Estimating the viral loads of SARS-CoV-2 in the oral cavity when complicated with periapical lesions.

BACKGROUND: The oral cavity represents a main entrance of the severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2). Angiotensin-converting enzyme 2 (ACE-2), neuropilin-1 (NRP-1), and transmembrane serine protease 2 (TMPRSS2) are essential for the entry of SARS-CoV-2 to the host cells. Both ACE-2 and NRP-1 receptors and TMPRSS2 have been identified in the oral cavity. However, there is limited knowledge about the impact of periapical lesions and their metabolites on the expression of these critical genes. This study aims to measure the impact of periapical lesions and their unique fatty

Intracranial Infections Arising From an Odontogenic Infection: A Report of 2 Cases.

INTRODUCTION: Intracranial empyema is a rare but serious and life-threatening infection. It is an accumulation of purulent material in the subdural or extradural space leading to development of subdural empyema or intracranial epidural abscess, respectively. The incidence of morbidity and mortality is high because the diagnosis is often unsuspected. Infections of dental origin could be responsible for such condition. CASE REPORTS: A 22-year-old female and 30-year-old male patients, both with no significant medical history, presented with subdural empyema and intracranial epidural abscess, res

Microbiology and management of endodontic infections in children.

The first step in the origination of caries is the formation of a dental plaque. Dental caries can lead to destruction of enamel and dentin resulting in bacterial invasion of the pulp. Invasion of the pulp and the periapical areas can promote the development of dento-alveolar abscess and spread of the infection to other anatomical areas. Several oral acid producing aerobic and anaerobic bacteria, including Streptococcus mutans, Lactobacillus acidophilus, and Actinomyces viscosus, are capable of initiating the carious lesion. The organisms that predominate in pulpitis and dento-alveolar abscess

Outcomes and Associated Prognostic Factors for Orthograde Canal Obturation Using Ortho MTA III: A Randomised Prospective Clinical Trial.

AIM: To prospectively compare treatment outcomes for orthograde canal obturation using Ortho MTA III (OMTA) with the continuous wave of compaction (CWC) using gutta-percha (GP) and AH Plus sealer, and to identify associated predictive factors. METHODOLOGY: Informed consent was obtained (110 patients), and single- or two-rooted permanent teeth (n = 120) diagnosed with pulp necrosis (or previously treated) and asymptomatic apical periodontitis or chronic apical abscess (periapical index, PAI ≥ 3) were randomly assigned to two groups (n = 60/group). The canals were prepared to a minimal apical s

Reddening of the upper central incisors associated with periapical granuloma in lepromatous leprosy.

Four years after starting treatment for lepromatous leprosy in England a male Pakistani aged 26 was found to have red discoloration of the upper central incisor teeth. A radiograph suggested periapical abscess on the right with haziness in a corresponding area on the left. Right apicectomy was performed with removal of a solid mass attached to the apex, sections revealing a lepromatous infiltrate with acid-fast fragments of Mycobacterium leprae in the cytoplasm of foamy macrophages. Clinical and archaeological evidence for the frequent involvement of these teeth in lepromatous leprosy is revie

18-year follow-up of dens invaginatus: retrograde endodontic treatment.

INTRODUCTION: Dens invaginatus is a rare developmental anomaly that occurs during odontogenesis, with a higher prevalence in the Caucasian race and no significant sex predilection; it can be bilateral. This anomaly occurs in approximately 1.26% of the teeth, and the maxillary lateral incisor is most often affected. METHODS: This case report describes a female patient with dens invaginatus in the maxillary right conoid lateral incisor. The patient presented to Araçatuba School of Dentistry, Universidade Estadual Paulista, São Paulo, Brazil, in 1995 with an acute periapical abscess in the palat

Poly(Lysine)-Derived Carbon Quantum Dots Conquer Enterococcus faecalis Biofilm-Induced Persistent Endodontic Infections.

INTRODUCTION: Persistent endodontic infections (PEIs) mediated by bacterial biofilm mainly cause persistent periapical inflammation, resulting in recurrent periapical abscesses and progressive bone destruction. However, conventional root canal disinfectants are highly damaging to the tooth and periodontal tissue and ineffective in treating persistent root canal infections. Antimicrobial materials that are biocompatible with apical tissues and can eliminate PEIs-associated bacteria are urgently needed. METHODS: Here, ε-poly (L-lysine) derived carbon quantum dots (PL-CQDs) are fabricated using

Examination of external apical root resorption with scanning electron microscopy.

