Electric cataract after high-voltage injury.

A 28-year-old man presented with a 2-year history of painless loss of vision in the left eye after accidental contact with a high-voltage electric wire. Examination revealed a distinctive linear scar extending from the scalp across the left forehead and face to the upper lip, reminiscent of a lightning-bolt mark (Figure 1A). The best-corrected visual acuity was 6/6 in the right eye and hand motions in the left eye. Slit-lamp examination of the left eye showed a mature white cataract with dense anterior capsular fibrosis (Figure 1B). Ultrasonography of the posterior segment was normal. A diagno

Shortening the biologics clinical timeline with a novel method for generating stable, high-producing cell pools and clones.

Reducing drug development timelines is an industry-wide goal to bring medicines to patients in need more quickly. This was exemplified in the coronavirus disease 2019 pandemic where reducing development timelines had a direct impact on the number of lives lost to the disease. The use of drug substances produced using cell pools, as opposed to clones, has the potential to shorten development timelines. Toward this goal, we have developed a novel technology, GPEx® Lightning, that allows for rapid, reproducible, targeted recombination of transgenes into more than 200 Dock sites in the Chinese ham

Risperidone treatment of motor restlessness following anoxic brain injury.

PRIMARY OBJECTIVE: This paper examined the use of an atypical neuroleptic medication, risperidone, in reducing the excessive motor activity of an adolescent with an anoxic brain injury following cardiac arrest from a lightning strike. Lower extremity restlessness caused the patient to develop skin breakdown and interfered with healing of existing burns. RESEARCH DESIGN: Single-blind, placebo-controlled single-subject experimental design. EXPERIMENTAL INTERVENTION: Escalating doses of risperidone up to 1 mg and in combination with methylphenidate (10 mg) and amantidine (100 mg). MAIN OUTCOME

Medical mountain rescue in the Mont-Blanc massif.

Because of its peculiar geographic location beneath Mont-Blanc, the Chamonix Hospital plays an important role in the management of mountaineering- and skiing-related traumatic injuries. The authors, thanks to the study of 5200 mountain rescue medical reports, set out to explain the different aspects of this activity: epidemiology, specific equipment, care in the field, reception and primary survey, dispatching, common therapeutic attitudes, progress, and results. In the dangerous environment of the high mountains, emphasis is put on the physicians' experience, allowing an early evacuation in t

Subarachnoid block and enlargement of the spinal canal in hypertrophic neuritis.

A case of Dejerine-Sottas hypertrophic neuritis is reported. The patient, a 45-year-old male, suffered from chronic hypertrophic polyneuropathy, abnormal pupils, fasciculations, tremor, back pain, impotence, sphincter disorders, cramps, and lightning pains in the lower extremities. Besides extensive subarachnoid block, there was X-ray evidence of enlargement of the bony spinal canal with scalloping of the lumbar vertebrae. Surgical exploration showed these abnormalities to be due to extreme hypertrophy of the cauda equina. Histologic findings in peripheral nerve and lumbar root biopsies were t

Diverse Recruitment for Clinical Trials Using Three Diversity-Enhancing Interventions: Qualitative Study.

BACKGROUND: Lack of diversity in clinical trials reduces the generalizability of treatment efficacy across different demographic groups, thereby diminishing the relevance, quality, and equity of research findings. OBJECTIVE: This study aimed to evaluate the implementation of 4 recruitment strategies and their impact on clinical trial enrollment through the Trial of Sites to Increase Diversity in Clinical Trials (TOTAL) project, a collaboration between Stanford and Morehouse School of Medicine funded by the American Heart Association. METHODS: Clinical trials were randomized to 1 of 3 recruit

The Vulnerability of People to Damaging Hydrogeological Events in the Calabria Region (Southern Italy).

Background: Damaging Hydrogeological Events (DHEs) are severe weather periods during which floods, landslides, lightning, windstorms, hail or storm surges can harm people. Climate change is expected to increase the frequency/intensity of DHEs and, consequently, the potential harm to people. Method: We investigated the impacts of DHEs on people in Calabria (Italy) over 37 years (1980-2016). Data on 7288 people physically affected by DHEs were gathered from the systematic analysis of regional newspapers and collected in the database named PEOPLE. The damage was codified in three severity levels

Corona chemistry in Titan.

The atmosphere of Titan is constantly bombarded by galactic cosmic rays and Saturnian magnetospheric electrons causing the formation of free electrons and primary ions, which are then stabilized by ion cluster formation and charging of aerosols. These charged particles accumulate in drops in cloud regions of the troposphere. Their abundance can substantially increase by friction, fragmentation or collisions during convective activity. Charge separation occurs with help of convection and gravitational settling leading to development of electric fields within the cloud and between the cloud and

Wilderness Neurology.

