Regional Factors and Ambulatory Care-Sensitive Condition Hospitalizations in Older Japanese Adults.

IMPORTANCE: Hospitalization in older adults leads to a decline in daily living activities, and countries experiencing demographic aging face an increase in hospitalization rates and health care costs. Health care systems that can reduce the probability of ambulatory care-sensitive condition (ACSC) hospitalization among older populations are needed. OBJECTIVE: To comprehensively assess the association between regional factors and the probability of ACSC hospitalization among older adults and the extent to which regional factors explain this probability. DESIGN, SETTING, AND PARTICIPANTS: A re

Hospitalizations for ambulatory care sensitive conditions as an indicator of access to primary care and excess of bed supply.

BACKGROUND: Ambulatory care sensitive conditions (ACSC) hospitalization is a widely accepted measure of the access to primary care. However, given its discretionary characteristics, the ACSC hospitalization can be a measure reflecting the influence of hospital bed supply. In Korea, where the quality of primary care and oversupply of hospital beds are coexistent concerns, ACSC hospitalization can be used to examine the impact of both factors. This study was performed to investigate the ACSC hospitalization rate as a measure of the hospital bed supply as well as access to primary care. METHODS:

Emergency department visits for ambulatory care sensitive conditions by persons with Rheumatoid Arthritis: A population-based study.

PURPOSE: We estimated emergency department (ED) visit rates for Canadian-indicator Ambulatory Care Sensitive Conditions (ACSCs) by persons with rheumatoid arthritis (RA) relative to age- and sex-matched general population controls. METHODS: Cases were identified using a validated definition based on International Classification of Diseases codes (years 2002-2023). We identified visits in the National Ambulatory Care Reporting System (NACRS) where the most responsible diagnosis was for any ACSC (grand mal seizures, chronic lower respiratory diseases, asthma, diabetes, heart failure and pulmona

An evidence-informed Delphi study of ambulatory care sensitive conditions in China: a policy tool to assess primary care performance.

BACKGROUND: Ambulatory care sensitive conditions (ACSCs) serve as a critical indicator for assessing healthcare system performance globally. However, China lacks a standardized ACSCs list adapted to its unique healthcare context and evolving medical needs. METHODS: This study employed a modified Delphi method combined with evidence-based medicine. First, we systematically reviewed international ACSCs lists and their development methodologies to identify potential diseases. Next, we evaluated the evidence of these potential conditions within China's healthcare system. Finally, a two-round Delp

Big Data Analytics to Reduce Preventable Hospitalizations-Using Real-World Data to Predict Ambulatory Care-Sensitive Conditions.

The purpose of this study was to develop a prediction model to identify individuals and populations with a high risk of being hospitalized due to an ambulatory care-sensitive condition who might benefit from preventative actions or tailored treatment options to avoid subsequent hospital admission. A rate of 4.8% of all individuals observed had an ambulatory care-sensitive hospitalization in 2019 and 6389.3 hospital cases per 100,000 individuals could be observed. Based on real-world claims data, the predictive performance was compared between a machine learning model (Random Forest) and a stat

Implementation of a Protocol to Manage Patients at Risk for Hospitalization Due to an Ambulatory Care Sensitive Condition.

BACKGROUND: Ambulatory care sensitive conditions (ACSCs), such as type 2 diabetes mellitus, chronic obstructive pulmonary disease, hypertension, congestive heart failure, urinary tract infections, asthma, dehydration, bacterial pneumonia, angina without an in-hospital procedure, and perforated appendix put patients at risk for hospitalization. Currently at the William S. Middleton Memorial Veterans Hospital in Madison, Wisconsin, no standardized process or protocol exists that can identify and optimize primary care for patients with ACSCs who have been hospitalized but are predicted to be at l

Hospitalization for ambulatory care-sensitive conditions: a method for comparative access and quality studies using routinely collected statistics.

BACKGROUND: Appropriate and timely provision of ambulatory care is an important factor in maintaining population health and in avoiding unneccessary hospital use. This article describes conditions for which hospitalization rates have a strong and inverse relationship to access to high-quality ambulatory care. METHODS: Three panels of Canadian physicians following different consensus techniques selected conditions for which the relative risk of hospitalization is inversely related to ambulatory care access. PRINCIPAL FINDINGS: All panels identified asthma, angina pectoris, congestive heart fa

Development of indicators to assess quality and patient pathways in interdisciplinary care for patients with 14 ambulatory-care-sensitive conditions in Germany.

BACKGROUND: In settings like the ambulatory care sector in Germany, where data on the outcomes of interdisciplinary health services provided by multiple office-based physicians are not always readily available, our study aims to develop a set of indicators of health care quality and utilization for 14 groups of ambulatory-care-sensitive conditions based on routine data. These may improve the provision of health care by informing discussions in quality circles and other meetings of networks of physicians who share the same patients. METHODS: Our set of indicators was developed as part of the l

Age-Standardized Incidence of Emergency Hospital Admission of Ambulatory Care Sensitive Conditions Was Reduced 54% in Finland, 2002-2024.

We describe annual trends of age-standardized periods of care of ambulatory care sensitive conditions (ACSC) in a 23-year time series (2002-2024) in Finland using a national Sotkanet Indicator Bank information. Overall, we observed a reduction of 54% of the baseline value in ACSC period of care during the study period. The decrease of period of care was associated with the COVID-19 pandemic years (2020-2022) in Finland. Forecast estimates of annual ACSC period of care in 2020-2024 were significantly higher than the actual numbers. During the first two years (2023-2024) of the major healthcare,

Admissions for ambulatory care sensitive conditions on rural islands and their association with patient experience: a multicentred prospective cohort study.

OBJECTIVES: The rate of admissions for ambulatory care sensitive conditions (ACSCs) is a key outcome indicator for primary care, and patient experience (PX) is a crucial process indicator. Studies have reported higher rates of admission for ACSCs in rural areas than in urban areas. Whether there is an association between admissions for ACSCs and PX in rural areas has not been examined. This study aimed to document admissions for ACSCs on Japanese rural islands, and assess whether there was an association between the rate of admissions for ACSCs and PX. DESIGN: Multicentred, prospective, cohor

The association between care modality and hospitalizations and emergency department visits for ambulatory care-sensitive conditions during and after the pandemic in Ontario, Canada.

The COVID-19 pandemic required a rapid transition to virtual care as a key strategy to maintain healthcare access while minimizing virus transmission risks. However, the impact of this shift on hospitalizations and emergency department (ED) visits for ambulatory care-sensitive conditions (ACSCs) remains unclear. This study aims to assess the relationship between the modality of outpatient care for ACSCs and their outcomes in Ontario, Canada. In this population-based retrospective cohort study, we analyzed hospitalization and ED visit data for ACSCs, including diabetes, epilepsy, congestive hea