UNLABELLED: External apical root resorption may be evident histologically but not visible radiographically until it is advanced. If working length is determined without considering this resorption, overinstrumentation or overfilling may occur. OBJECTIVE: This study used scanning electron microscopy to examine the appearance and to compare the incidence of external apical root resorption in 40 teeth with differing pulpal and periapical diagnoses. STUDY DESIGN: After extraction, 46 roots were sectioned horizontally 6 mm from the anatomic apex and prepared for scanning electron microscopy exami

Management of rapidly progressing periopical pathologies: a case report.

Dentists often treat swellings from odontogenic periapical pathologies. One management option involves immediate treatment with antibiotics, followed by surgical intervention. We report a clinical case in which an 8-year-old patient sought care for such a lesion and received pharmacological therapy alone. The lesion expanded into multiple facial spaces approximating the ocular organ and other vital cranial structures. Eventual treatment of the lesion required a CT-scan, followed by an intubated general anesthetic, incision and drainage, extraction of the involved dentition and an overnight hos

Resolution of Odontogenic Sinus Pathosis by Endodontic Microsurgery.

A case report is presented, in which a tooth with a chronic apical abscess that has caused a maxillary sinusitis was treated with an endodontic microsurgery. A 65-year-old female patient reported discomfort in a maxillary tooth gingiva. Clinically, a dental metal-ceramic bridge with adequate marginal adaptation, involving teeth from maxillary right canine to maxillary right second molar, was observed. The periapical radiograph showed the adequate adaptation of the crown and post in the maxillary right first premolar, an apical lesion, and the heavy thickening of the maxillary sinus floor mucos

Oxidative stress in patients with endodontic pathologies.

BACKGROUND: Apical periodontitis (AP) is an inflammatory disease affecting periradicular tissues. It is a widespread condition but its etiopathogenetic mechanisms have not been completely elucidated and the current treatment options are not always successful. PURPOSE: To compare oxidative stress (OxS) levels in the saliva and the endodontium (root canal [RC] contents) in patients with different endodontic pathologies and in endodontically healthy subjects. PATIENTS AND METHODS: The study group of this comparison study included 22 subjects with primary chronic apical periodontitis (pCAP), 26

Diagnostic terminology: report of an online survey.

INTRODUCTION: Diagnostic terminology used in endodontics has been based on historical use, textbook and glossary terms, best-available science, and personal conviction. There is minimal evidence that establishes an absolute set of terms that can be used to make a definitive diagnosis. As a component of the Consensus Conference on Diagnostic Terminology held in October 2008, an online survey was conducted to invitees to assess their opinions on diagnosis. METHODS: Thirty-six questions were developed by the oversight committee and chairs of the subcommittees. Most questions used a modified Like

Fixed prosthodontic rehabilitation using a facially driven fully digital workflow of a patient with syndromic amelogenesis imperfecta associated with a rare form of ectodermal dysplasia, tricho-dento-osseous (TDO) syndrome.

Tricho-dento-ossseous (TDO) syndrome is a rare form of ectodermal dysplasia (ED) characterized by defects in the hair, bones, nails, and teeth. Dental findings consistently seen in patients with TDO syndrome include amelogenesis imperfecta and taurodontism, but additional findings such as periapical abscesses, mandibular prognathism, and impacted teeth have been reported. Because of the rarity of this disorder, the prevalence is unknown and limited research is available regarding its dental management. This clinical report describes the fixed prosthodontic implant rehabilitation of an adult pa

[Pathogen spectrum and resistance status of exclusively anaerobic odontogenic infections].

Out of 440 dentogenic pyogenic infections, 171 exclusively caused by anaerobes were investigated to understand the importance of anaerobic bacteria in dental pyogenic processes better. Grampositive anaerobic bacteria dominated. The predominant grampositive isolates in monoinfections were Peptostreptococci and in the case of mixed infections, strains of the genus Eubacterium. Strains of Prevotella and Porphyromonas dominated the gramnegative anaerobic spectra. The resistance to penicillin was very low. Altogether, only one strain of Prevotella oris and one strain of Prevotella oralis showed res
