Association of VO₂max With Postoperative Respiratory Complications in Patients Undergoing Lung Resection Surgery: An Exploratory Study.

INTRODUCTION: Preoperative risk assessment is important in patients undergoing lung resection, as postoperative respiratory complications and early mortality remain significant concerns. Cardiopulmonary exercise testing provides an integrated assessment of functional capacity, and maximum oxygen consumption (VO₂max) reflects overall cardiopulmonary reserve. It may therefore help identify patients at increased perioperative risk. OBJECTIVE: The primary aim of the study was to predict early postoperative respiratory complications using preoperative VO₂max in subjects undergoing lung resection.

Mapping Perioperative Vulnerability and Optimization in Hepatectomy: A Bibliometric and Scientometric Analysis of Sarcopenia, Frailty, and Prehabilitation.

PURPOSE: Risk assessment before hepatectomy is no longer based only on tumor burden, liver function, and technical resectability. Muscle status, frailty, nutrition, exercise capacity, and prehabilitation are now discussed more often in this setting. This study reviewed how these topics appear in the hepatectomy literature and how they relate to the more established body composition literature. METHODS: Relevant publications were retrieved from the Science Citation Index Expanded of the Web of Science Core Collection. Cardiopulmonary exercise testing prior to radical cystectomy: a systematic review and meta-analysis.

OBJECTIVE: To identify the association between cardiopulmonary exercise testing (CPET) and outcomes of radical cystectomy (RC), as RC is historically associated with high rates of short- and long-term morbidity and mortality. METHODS: This quantitative systematic review and meta-analysis was conducted in accordance with the Preferred Reporting Items for Systematic Review and Meta-Analysis guidelines. An electronic literature search was conducted to identify all relevant studies evaluating the relationship between CPET parameters and RC outcomes. The primary outcome was short-term mortality. S

Perioperative Assessment/Prehabilitation in Larynx Cancer.

Head and neck cancer (HNC) survivorship is increasing, and with it, a shift in treatment practices has occurred. Radical surgical resections for the treatment of HNC have decreased, and organ preservation treatments have increased. Although effective in treating HNC, chemoradiation therapy toxicities can be detrimental to a patient's overall health, nutrition status, and quality of life (QOL). Considering that dysphagia is typically a driving element of dysfunction, speech-language pathologists are vital to the prehabilitation phase. Prehabilitation programs include a variety of components, wi

Cardiac Prehabilitation.

The care of patients undergoing cardiac surgery is becoming more complex, in part owing to the increasing burden of comorbid disease, frailty, and psychosocial issues. Many risk factors for postoperative morbidity and mortality are potentially modifiable if identified and treated in a timely fashion before surgery. Cardiac prehabilitation, draws from strategies currently undertaken in cardiac rehabilitation but implements them proactively rather than reactively. There is substantial evidence that in multiple domains, including aerobic conditioning, respiratory muscle training, lifestyle modifi

Opening of native aortic valve accomplished after left ventricular assist device implantation in patients with insufficient preoperative Beta-blocker treatment.

Although we recently demonstrated that opening of a native aortic valve (AV) after left ventricular assist device (LVAD) implantation is a sufficient condition to prevent development of aortic insufficiency (AI), its preoperative predictors remain unknown. Data were obtained from 58 patients who had been treated with continuous flow LVAD for ≥ 6 months at our institute between 2006 and 2014. Opening of native AV was accomplished in 21 patients (36%) at postoperative 6 months. Uni/Multivariate logistic regression analyses demonstrated that a preoperative lower cumulative dose of β-blocker was t

[Importance of the exercise test in the follow-up of surgically treated congenital aortic stenoses].

Sixty four children with isolated congenital aortic stenosis (39 valvular, 16 fixed subvalvular, 4 supravalvular and 5 multiple) were operated at a mean age of 11,5 years. Valve repair was possible in all but three patients who had to undergo valvular replacement. Myotomy was associated in 18 cases (28 p. 100). The mean systolic pressure gradient was 79,9 mmHg (+/- 17,8); there was associated aortic regurgitation in 21 patients but this was minimal except in one case. Twenty children (31 p. 100) had symptoms on effort and the basal ECG showed ST-T wave changes in the left precordial leads in 3

Normalized cardiopulmonary exercise function in patients with pectus excavatum three years after operation.

