AL amyloidosis belongs to the group of conformational diseases. It is the most common type of amyloidosis with an estimated 500 new cases per year in France. It is due to a small and usually indolent plasma cell clone which synthesizes an unstable, misfolded monoclonal immunoglobulin light chain that is prone to aggregate and form amyloid fibrils. Non-invasive biopsy such as abdominal fat aspiration or minor salivary gland biopsy should be performed to confirm the diagnosis and if negative, involved tissues have to be examined. Clinical presentation is very diverse, as AL amyloidosis can affec

AL amyloidosis is a systemic disease characterised by pathogenetic proteins produced by malignant plasma cells and the deposition of them in different organs of the body. Amyloidogenic protein is the light chain of the monoclonal immunoglobulin, which becomes water insoluble, precipitates and deposites in the extracellular space resulting damage of organ function. AL amyloidosis belongs to plasma cell dyscrasias or it can associate to other monoclonal B-cell diseases. Diagnosis - such as in case of other types of amyloidosis - is based on histology. Identification of the amyloidogenic protein

Amyloidosis is generally classified as either systemic or cutaneous, with both primary and secondary forms. There are also heredofamilial and hemodialysis-associated varieties of amyloidosis, all with specific amyloid fibril derivatives. Nodular cutaneous amyloidosis is the most rare form of primary cutaneous amyloidosis. Lesions typically present as a crusted nodule on the face, extremities, or acral sites. The amyloid fibrils are immunoglobulin-derived and either kappa or lambda light chains. Systemic involvement is dependent on plasma cell amyloid protein deposition. Lesions may otherwise b

Renal amyloid deposition is common in systemic amyloidosis. Presentation is usually with proteinuria renal impairment. With effective treatment of the underlying amyloidotic condition and good supportive care renal function can stabilize or improve but many patients still progress to end-stage renal failure.

Amyloidoses are a heterogeneous group of multisystem disorders, which are characterized by an extracellular deposition of amyloid fibrils. Typically affected are the heart, liver, kidneys, and nervous system. More than half of the patients die due to cardiac involvement. Clinical signs of cardiac amyloidosis are edema of the lower limbs, hepatomegaly, ascites and elevated jugular vein pressure, frequently in combination with dyspnea. There can also be chest pain, probably due to microvessel disease. Dysfunction of the autonomous nervous system or arrhythmias may cause low blood pressure, dizzi

Inflammatory amyloidosis consists of AA protein. In developed countries, its predominant causes are chronic inflammatory rheumatism (rheumatoid arthritis, chronic juvenile arthritis, spondylarthropathy, etc.). More rare are other chronic inflammatory disorders (Crohn's disease, ulcerative colitis, Whipple's disease, etc.), hemopathies and neoplasia. In developing countries, infectious causes are the most common (tuberculosis, leprosy, chronic bacterial suppuration). Other infectious diseases complicated with amyloidosis are less common (bronchial dilatation, mucoyiscidosis, heroin injection-re

Although bullous amyloid lesions are very rare, the cutaneous lesions of this type can be a crucial manifestation of plasma cell dyscrasia. [Case Report] A 66-year-old man with a six-year history of multiple myeloma (IgG-lambda, lambda-type Bence Jones proteins) was admitted to the hospital because of hemorrhagic bullous lesions of the skin, chronic diarrhea and general malaise. A diagnosis of myeloma-associated amyloidosis with renal failure was made. One month later, he died as a consequence of progressive renal failure and systemic amyloidosis. A postmortem examination revealed myelomatous

An unusual intraoral form of amyloidosis, the nodular form, is discussed. Attention is directed to clinical, microscopic and immunohistochemical features. More specifically, the nodular form of this condition and the specific relationship in this patient to chronic hemodialysis and associated beta-2-microglobulin deposition is highlighted. An overview of other forms of amyloidosis is provided along with the chemical identity of each.

