Prospective randomized clinical trial comparing patient-controlled intravenous analgesia with patient-controlled epidural analgesia after lumbar spinal fusion.

STUDY DESIGN: A prospective, randomized, double-blind clinical trial was conducted. OBJECTIVE: To compare the efficacy of patient-controlled analgesia (PCA) with that of patient-controlled epidural analgesia (PCEA) in terms of overall patient satisfaction with postoperative pain management after lumbar spine fusion. SUMMARY OF BACKGROUND DATA: In numerous surgical disciplines, PCEA and PCA have proved to be effective methods of postoperative pain control. The literature states that with PCEA, less opioid use is required during the immediate postoperative period to maintain equivalent pain co

Postoperative pain therapy with hydromorphone; comparison of patient-controlled analgesia with target-controlled infusion and standard patient-controlled analgesia: A randomised controlled trial.

BACKGROUND: The challenge of managing acute postoperative pain is the well tolerated and effective administration of analgesics with a minimum of side effects. The standard therapeutic approach is patient-controlled analgesia (PCA) with systemic opioids. To overcome problems of oscillating opioid concentrations, we studied patient-controlled analgesia by target-controlled infusion (TCI-PCA) as an alternative. OBJECTIVE: To compare efficacy, safety and side effects of standard PCA with TCI-PCA for postoperative pain therapy with hydromorphone. DESIGN: Single-blinded, randomised trial. SETTIN

Patient-controlled analgesia for children at home.

CONTEXT: Pain is a common and significant symptom experienced by children with advanced malignant disease. There is limited research on pain management of these children at home. OBJECTIVES: To describe and review the indications for using patient-controlled analgesia (PCA) in the form of a Computerized Ambulatory Drug Delivery device (CADD(®)) in the home setting. METHODS: A retrospective chart review was conducted in children discharged home with opioid infusions using a CADD. Charts from January 2008 to February 2012 were surveyed. RESULTS: Thirty-seven CADDs were dispensed during the st

Patient-controlled analgesia and intra-operative suggestion.

In a double-blind randomised study, we investigated the influence of positive intra-operative suggestions, presented to anaesthetised patients undergoing total abdominal hysterectomy, on postoperative pain, nausea and vomiting. One hundred and forty patients were randomly allocated to listen to one of four tapes containing either white noise or positive suggestions. The positive suggestions related to pain, postoperative nausea and vomiting, or both. We found that the positive intra-operative suggestions had no beneficial effects in reducing postoperative pain or nausea scores, nor was the con

Patient-controlled lidocaine analgesia for acromioplasty surgery.

Twenty-four consecutive patients undergoing shoulder acromioplasty were given postoperative analgesia with a new method in which a patient-controlled continuous infusion of lidocaine infiltrated the subacromial space. Seventeen of the acromioplasties were done with arthroscopy, whereas 7 were performed with an open procedure. A 2% solution of lidocaine without epinephrine was used for both a continuous dose of 2 cc/h and patient-controlled interval doses of 1 cc administered at 15-minute intervals. The catheter was left in place for 72 hours. We prospectively studied complications, the patient

Patient-controlled sedation and analgesia during SWL.

Sixty unpremedicated outpatients undergoing elective extracorporeal shockwave lithotripsy (SWL) using a Dornier MPL 9000 lithotripter were randomly assigned to receive either propofol-alfentanil (PA group; N = 30) or midazolam-alfentanil (MA group; N = 30) by a patient-controlled analgesia (PCA) device for sedation and analgesia. Although pain intensity scores were lower after 20 minutes and sedation was more pronounced in the MA group, both drug regimens produced satisfactory sedation and analgesia and allowed the maximum number of shockwaves to be given. Alfentanil consumption was less in th

Patient-controlled analgesia with fentanyl in children.

We have found that fentanyl delivered by PCA provides an effective alternative when adverse effects occur with other more frequently used narcotics. Although anecdotal experience exists concerning the use of fentanyl PCA in adults, dosing guidelines in children must depend on consideration of the current narcotic regimen and the use of equipotent doses of fentanyl. Our initial experience suggests that this is a safe and reliable technique; however, until prospective studies further delineate dosing guidelines, close observation (continuous pulse oximetry and hourly checks of respiratory rate)

Postprocedure Intravenous Patient-Controlled Analgesia Compared to Epidural Patient-Controlled Analgesia Use after Uterine Fibroid Embolization: A Clinical Audit.

