BACKGROUND: Inpatient psychiatry settings rely heavily on non-licensed direct care behavioral health staff to provide continuous patient supervision, therapeutic support, and effective crisis response. Despite their central role, they remain underrepresented in workforce literature, particularly with respect to factors influencing retention and turnover.
AIMS: The purpose of the study was to explore the lived experiences of non-licensed direct care behavioral health staff in inpatient psychiatry and identify factors that influence intentions to remain in or leave their roles.
METHODS: A qual

This paper evaluates an established five-day drama project, designed, and delivered by a professional company, aimed to support the development of self-confidence of seven men with a history of substance misuse in a category C prison. The project involved creation of a safe space, improvised role-play, development of communication skills, and exploration of substance misuse, culminating in a performance. Audience members included prison staff, governors, healthcare staff, and prisoners. A mixed method approach was used to evaluate the project. Participant's pre and post project self-confidence

WHAT IS KNOWN ON THE SUBJECT?: Recovery-oriented care is a means of providing mental health treatment, focused on the patient's individual needs and active involvement in one's own care. However, this approach presents with challenges, particularly in psychiatric hospitals, which tend to be focused on symptom reduction. WHAT DOES THE PAPER ADDS TO EXISTING KNOWLEDGE?: This study examines the influence of three different recovery-oriented training programmes/interventions (namely, illness management and recovery, peer support, and psychiatric advance directives) on the attitudes and practice of

Shortcomings in the technology for training support staff in methods of teaching people with severe disabilities recently have resulted in calls to improve the technology. We evaluated a program for training basic teaching skills within one day. The program entailed classroom-based verbal and video instruction, practice, and feedback followed by on-the-job feedback. In Study I, four undergraduate interns participated in the program, and all four met the mastery criterion for teaching skills. Three teacher aides participated in Study 2, with results indicating that when the staff applied their

Background: International estimates suggest that up to one in three public safety personnel experience one or more mental disorders, including post-traumatic stress disorder (PTSD). Canadian data have been sparse until very recently, and correctional officers and forensic psychiatric staff have rarely been included. Working as a correctional officer is associated with negative health outcomes and increased work-related stress, with several variables affecting reported levels of stress. Healthcare staff also report higher rates of PTSD, especially those who are exposed to aggression in their wo

RATIONALE, AIMS AND OBJECTIVES: Increasing evidence indicates that sex-specific issues may have impact on prevention, diagnosis, or treatment. These issues are not systematically considered during the development of Dutch clinical practice guidelines. The aim of this study is to identify how members of guideline development groups discuss sex-specific evidence, and whether and how the outcomes of these discussions are reflected in the guideline.
METHODS: Six guideline development committees (GDCs) were studied. Each committee was supported by a staff member from the guideline organization who

Physical restraint persists at high rates in human services settings supporting individuals with intellectual and developmental disabilities (IDD), even within organizations that have formally adopted positive behavior support (PBS) frameworks and invested substantially in staff training. This perspective article introduces the Staff Decision Ecology Model, a decision ecology account of physical restraint, which conceptualizes restraint as a context-dependent, staff-mediated decision shaped by nested individual, environmental, and organizational variables, and argues that leadership functions

BACKGROUND: During the COVID-19 pandemic, frontline care workers in day centre services for adults with intellectual disabilities rapidly adapted their roles through use of digital technologies. This study examines how these developments shaped professional roles and identities.
METHOD: The study draws on qualitative interviews with 14 frontline staff and 5 managers in day centres that shifted from on-site to remote services. Thematic analysis was guided by the concepts of boundary work and professional capital.
RESULTS: Digitalisation temporarily reshaped professional identity by enabling i

The COVID-19 pandemic led to unprecedented challenges to healthcare quality in the emergency department, including directly impacting quality metrics and worsening barriers to the quality improvement process such as burnout, staff turnover, and boarding. We aimed to develop a blueprint for postpandemic quality improvement to address these specific barriers, focused on prioritizing frontline staff engagement from idea generation to implementation and assessment. Drawing from teamwork literature, we constructed a process that emphasized egalitarian conversations, psychological safety, and creati

We developed a clinical decision support system--ventilation protocols--that managed tidal volume and ventilator rate settings during mechanical ventilation of patients with the Adult Respiratory Distress Syndrome (ARDS). We applied these protocols for a total of 10,903 hours in 40 ARDS patients. The clinical staff suspended the protocols for only 5% of the total application time due to medical procedures, surgeries, transient clinical problems not addressed by the protocols, or because of attending physician request. Of 3,148 instructions generated by the ventilation protocols, the clinical s

To emerge from a significant quality crisis, hospital administration recognized the need for physician leadership to drive improvements. A framework is presented for a physician-led Quality Summit to select best practice initiatives for implementation over 1 year. Results demonstrated statistically significant reductions in ventilator-associated pneumonia, decreasing from the first quarter 2009 baseline of 8.34 per 1000 ventilator days to 3.32 per 1000 ventilator days in second quarter 2010 (P = .0055). During the same time frame, catheter-associated urinary tract infections decreased from 4.3

To administer a flexible-working-time system in a busy medical microbiology department, without increased administrative workload, a set of programs was produced for computer monitoring of staff time-keeping records. The programs--initially written to facilitate management and minimise clerical work, and extended to encompass all staff absences--are collectively called MANAGER. They allow efficient management of working hours and staff absences, using a multi-user laboratory computer. The programs were all written in MUMPS and evolved in-house, but could be developed for any multi-user laborat

BACKGROUND: Adolescents with haematological malignancies face significant emotional and relational challenges, often accompanied by difficulties in communicating their needs within the healthcare context. To address these issues, a narrative-based psycho-educational intervention based on the creation and prescription of Ironic Medications was developed. These fictional remedies were designed to express emotions, experiences and needs in a playful yet meaningful way while exploring differences in how adolescents and healthcare staff perceive communication and relational needs.

BACKGROUND: Healthcare worker burnout is a national challenge, with implications for healthcare worker well-being and mental health, the quality of patient care, and healthcare costs. The Clinician Wellness Council (CWC), a 15-month well-being leadership development training program, was designed and implemented to provide practicing clinicians ("CWC members") across several clinical groups within the Providence health system with skills, coaching, and community to address the local drivers of their clinical groups' burnout. The purpose of this study was to explore CWC members' experiences wit

The transition to the assessment of entrustable professional activities as part of competency-based medical education (CBME) has substantially increased the number of assessments completed on each trainee. Many CBME programs are having difficulty synthesizing the increased amount of assessment data. Learning analytics are a way of addressing this by systematically drawing inferences from large datasets to support trainee learning, faculty development, and program evaluation. Early work in this field has tended to emphasize the significant potential of analytics in medical education. However, c

Nurse-to-nurse, or horizontal, hostility creates a multitude of problems. There are a number of specific and direct problems, including impact on the culture of nursing, stress, healthcare economics, and patient outcomes, that are illustrated in the literature. Horizontal hostility is a pervasive and destructive force that must be addressed in every venue in which nursing is involved. This study discusses the problem of horizontal hostility and the impact it has on nursing and healthcare. Nursing theory is applied to practice and provides concepts and frameworks that are utilized to underscore
