motion reflects the outcome of this comparison rather than retinal image slip. To localize the cortical networks underlying this comparison, we compared magnetoencephalography (MEG) responses under two condi

The Tampa Scale for Kinesiophobia: further examination of psychometric properties in patients with chronic low back pain and fibromyalgia.

The present study attempted to replicate the robustness of a two-factor model of the Tampa Scale for Kinesiophobia (TSK) in chronic low back pain (CLBP) patients and fibromyalgia patients, by means of confirmatory factor analysis. Construct and predictive validity of the TSK subscales were also examined. Results clearly indicated that a two-factor model fitted best in both pain samples. These two factors were labelled somatic focus, which reflects the belief in underlying and serious medical problems, and activity avoidance, which reflects the belief that activity may result in (re)injury or i

[Mutual guiding and following of movement - The interaction process in mobilisation based on the example of kinaesthetics: A qualitative study].

Background: Mobility impairment is often seen as a reason for needing long-term care. Thus, promoting mobility becomes increasingly significant in nursing homes. The kinaesthetic approach offers a way to support nursing home residents in using their own resources to maintain or improve their mobility. Aim: The present study intends to identify the characteristics of the interaction between nursing home residents with impaired mobility and kinaesthetic trainers during mobilisation. Methods: This secondary analysis comprises nine video sequences interpreted according to Grounded Theory-principle

Motor imagery in stroke patients, or plegic patients with spinal cord or peripheral diseases.

OBJECTIVES: When motor imagery (MI) is impaired in stroke patients, it is not clear, whether this is caused by the central lesion with a disruption of networks or this may be due to inactivity/lack of practice following hemiparesis. To answer this question, we investigated MI in two groups of patients: stroke patients and patients with no central lesion, who suffered high-grade tetraparesis caused by myopathy or spinal muscular atrophy. MATERIALS AND METHODS: The first study measured MI in 31 sub-acute and chronic stroke patients with hand paresis. We used self-assessment questionnaires [Kina

Head repositioning accuracy to neutral: a comparative study of error calculation.

Deficits in cervical proprioception have been identified in subjects with neck pain through the measure of head repositioning accuracy (HRA). Nevertheless there appears to be no general consensus regarding the construct of measurement of error used for calculating HRA. This study investigated four different mathematical methods of measurement of error to determine if there were any differences in their ability to discriminate between a control group and subjects with a whiplash associated disorder. The four methods for measuring cervical joint position error were calculated using a previous da

Brain networks of spatial awareness: evidence from diffusion tensor imaging tractography.

Left unilateral neglect, a dramatic condition which impairs awareness of left-sided events, has been classically reported after right hemisphere cortical lesions involving the inferior parietal region. More recently, the involvement of long range white matter tracts has been highlighted, consistent with the idea that awareness of events occurring in space depends on the coordinated activity of anatomically distributed brain regions. Damage to the superior longitudinal fasciculus (SLF), linking parietal to frontal cortical regions, or to the inferior longitudinal fasciculus (ILF), connecting oc

High sensitivity to multisensory conflicts in agoraphobia exhibited by virtual reality.

The primary aim of this study was to evaluate the effect of auditory feedback in a VR system planned for clinical use and to address the different factors that should be taken into account in building a bimodal virtual environment (VE). We conducted an experiment in which we assessed spatial performances in agoraphobic patients and normal subjects comparing two kinds of VEs, visual alone (Vis) and auditory-visual (AVis), during separate sessions. Subjects were equipped with a head-mounted display coupled with an electromagnetic sensor system and immersed in a virtual town. Their task was to lo

A single case-study of diagonistic dyspraxia.

Diagonistic dyspraxia is a clinical syndrome usually characterized by involuntary and conflicting behaviors between the hands following corpus callosum lesions. In the present study, we report the case of a patient who presents such symptoms, along with a series of complex abnormal behaviors, such as carrying out an action and subsequently doing the exact opposite, or being unable to choose between two alternative decisions. The data reported in this study indicate that, at least in some patients, diagonistic dyspraxia can be associated with abnormal, antagonistic, behaviors not limited to the

The human cerebellum and associative learning: dissociation between the acquisition, retention and extinction of conditioned eyeblinks.