PURPOSE: Postprocedure epidural analgesia has a proven benefit over intravenous (i.v.) analgesia for pain management, but has not yet been demonstrated for uterine fibroid embolization (UFE). The objectives of this clinical audit were to determine if epidural patient-controlled analgesia (PCA) was beneficial to patient outcome as compared to i.v. PCA in decreasing parenteral opioid requirements and its associated side effects and determine if there is a difference in required oral opioids after the PCA is stopped. MATERIALS AND METHODS: This manuscript audited postprocedural pain management o

New epidural techniques for labour analgesia: patient-controlled epidural analgesia and combined spinal-epidural analgesia.

Epidural analgesia in labour aims to provide high-quality pain relief of rapid onset and prolonged duration, while minimizing both maternal side-effects, particularly impairment of mobility, and impact on the fetus or on the outcome of labour. In conjunction with pharmacological research on spinal analgesics (local anaesthetics, opioids and other drug classes), refinement of new or established drug delivery techniques has allowed progress toward more reliable and improved pain relief at all stages of labour and childbirth, reduced individual drug doses with reduction of unwanted effects and gr

[Patient-controlled analgesia by the peridural route and classical methods of analgesia].

Patient controlled analgesia was developed to compare drug effects. Later its psychological implications were studied. Drug administration by intravenous or epidural injection has been used. The latter provides relief with much smaller doses. Simultaneous use of opioids and anti-inflammatory drugs enhances the analgesic effects. For pain relief in childbirth, opioids need to be combined with local anaesthetics for best results. Little has been achieved by patient controlled analgesia in the treatment of chronic pain.

A micro-costing study: comparing a standard intravenous patient-controlled analgesia system with a novel sublingual patient-controlled analgesia system.

BACKGROUND: Judicious spending in healthcare is of paramount importance, particularly when introducing new devices or interventions. These products or interventions need to be economically efficient both directly and indirectly. An accepted method of cost estimation is micro-costing. Micro-costing involves direct enumeration and costing of every input consumed in the treatment of a particular patient when using new device, medicine or intervention. In our study, we investigated the cost of using a novel sublingual (SL) patient-controlled analgesia (PCA) device and compared it with our conventi

Outpatient Patient Controlled Analgesia (PCA) for the Palliative Care Patient.

This study explores the under-researched domain of patient-controlled analgesia (PCA) for cancer pain management in adult outpatients, focusing on the transition from patient-controlled analgesia pumps (PCA pump) to oral medications. While existing literature primarily addresses the use of PCA in inpatient settings, this descriptive study investigates the initiation of outpatient PCA in palliative care patients. The retrospective chart review includes data from all admissions between July 1, 2014, and December 31, 2020. Among the 49 identified patients, 41 were admitted for cancer-related pain

Using remifentanil in labour via patient-controlled analgesia.

Pain relief during labour is problematic. The expectations of the mother have to be balanced against compromising the baby. A new drug that has been used in labour is remifentanil. This paper examines the indications for its use and the possible place it may find in providing another pain-relieving option for labouring women.

Sickle cell disease patients and patient-controlled analgesia.

Sickle cell disease (SCD) is one of the world's commonest hereditary disorders. Painful episodes are the overriding manifestation. SCD pain is largely opioid-sensitive and, in severe cases, adult patients warranting hospitalization often need parenteral opioids. Pain is essentially subjective and is often best controlled by patients themselves. This study looks at SCD patients' perceptions of self-administering parenteral opioids. Using a mainly multiple-choice questionnaire and a focus group interview, data were gathered from 40 adult SCD patients during hospitalization over a 4-month period.

[Patient-controlled postoperative analgesia in obstetrics and gynaecology].

A prospective study researches features of patient-controlled epidural analgesia (PCEA) in 42 women after cesarean section, mid- and high-traumatic gynaecological surgery. It was found out that the quality of while using the PCEA is significantly better than while using a traditional parenteral infusion of analgesics. Use of PCEA favours the circulatory and breathing systems functioning. Compared to continuous epidural infusion of analgesics and anaesthetics, use of PCME returns the expenses better due to less consumption of the drugs up to 35% during the first 24 hours after the surgery. Post-operative epidural versus intravenous patient-controlled analgesia.

Patient-controlled analgesia techniques have opened a new dimension to individualize patient's need for analgesia, in the treatment of acute post-operative pain. These techniques can be used intravenously, in the epidural space, and into peripheral nerve sheets. There is a common consensus that intravenous patient-controlled analgesia should not have a continuous infusion while epidural patient-controlled analgesia (PCEA) should be programmed with a continuous infusion. The drugs used for epidural analgesia are: opioids, local anaesthetics or the combination of both. The combinations seem to p