The study of wilderness medicine is within the scope of medical care in the austere environment and addresses medicine as practiced in the setting of delayed access to definitive medical care, hostile environment, limited equipment, and inherent risks to the patient and/or rescuers. Part of this topic includes the care of patients with neurologic illness and/or injury. We will address the five most important skills of a wilderness medicine professional: decision making, prevention, preparation, protocol development, and education by applying the principles to select common neurologic problems t

[The hemodynamic and metabolic changes following tourniquet release during halothane anesthesia].

We measured the hemodynamic and metabolic changes following the release of lower limb tourniquet during halothane anesthesia, and discussed the cause of reduction in the blood pressure and the effective method for prevention. Cardiac index decreased transiently following the release, and gradually increased afterward. Systemic vascular resistance index decreased continuously and progressively until 10 minutes after the release. These results indicate that the causative factors of reduction in the blood pressure are temporary decrease in cardiac output and subsequent decrease in systemic vascul

Anti-IgLON5 disease: A case with intestinal obstruction and peripheral neuropathy.

IgLON5 autoimmunity is a novel antibody-mediated disorder characterized by serum and/or cerebrospinal fluid (CSF) positivity for IgLON5 antibody. Anti-IgLON5 disease mainly manifests as sleep disturbances, movement disorders and brainstem syndromes. In this study, we report the case of a patient with anti-IgLON5 disease who presented with abdominal distension, abdominal pain, intermittent dysuria and constipation, and intermittent lightning pain in the extremities, which are atypical of anti-IgLON5 disease and could easily lead to misdiagnosis. After performing autoantibody screening, we consi

[Emergency Medicine in Remote Settings].

For many acute diseases and injuries, treatment does not differ between industrialised environment and wilderness setting. However, for some emergencies, treatment needs to be adapted if advanced medical care facilities cannot be reached within 4 hours. In these situations, dislocated joints and fractures should be reduced quickly. Contaminated wounds should be cleaned carefully, with drinking water being sufficient when no sterile solution is available. A patient with a contaminated wound should receive a systemic antibiotic coverage within one hour. Keeping an injured patient warm is crucial

Thoracic compression myelopathy due to the progression of dystrophic scoliosis, the presence of a paraspinal tumor, and high and excessive amplitude movement of the shoulder.

The authors present a case of 45-year-old man with neurofibromatosis type 1 (NF-1) and thoracic scoliosis, previously undergoing fusion surgery, who developed myelopathy. This patient further complained of lightning pain when he extended and horizontally abducted the convex-side shoulder. Radiological examination revealed the progression of dystrophic scoliosis with opened spinal canals and the presence of a neurofibroma behind the spinal cord at the apical levels. Delayed development of spinal instability can occur due to dystrophy even postoperatively in patients with NF-1. After tumor resec

[Opsoclonus and myoclonus syndrome. Clinical and electronystagmography findings with course studies].

This review is based on the clinical experience with 7 patients and a total of 171 additional cases described in literature. Clinical features are presented of this "benign" cerebellar encephalopathy, which occurs preferably in association with viral infections, neuroblastomas or rarely carcinomas. Numerous descriptions of single cases stress a heterogeneous variety of other diseases which possibly may cause this syndrome. Special emphasis is put on the electronystagmographic documentation of eye movements with follow up of the spontaneous recovery over several years. The eye movement characte

Super-refractory Status Epilepticus and Cerebellar Involvement Following High-Voltage Electrical Injury.

Electrocution injuries can impact any organ or system in the body. The symptoms vary widely, from immediate cardiac arrest due to high-voltage current to mild discomfort caused by low-voltage current. There are four main types of electric injuries: flash, flame, lightning, and true direct injuries. Among these true and flame electric injuries are those that involve the body and have distinct entrance and exit sites.  Immediate effects on the central nervous system following a high-voltage injury may include loss of consciousness, confusion, seizures, and poor recall. However, seizures as a com

A uniQue wAy of Transcatheter mAss extraction: The QATAR Procedure.

BACKGROUND: Intracardiac masses traditionally require surgical resection, but transcatheter techniques offer a less invasive alternative. CASE SUMMARY: A 57-year-old woman with rheumatic heart disease and severe mitral stenosis was evaluated for percutaneous mitral commissurotomy. Imaging revealed a mobile right atrial mass near the inferior vena cava, posing a risk for embolization. Given the need for mass removal before the procedure, we performed the QATAR technique using the Penumbra Lightning 16 system, a loop snare, and electrocautery. The mass was successfully extracted without complic

BSER abnormalities in a multiple sclerosis patient with normal peripheral hearing acuity.

The measurement of brain stem evoked potentials has proven clinically useful in determining the presence of multiple sclerosis plaques along the auditory areas of the brain stem. Abnormalities in brain stem evoked responses (BSER) have been identified in patients who manifest normal hearing acuity when evaluated via conventional audiometric techniques. The following case presentation illustrates this confounding clinical finding. The patient was a 20-year-old female who had been previously diagnosed as having multiple sclerosis. Within one month after being evaluated by BSER testing, she was s
