[Wolff-Parkinson-White syndrome. Outcome of patients treated with anti-arrhythmia agents from data of electrophysiological examinations].

Seventy-two consecutive patients with electrocardiographic evidence of Wolff-Parkinson-White syndrome underwent electrophysiological study (EPS). Fifty-five of these patients (76 p. 100) had episodes of tachycardia, 11 experienced palpitations or syncopes and 6 were asymptomatic. The decision to prescribe an antiarrhythmic agent was reached on the basis of the patients' symptoms and EPS data. One patient was treated by surgery before the medical treatment was tried; 17 patients were discharged without treatment, 4 were discharged with an episodic and 50 with a preventive antiarrhythmic treatme

[Experimental and initial clinical experiences with the new class Ib anti-arrhythmia agent barucainide].

Barucainide is a new class Ib-antiarrhythmic compound which causes a reduction of the upstroke velocity and shortens the duration of the action potential in a dose-dependent fashion. The electrophysiological, hemodynamic and antiarrhythmic effects of the new drug were analyzed in experimental and clinical investigations. The results show that barucainide, in a dose range varying between 0.125-4.0 mg/kg i.v. fails to exert any significant effect on cardiac conduction and refractory times, heart rate, arterial blood pressure, cardiac output and left ventricular contractility. In a standardized a

[Autonomic effects of scaritoxin in the cat and guinea pig. Cardiac protection by an anti-arrhythmia agent].

A purified extract from the muscles of the fish Scarus gibbus (93% scaritoxin and 7% ciguatoxin) was tried on anesthetized cats and isolated guinea-pig atria. Intra-venous injection to cats (10, 20 and 40 mg/kg) induced deep respiratory depression, inhibition of duodenal contractions, increased salivary secretion, mydriasis and lacrymal secretion. Heart rate was slowed by 17 to 44 per cent. Impaired cardiac conduction led to different types of blocks. On the contrary, cardiac excitability was enhanced, as evidenced by the occurence of many auricular and mostly ventricular extra-systoles, often

[Effect of the anti-arrhythmia agent ethmozin on the refractory period of the accessory pathway in Wolff-Parkinson-White syndrome].

Sudden onset of atrial fibrillation can be very dangerous in patients with WPW syndrome, because it may result in high ventricular rate. The ventricular rate depends upon the length of the effective refractory period (ERP) of the accessory pathway. It is appropriate, therefore, to test newer antiarrhythmic agents on their effect on the ERP of the accessory bundle. 13 patients underwent the studies. Ethmozin was given intravenously 1 mg/kg body weight. The ERP increased significantly after Ethmozin (p less than 0.05). The increase was more prominent in those patients who had longer basal values

[New aspects of the clinical use of anti-arrhythmia agents with special reference to acute therapy of ventricular tachycardia (lidocaine vs. ajmaline)].

Antiarrhythmic treatment is based on the hypothesis that ventricular premature beats (VPBs), in the presence of underlying cardiac disease and impaired ventricular function, may predispose to sudden cardiac death. The effectiveness of treatment, however, has not been proven. For acute treatment of paroxysmal ventricular tachycardia, on comparison of the effectiveness of lidocaine and ajmaline, some new aspects have been rendered. VENTRICULAR PREMATURE BEATS (VPB): Isolated VPBs can be found in 40 to 75% of healthy subjects; if their number is substantial, investigation is warranted. For VPBs w

Anti-arrhythmic agents in ischemic heart disease: supraventricular arrhythmias, digitalis toxicity and chronic stable ventricular ectopic beats.

Digoxin remains a very useful agent for chronic atrial fibrillation or for the ectopic beats associated with heart failure. But when rapid control of the ventricular rate is required to arrhythmias such as atrial fibrillation, atrial flutter, or paroxysmal atrial tachycardia, a slow infusion of verapamil is the agent of choice. In general, verapamil may be added to digoxin or given intravenously while a digoxin effect is awaited, unless there is digitalis toxicity. In digitalis toxicity, lignocaine remains the agent of choice for ventricular arrhythmias, and is given in the same doses as for a

Protective effects of saffron (Crocus sativus) against lethal ventricular arrhythmias induced by heart reperfusion in rat: a potential anti-arrhythmic agent.

CONTEXT: Saffron (Crocus sativus L.) has been used as a cuisine spice in eastern and western societies for thousands of years. In traditional medicine, saffron is recommended for the treatment of various kinds of disorders including heart palpitations. OBJECTIVE: We investigated the hypothesis of the protective effect of saffron on lethal cardiac arrhythmias induced by heart ischemia-reperfusion in rat. MATERIALS AND METHODS: Animals were divided into a control (CTL) group that received tap water, Saf50, Saf100 and Saf200 groups that were orally treated with aqueous extracts of saffron, at d

The role of inflammation in the pathogenesis and treatment of arrhythmias.

