Respiratory muscle training with enzyme replacement therapy improves muscle strength in late - onset Pompe disease.

BACKGROUND: Pompe disease is an autosomal recessive metabolic disorder caused by the deficiency of the lysosomal enzyme acid α-glucosidase. This deficiency leads to glycogen accumulation in the lysosomes of muscle tissue causing progressive muscular weakness particularly of the respiratory system. Enzyme replacement therapy (ERT) has demonstrated efficacy in slowing down disease progression in infants. Despite the large number of studies describing the effects of physical training in juvenile and adult late onset Pompe disease (LOPD). There are very few reports that analyze the benefits of res

A likely case of chronic graft-versus-host disease polymyositis affecting respiratory and truncal muscles - a comprehensive diagnostic approach.

BACKGROUND: Muscle involvement, termed polymyositis, is an uncommon manifestation of graft-versus-host disease (GvHD) in which the upper and lower limbs are commonly affected. However, respiratory failure due to diaphragmatic weakness has rarely been reported. Diagnosis is usually based on a combination of elevated muscle enzyme levels in the blood, neurophysiological studies, and muscle biopsies. CASE REPORT: A 23-year-old man who presented with Philadelphia chromosome (Ph)-positive chronic myeloid leukemia in myeloid blast crisis, underwent HLA-matched sibling (sister) hematopoietic stem ce

Correlation Between Respiratory Accessory Muscles and Diaphragm Pillars MRI and Pulmonary Function Test in Late-Onset Pompe Disease Patients.

Objectives: Pompe disease is a rare genetic disease produced by mutations in the GAA gene leading to progressive skeletal and respiratory muscle weakness. T1-weighted magnetic resonance imaging is useful to identify fatty replacement in skeletal muscles of late-onset Pompe disease (LOPD) patients. Previous studies have shown that replacement by fat correlates with worse results of muscle function tests. Our aim was to investigate if fat replacement of muscles involved in the ventilation process correlated with results of the spirometry and predicted respiratory muscle impairment in LOPD patien

Morphological and functional characterization of respiratory muscles in male runners across different race distances: an ultrasound and physiological assessment.

BACKGROUND: Running performance depends not only on aerobic capacity but also on the functional integration of the respiratory system and core musculature. However, morphological and functional adaptations of the diaphragm and abdominal muscles across different running disciplines remain insufficiently characterized. The aim of this study is to analyze diaphragm and abdominal muscle morphology using ultrasound, as well as respiratory muscle strength and pulmonary function, in amateur runners classified according to their primary competition distance. METHODS: A cross-sectional observational s

Physiological changes and compensatory mechanisms by the action of respiratory muscles in a porcine model of phrenic nerve injury.

Phrenic nerve damage may occur as a complication of specific surgical procedures, prolonged mechanical ventilation, or physical trauma. The consequent diaphragmatic paralysis or dysfunction can lead to major complications. The purpose of this study was to elucidate the role of the nondiaphragmatic respiratory muscles during partial or complete diaphragm paralysis induced by unilateral and bilateral phrenic nerve damage at different levels of ventilatory pressure support in an animal model. Ten pigs were instrumented, the phrenic nerve was exposed from the neck, and spontaneous respiration was

The effect of acute non-invasive ventilation on corticospinal pathways to the respiratory muscles in chronic obstructive pulmonary disease.

Chronic obstructive pulmonary disease is associated with altered cortical excitability. The relevance of this to the need for non-invasive ventilation is not known. We assessed the diaphragm response to transcranial magnetic stimulation in terms of motor threshold and latency as well as assessing intracortical excitability using paired stimulation in eight long-term users and six non-users of home ventilation with COPD. Overall, intracortical facilitation was strongly correlated with inspiratory muscle strength (r2 0.72, p<0.001) whereas intracortical inhibition was correlated with PaCO2 (r2 0

Reproducibility and responsiveness of a noninvasive EMG technique of the respiratory muscles in COPD patients and in healthy subjects.

In the present study, we assessed the reproducibility and responsiveness of transcutaneous electromyography (EMG) of the respiratory muscles in patients with chronic obstructive pulmonary disease (COPD) and healthy subjects during breathing against an inspiratory load. In seven healthy subjects and seven COPD patients, EMG signals of the frontal and dorsal diaphragm, intercostal muscles, abdominal muscles, and scalene muscles were derived on 2 different days, both during breathing at rest and during breathing through an inspiratory threshold device of 7, 14, and 21 cm H2O. For analysis, we use

Assessing respiratory muscle effort during noninvasive respiratory support in acute hypoxemic respiratory failure: A State-of-the Art Review from physiology to bedside.

OBJECTIVES: Respiratory muscle effort is an important determinant of pathophysiology and clinical outcomes in patients with acute hypoxemic respiratory failure receiving noninvasive respiratory support (NIRS). Excessive effort reflects an imbalance between ventilatory load and muscle capacity and has been associated with patient self-inflicted lung injury, treatment failure, and worse outcomes, particularly when escalation to invasive ventilation is delayed. This review provides a clinically focused synthesis of the physiological determinants, bedside assessment, and practical implications of

Effects of respiratory muscle training on respiratory function in patients with amyotrophic lateral sclerosis: a systematic review and meta-analysis.

BACKGROUND: Respiratory decline is prognostically important in amyotrophic lateral sclerosis (ALS), but the efficacy of respiratory muscle training (RMT) remains uncertain. We synthesized randomized evidence on respiratory and related outcomes. METHODS: We searched databases, specialized registers, and trial registries through June 19, 2025, for randomized RMT trials in ALS. Risk of bias was assessed with the Cochrane tool. Random-effects meta-analyses reported standardized mean differences (SMDs) with 95% confidence intervals (CIs), and certainty was rated with GRADE. RESULTS: Six studies w

Maximal Respiratory Pressure Reference Equations in Healthy Adults and Cut-off Points for Defining Respiratory Muscle Weakness.

INTRODUCTION: Maximal inspiratory and expiratory pressures (PImax/PEmax) reference equations obtained in healthy people are needed to correctly interpret respiratory muscle strength. Currently, no clear cut-off points defining respiratory muscle weakness are available. We aimed to establish sex-specific reference equations for PImax/PEmax in a large sample of healthy adults and to objectively determine cut-off points for respiratory muscle weakness. METHODS: A multicentre cross-sectional study was conducted across 14 Spanish centres. Healthy non-smoking volunteers aged 18-80 years stratified
