BACKGROUND: The rapid development of ascites in a patient without known liver disease is an uncommon occurrence in the Emergency Department. Initial stabilization may include therapeutic peritoneal lavage to reduce diaphragmatic pressure and halt the progression of respiratory compromise. In the absence of liver disease, the differential diagnosis should include a search for malignancy, which has been reported to account for up to 10% of all cases of newly diagnosed ascites.

A case of thrombosis in the superior mesenteric artery simulating acute pancreatitis, but causing intestinal gangrene, is described. The possibility of other simulating diseases must always be considered when non-operative peritoneal lavage is used as a treatment of acute pancreatitis. Bacteriological examination of the peritoneal fluid might be beneficial. Laparotomy is indicated when there is a marked discrepancy between the general condition of the patient and the objective clinical signs of pancreatitis.

A 58-year-old man underwent an emergency laparoscopic procedure for small bowel perforation with peritonitis after the ingestion of a wooden toothpick. Treatment consisted of laparoscopic removal of the foreign body, followed by lavage of the abdominal cavity and laparoscopic closure of the perforation, including omentoplasty. The patient recovered from peritonitis and was discharged from the hospital on day 14 after the operation.

Over the period 1990 through 1995, thirty-five patients with multiorgan insufficiency (MOI) undergo treatment. Abdominal operations are performed in all of them, except for one male patient aged 31 years, dying of bilateral pneumonia and metabolic disorders resulting from duodenal stenosis. Many types of immune cells appear in peritoneal cavity after abdominal surgery. Seventy consecutive surviving patients with pelvic fractures were reviewed to define those at high risk for intra-abdominal injury. Eleven (16%) had serious associated visceral injuries. UNLABELLED: Clandestine abortion is known to be a major contributor to maternal mortality. We report a case of a 25-year old patient in her 12th week of amenorrhea with peritonitis due to uterine perforation following abortion, admitted with abdomen and pelvis pain, vomiting and diarrhea. Clinical examination on admission showed asthenic peritonitis. Surgical exploration showed widespread acute peritonitis secondary to a perforation of the uterine dome, with collection of 1500 cc of purulent material, dilated bowel loops and multiple false membranes.
