Journal of cystic fibrosis : official journal of the European Cystic Fibrosis Society2009eng
BACKGROUND: Achromobacter xylosoxidans infection may cause conspicuous chronic pulmonary inflammation in cystic fibrosis (CF) patients similar to Pseudomonas aeruginosa and the Burkholderia cepacia complex (Bcc). Evolution in lung function was compared in chronically infected patients. Cytokine concentrations in CF patients with and without chronic infection were compared to healthy controls.
METHODS: Cytokines in serum and sputum were measured using multiplex bead based immunoassay.
RESULTS: Sixty CF patients, 11 with A. xylosoxidans, 11 with Bcc, 21 with P. aeruginosa and 17 non-infected C

Achromobacter xylosoxidans is a rare, opportunistic infection most commonly encountered in immunocompromised patients during hospitalization. Primary uncomplicated bacteremia, catheter-associated infections, and pneumonia have been reported as the most common clinical presentations; skin and soft tissue infections from A. xylosoxidans are rare. We describe a case of A. xylosoxidans presenting as cellulitis and bacteremia in an immunocompromised patient.

Achromobacter xylosoxidans, a rare gram-negative rod, often causes pneumonia. Here, we describe the case of a previously healthy 78-year-old woman who worked at a vegetable plant nursery. A 4.5-cm cavitary lesion with a 2.5-cm-diameter lobulated nodule in the left lingula segment, resembling an aspergillosis fungus ball, was coincidentally detected on chest computed tomography. Bronchoscopic culture eventually revealed A. xylosoxidans, which was sensitive to penicillin; however, the infection did not respond to oral antibiotics. Thus, surgical resection is likely to be indicated for A. xylosox

The rise of antimicrobial resistant (AMR) bacteria is a global public health threat. AMR Achromobacter bacteria pose a challenging clinical problem, particularly for those with cystic fibrosis (CF) who are predisposed to chronic bacterial lung infections. Lytic bacteriophages (phages) offer a potential alternative to treat AMR infections, with the possible benefit that phage selection for resistance in target bacteria might coincide with reduced pathogenicity. The result is a genetic "trade-off," such as increased sensitivity to chemical antibiotics, and/or decreased virulence of surviving bac

PURPOSE: To report Achromobacter xylosoxidans as a cause of both acute-onset and delayed-onset postoperative endophthalmitis after cataract surgery.
METHODS: A noncomparative consecutive case series of patients with culture-proven A. xylosoxidans endophthalmitis between 1970 and 2012. Cataract surgery and intraocular lens placement were performed in all patients before endophthalmitis. Positive cultures were obtained from the vitreous, capsular bag, and/or the removed intraocular lens.
RESULTS: The clinical diagnosis was confirmed in four patients with positive cultures. Two patients with en

Liver cirrhosis results from progressive hepatic fibrosis and is generally considered irreversible. One of the many consequences of cirrhosis is spontaneous bacterial peritonitis. This typically presents in patients with decompensated cirrhosis due to bacterial translocation, most commonly from the intestinal bacterial flora seeding into the ascitic fluid. We present a rare case of spontaneous bacterial peritonitis caused by Achromobacter xylosidans. This bacterium is mostly associated with nosocomial infections, and due to its multidrug-resistant nature, treatment options are often limited. T

We herein discovered a highly resistant clinical isolate of Pseudomonas aeruginosa with MICs to amikacin, gentamicin, and arbekacin of 128 μg/mL or higher in a drug sensitivity survey of 92 strains isolated from the specimens of Yoka hospital patients between January 2009 and October 2010, and Achromobacter xylosoxidans was separated from this P. aeruginosa isolate. The sensitivity of this bacterium to 29 antibiotics was investigated. The MICs of this A. xylosoxidans strain to 9 aminoglycoside antibiotics were: amikacin, gentamicin, arbekacin, streptomycin, kanamycin, neomycin, and spectinomyc

Achromobacter xylosoxidans is a ubiquitous Gram-negative non-fermenting rod, recently characterized as an emerging pathogen in cystic fibrosis (CF) patients. Its pathogenic potential and prevalent transmission routes are still unclear. This study investigated the PFGE genetic pattern and antimicrobial resistance profile of 42 A. xylosoxidans isolates obtained over 4 years from the respiratory tract of 22 CF patients. By genotypic analysis, 31 isolates were attributed to 8 distinct PFGE patterns (A-H), whereas 11 isolates were not typable because their DNA was not restricted by XbaI and DraI re

BACKGROUND: The soil environment is a natural reservoir of antibiotic resistance genes (ARG). It plays a crucial role in the emergence and dissemination of antibiotic resistance determinants. Anthropogenic activities, such as using antibiotics in healthcare facilities, aggravate the proliferation of ARG in soil. Exploring the soil environment can help in gaining insight about the presence, prevalence, and distribution of antibiotic resistance determinants. In the current study, we tried to isolate antibiotic-resistant bacteria from soil samples collected from around the incinerator at Shri Mat