BACKGROUND: During exercise cardiac function is often limited in patients with pectus excavatum. Therefore, we hypothesized that cardiopulmonary exercise function would improve after the Nuss procedure. METHODS: Seventy-five teenagers (49 patients, 26 controls) were investigated at rest and during bicycle exercise before surgery, and 1 year and 3 years postoperatively (after pectus-bar removal). Echocardiography and lung spirometry were performed at rest. Cardiac output, heart rate, and aerobic exercise capacity were measured using a photoacoustic gas-rebreathing technique during rest and exe

Exercise cardiorespiratory function before and one year after operation for pectus excavatum.

In 35 patients with pectus excavatum (aged 17.9 +/- 5.6 years) pulmonary function and maximal exercise test results were compared before and at 1 year after operation. The lower posteroanterior chest diameter on the lateral x-ray film was significantly smaller than normal (p < 0.0001) and increased significantly after operation (p < 0.0001). Preoperatively, total lung capacity (86.0% +/- 14.4%; p = 0.0001) and inspiratory vital capacity (79.7% +/- 16.2; p = 0.0001) were significantly smaller than predicted and further decreased after operation (-9.2% +/- 9.2%; p = 0.0001 and -6.6% +/- 10.7%; p

Exercise prehabilitation in head and neck cancer patients proposed for definitive chemoradiotherapy: The FIT4TREAT randomized controlled trial.

BACKGROUND: Patients with head and neck cancer (HNC) initially scheduled for definitive chemoradiotherapy (CRT) often experience early functional decline and deterioration in health-related quality of life (HRQoL) even before treatment initiation. Evidence for prehabilitation in this non-surgical setting remains limited. This study evaluated whether exercise prehabilitation (EP) initiated before CRT improves functional capacity compared with usual care (UC). METHODS: FIT4TREAT (ClinicalTrials.gov: NCT05418842) was a prospective, single-center, randomized clinical trial. Adults with HNC propos

A single-arm feasibility study protocol for a multiphasic multimodal exercise prehabilitation intervention in individuals receiving allogeneic stem cell transplant.

BACKGROUND: Allogeneic hematopoietic stem cell transplantation (allo-HSCT) can be a life-saving treatment for individuals diagnosed with acute leukemia. However, allo-HSCT can lead to adverse effects, such as reduced physical function. Exercise has demonstrated benefits in post-allo-HSCT recovery, but feasibility issues persist in tailored prehabilitation interventions. OBJECTIVE: To present a multiphasic exercise prehabilitation protocol. The study aims to assess feasibility, safety, and impact while establishing screening and referral pathways to community-based exercise oncology resources.

Long-term lifestyle effects of prehabilitation in colorectal cancer.

BACKGROUND AND PURPOSE: Prehabilitation-a multimodal intervention aimed at optimizing patients' physical, nutritional and psychological status prior to surgery-has been shown to improve postoperative outcomes. However, the long-term impact of such programs on lifestyle remains poorly understood. Aim is to evaluate long-term lifestyle changes following a 4-week prehabilitation program prior to elective colorectal cancer (CRC) surgery and to compare patient-reported outcomes over a 24-month postoperative period between prehabilitation and control groups. Patient/material and methods: This single

The whole-course of supervised home-based multi-modal prehabilitation to improve clinical outcome in patients undergoing neoadjuvant chemotherapy prior to gastrectomy: a single-center randomized controlled trial.

BACKGROUND: Although neoadjuvant chemotherapy offers survival benefits to advanced gastric cancer patients, the severe chemotherapy-related toxic side effects and the decline in physical fitness that accompany it increase the difficulty of perioperative management and may result in adverse clinical outcomes for patients undergoing surgery. METHODS: An open-label, Phase II, single-center randomized clinical trial was conducted in the Affiliated Hospital of Qingdao University. Patients with advanced gastric cancer who were planned to undergo gastrectomy after neoadjuvant chemotherapy were random

[Update on current care guidelines: acute respiratory failure--preoperative evaluation].

Concomitant diseases, the patient's general condition, exercise capacity and the extent of surgery are determinants of the operative risk. Increasing number of patients with endovascular stents and antithrombotic medication need special perioperative precautions as well as the eventual endocarditis prophylaxis. The risk of perioperative complications can probably be decreased by respiratory physiotherapy, correction of anaemia and smoking cessation. Severe liver or kidney insufficiency need be evaluated. Principles of preoperative fasting and perioperative strategies with concomitant medicatio