A series of 29 cases of amyloidosis of the alimentary tract is reported. Five cases (17%) were primary amyloidosis; 14 cases (48%) were amyloidosis secondary to other diseases (such as chronic inflammatory and neoplastic diseases); 10 cases (35%) were amyloidosis of the heredo-familial type connected with Familial Mediterranean Fever. In 23 patients (79%) the diagnosis was established by biopsies, and in 6 more cases on autopsy. Gastrointestinal involvement was found in all age groups. Gastro-enterologic complications observed in the present series include: diarrhea, malabsorption, ileus and g

We reviewed the clinical records of 769 patients with primary systemic amyloidosis who had been examined at Mayo Clinic Jacksonville (Jacksonville, Florida) or Mayo Clinic Rochester (Rochester, Minnesota) during a 12-year period (1978 through 1989). Of these 769 patients, 59 (8%) had biopsy-established gastrointestinal amyloidosis, and 8 (1%) had symptomatic gastric amyloidosis. All eight patients with symptomatic gastric amyloidosis had hematemesis or prolonged nausea and vomiting in association with weight loss. Additional findings were gastroparesis (in three patients), gastric tumor (in on

A patient with primary amyloidosis of the upper air passages was found to have cutaneous macular amyloidosis. A review of the literature indicates that this may represent the first report of these two uncommon forms of amyloidosis occurring in the same individual. Biopsy material from the nasopharynx, tonsillar areas, larynx, trachea, and skin showed amyloid in the tissue sections. Special stains were used and polariscopic studies were performed, and the hyalin material met the established criteria for amyloid. Studies of tissues from other parts of the body of this patient failed to demonstra

Elastosis amyloidosis is an exceptionally rare form of cutaneous amyloidosis characterized by amyloid deposition around dermal elastic fibers. To date, only 11 cases have been documented in the literature, some of them with associated systemic involvement. We report a case of a 60-year-old man presenting with an asymptomatic umbilical nodule. Histopathological analysis revealed amyloid deposits in the upper reticular dermis with characteristic distribution around the elastic fibers, confirmed by positive staining with PAS and Congo red. Immunohistochemical staining was positive for cytokeratin

PURPOSE: Hereditary transthyretin amyloidosis (hATTR amyloidosis) is a progressive disease primarily characterized by adult-onset sensory, motor, and autonomic neuropathy. In this article, we discuss the pathophysiology and principal findings of autonomic neuropathy in hATTR amyloidosis, the most common methods of assessment and progression, and its relation as a predictive risk factor or a measure of progression in the natural history of the disease.
METHODS: A literature search was performed using the terms "autonomic neuropathy," "dysautonomia," and "autonomic symptoms" in patients with he

Systemic amyloidosis has recently emerged as a major cause of nephropathy among heroin abusers in New York City. Although focal glomerulosclerosis is typically seen in intravenous drug abusers who present with the nephrotic syndrome, those who escape this complication are at risk for the later development of amyloidosis related to their use of the subcutaneous route. Twenty such addicts identified between 1981 and 1984 are described. Patients typically present with chronic suppurative skin infections, edema, the nephrotic syndrome, benign urinary sediment, and normal-sized or enlarged kidneys.

Hematologists have a significant role in the diagnosis and management of light-chain amyloidosis and are frequently involved in the diagnosis of transthyretin and other more rare types of amyloidosis. Recent advances in diagnostic techniques and therapies are dramatically improving patient prognosis. Radionuclide imaging methods are emerging as a highly specific and noninvasive way to diagnose, quantify, and monitor organ involvement representing a major advance in amyloidosis management.

Ureteral amyloidosis is a rare entity and of interest to urologists, hematologists, radiologists, and pathologists because it mimics urothelial cell carcinoma clinically, endoscopically and radiologically. A pre-operative ureteroscopy or surgical biopsy is required, and it is essential to exclude systemic amyloidosis. We report a male who was diagnosed with IIIA stage lymphoplasmacytic lymphoma associating systemic amyloidosis with concomitant hematuria. Urine cytology was negative and computerized tomography urography (CTU) scan evidenced bilateral, proximal and medium, ureteral stenosis and