Achromobacter xylosoxidans is associated with resilient nosocomial infections, with bacteraemia, pneumonia and chronic cystic fibrosis lung infection being the most common clinical presentations. Innate multi-drug resistance and a suite of virulence factors select for A. xylosoxidans infection during long-term antibiotic therapy, contributing to its persistence, treatment recalcitrance, association with poor clinical outcomes and emergence as a problematic pathogen. Horizontal gene transfer and maintenance of large genomes underpin the resilience and cosmopolitan lifestyle of A. xylosoxidans,

A 31-year-old man was referred to us 2 months after bilateral laser in situ keratomileusis (LASIK). On presentation, the corrected distance visual acuity was hand motion in the right eye and 20/25 in the left eye. Slitlamp examination showed a diffuse central stromal infiltrate, flap melting, and hypopyon in the right eye and marked interface opacities with crystal-like edges in the left eye. Flap lift and irrigation were performed. Because of the progressive keratitis, penetrating keratoplasty (PKP) was done in both eyes. Achromobacter xylosoxidans was isolated from both corneal buttons, and

Change in sputum bacterial density has been used to measure efficacy in clinical trials of novel therapies for cystic fibrosis (CF); however, the baseline degree of day-to-day bacterial density change is not well understood. Recent data show that Pseudomonas density in CF sputum has the potential to vary considerably day-to-day even in the absence of antimicrobial treatment. Here we report daily variability of Burkholderia and Achromobacter density in 288 pairs of sputum samples obtained from eight adults with CF. Compared to similar previous analyses of Pseudomonas density, overall variabilit

Bacterial pneumonia causes significant morbidity and mortality especially in elderly and immunocompromised hosts. Achromobacter xylosoxidans denitrificans pneumonia is very rarely reported. However, the reported cases have been in patients who are either immunocompromised or have bronchiectasis. We hereby present a unique case of Achromobacter xylosoxidans denitrificans pneumonia in an immunocompetent patient with advanced chronic obstructive pulmonary disease (COPD). Our patient is a Caucasian male admitted with shortness of breath, fever and cough. Chest X-ray demonstrated right-sided infilt

INTRODUCTION: Achromobacter xylosoxidans is a gramnegative bacillus resistant to multiple antibiotics, present both in the environment and in hospitals. This study describes an outbreak of colonizations and infections by A. xylosoxidans in the urodynamics unit of the Puigvert Foundation.
METHODS: On November 11, 2022, a patient developed a fever, and A. xylosoxidans was detected in their urine and blood. The case was linked to a recent urodynamic study. As a result, all A. xylosoxidans cases since 2018 were reviewed, and inspections were conducted in the unit, along with the collection of 24

BACKGROUND: Achromobacter xylosoxidans is an emerging cause of intra-abdominal infections, typically after healthcare exposure or in patients with indwelling devices. Intrinsic multidrug resistance and biofilm formation impede empiric therapy and often necessitate source control (catheter/stent exchange or removal). Because guidance is scattered across case reports, we systematically reviewed published episodes to quantify healthcare exposure, antimicrobial selections and susceptibilities, use of source control, and outcomes.
METHODS: In accordance with PRISMA 2020, we searched PubMed, Scopus

Achromobacter species can cause opportunistic infections which are difficult to treat due to a variety of antimicrobial resistance (AMR) mechanisms. As such, antimicrobial susceptibility testing (AST) is a cornerstone to successful treatment for these organisms. Currently, there are no specific Clinical and Laboratory Standards Institute (CLSI) AST guidelines for Achromobacter species. The purpose of this study was to (i) establish tentative CLSI M45 MIC and disk diffusion (DD) breakpoints (BPs), (ii) evaluate the modified carbapenem inactivation method (mCIM), and (iii) understand the mechani

Achromobacter spp. are intrinsically resistant to multiple antibiotics and can also acquire resistance to those commonly used for the treatment of respiratory infections, especially in patients with cystic fibrosis. The aim of this study was to perform the genetic and biochemical characterization of AXC-2 from A. ruhlandii and to analyze all available AXC variants. Steady-state kinetic parameters were determined on a purified AXC-2 enzyme. It exhibited higher catalytic efficiencies towards amino-penicillins and older cephalosporins, while carbapenems behaved as poor substrates. Phylogenetic an

Achromobacter xylosoxidans, subspecies denitrificans, is a gram-negative rod recently implicated as an emerging cause of infection in both immunosuppressed and immunocompetent populations. Few cases are reported in literature involving multiple body systems. Diagnosis depends on cultures of appropriate specimens, and management usually is by administration of appropriate antibiotics (usually agents with antipseudomonal activity). We report a rare case of pneumonia due to infection with this organism, in a patient with preexisting bronchiectasis secondary to chronic aspiration.