The present paper is part of a systematic exploration of the neural substrates of conditioned eyeblink responses in humans. Normal subjects and patients with lesions restricted to the cerebellum were examined for their ability to acquire new classically conditioned eyeblinks to an auditory conditioned stimulus and whether they were able to perform and extinguish a previously learned natural anticipatory eyeblink response - the kinesthetic threat eyeblink response (KTER). In classical conditioning to an auditory conditioned stimulus, cerebellar patients failed to acquire new conditioned respons

Human vestibular cortex as identified with caloric stimulation in functional magnetic resonance imaging.

Anatomic and electrophysiological studies in monkeys have yielded a detailed map of cortex areas receiving vestibular afferents. In contrast, comparatively little is known about the cortical representation of the human vestibular system. In this study we applied caloric stimulation and fMRI to further characterize human cortical vestibular areas and to test for hemispheric dominance of vestibular information processing. For caloric vestibular stimulation we used cold nitrogen to avoid susceptibility artifacts induced by water calorics. Right and left side vestibular stimulation was repetitivel

Self-motion perception and vestibulo-ocular reflex during whole body yaw rotation in standing subjects: the role of head position and neck proprioception.

Self-motion perception and vestibulo-ocular reflex (VOR) were studied during whole body yaw rotation in the dark at different static head positions. Rotations consisted of four cycles of symmetric sinusoidal and asymmetric oscillations. Self-motion perception was evaluated by measuring the ability of subjects to manually track a static remembered target. VOR was recorded separately and the slow phase eye position (SPEP) was computed. Three different head static yaw deviations (active and passive) relative to the trunk (0°, 45° to right and 45° to left) were examined. Active head deviations had

A place principle for vertigo.

OBJECTIVE: To provide a road map of the vestibular labyrinth and its innervation leading to a place principle for different forms of vertigo. METHOD: The literature describing the anatomy and physiology of the vestibular system was reviewed. RESULTS: Different forms of vertigo may be determined by the type of sense organ, type of ganglion cell and location in the vestibular nerve. CONCLUSION: Partial lesions (viral) of the vestibular ganglion are manifested as various forms of vertigo.

Timing of bimanual movements and deafferentation: implications for the role of sensory movement effects.

In a repetitive tapping task, the within-hand variability of intertap intervals is reduced when participants tap with both hands instead of single-handedly. This bimanual advantage has been attributed to timer as opposed to motor variance (according to the Wing-Kristofferson model; Helmuth and Ivry 1996) and related to the additional sensory consequences of the movement of the extra hand in the bimanual case (Drewing et al. 2002). In the present study the effect of sensory feedback of the movement on this advantage was investigated by comparing the results of a person (IW) deafferented below t

Perceptual studies in patients with vestibular neurectomy.

Twelve patients undergoing unilateral vestibular neurectomy for the treatment of refractory vertigo were investigated. Vestibular motion perception was assessed using a self-rotational task and "vestibular remembered saccades". Cervical perception was also measured with remembered saccades. The tests were performed pre- and post-operatively to examine changes in vestibular and cervical perception following an acute vestibular lesion, and to monitor the progress of vestibular compensation. These perception tests were carried out in conjunction with a conventional evaluation of the vestibular oc

[Investigations concerning the head-trunk-coordination].

BACKGROUND: In the assessment of vertigo patients with suspected cervicocephalic somatosensory deficit, the studies published thus far have focused on reflectory systems like the cervico-ocular reflex or the postural systems. Our study examines the cervicocephalic kinesthetic sensibility of head and body coordination. We therefore introduce two preliminary test series: the "subjective zero chin-knee-angle of the occipital joints" and the "saccadic repositioning during trunk rotation". METHODS: Subjective recordings of dizziness complaints were done using the Dizziness Handicap Inventory. PAT