The pathogenesis of arrhythmias is complex and multifactorial. The role of inflammation in the pathogenesis of both atrial and ventricular arrhythmias (VA) has been explored. However, developing successful pharmacotherapy regimens based on those pathways has proven more of a challenge. This narrative review provides an overview of five common arrhythmias impacted by inflammation, including atrial fibrillation (AF), myocardial infarction, arrhythmogenic cardiomyopathy, cardiac sarcoidosis, and QT prolongation, and the potential role for anti-inflammatory therapy in their management. We identifi

Antimicrobial agents-associated with QT interval prolongation.

QT interval prolongation is one of the most important causes of withdrawal of drugs from the market, due to its association with Torsades de Pointes (TdP), a potentially fatal arrhythmia. Although many antimicrobial drugs are capable of inducing this type of arrhythmia, the importance of this effect is usually underestimated. Macrolides, quinolones, azoles, pentamidine, protease inhibitors, antimalarial drugs and cotrimoxazole are the anti-infective agents more frequently associated with this adverse effect. Despite the fact that the risk of QT prolongation and TdP under single antimicrobial t

Anti-ischemic intervention as prognosis improvement in patients with coronary artery disease, with special focus on verapamil.

Angina pectoris is a significant risk predictor in patients with atherosclerotic heart disease. The major complications are myocardial infarction, heart failure, and arrhythmias. Plaque rupture turns stable angina pectoris into acute coronary syndrome by provoking platelet aggregation and thereby thrombus formation. Verapamil significantly inhibits platelet aggregation and thrombus formation, which may be one of several reasons for the protective effect of verapamil on reinfarction in patients recovering from myocardial infarction. Ischemia may lead to left ventricular dilation and diastolic d

Coronavirus Disease 2019 and Hypertension: How Anti-hypertensive Drugs Affect COVID-19 Medications and Vice Versa.

BACKGROUND: As a medical problem, hypertension is one of the most common disorders in cardiovascular disease. High blood pressure has been identified as one of the most familiar risk factors for the ongoing COVID-19 pandemic. We planned to explore the possible interactions between anti-hypertensive agents and drugs targeting SARS-CoV-2 with broad investigations of these medications' mechanism of action and adverse effects. METHODS: Two co-authors searched the electronic databases (PubMed, Scopus, and Google Scholar) to collect papers relevant to the subject. The keywords searched were angiote

Management of unstable arrhythmias in cardiogenic shock.

Atrial and ventricular arrhythmias commonly arise in the setting of cardiogenic shock and often result in hemodynamic deterioration. Causative factors include myocardial ischemia, volume overload, and metabolic disturbances. Correcting these factors plays an important role in managing arrhythmias in this setting. Ventricular arrhythmias are more ominous compared to atrial arrhythmias but both require prompt intervention with electrical shock and anti-arrhythmic drug suppression. Coronary reperfusion is key to improving survival, including reducing the risk of sudden cardiac arrest, in acute my

Advances in Antiarrhythmic Drug Therapy: Potentials and Challenges.

Antiarrhythmic drug therapy continues to be the cornerstone of management of both atrial and ventricular arrhythmias. Advances in the field were slowed by regulatory, scientific, clinical, and economic barriers. This review outlines the impediments to antiarrhythmic drug innovation and explores future therapies, including novel delivery platforms, atrial-selective drugs, and hybrid approaches. As the global prevalence of arrhythmias continues to rise, renewed investment in patient-centered, mechanism-driven pharmacotherapy is essential to complement existing procedural options and improve long

Nonreentrant atrial tachycardia occurs independently of hypertrophic cardiomyopathy in RASopathy patients.

Multifocal atrial tachycardia (MAT) has a well-known association with Costello syndrome, but is rarely described with related RAS/MAPK pathway disorders (RASopathies). We report 11 patients with RASopathies (Costello, Noonan, and Noonan syndrome with multiple lentigines [formerly LEOPARD syndrome]) and nonreentrant atrial tachycardias (MAT and ectopic atrial tachycardia) demonstrating overlap in cardiac arrhythmia phenotype. Similar overlap is seen in RASopathies with respect to skeletal, musculoskeletal and cutaneous abnormalities, dysmorphic facial features, and neurodevelopmental deficits.

The peri-operative management of atrial fibrillation.

Atrial fibrillation is a common arrhythmia frequently seen in surgical patients. The onset of new atrial fibrillation during the peri-operative period is less common. There are many possible precipitating factors, although volatile agents themselves may have an antifibrillatory action. The management of atrial fibrillation includes removal of any precipitating factors and treatment of the arrhythmia itself. Immediate management of acute-onset atrial fibrillation is usually direct current cardioversion. Alternatively, anti-arrhythmic drugs can be used to achieve cardioversion. In patients with

[Nonparoxysmal tachycardia and accelerated ectopic rhythms].

Ectopic rhythms with impulse frequency of 60-90 per minute are referred to as "accelerated"; those with the frequency of 91-130 impulses per minute are termed "nonparoxysmal tachycardia". Forty patients (27 with nonparoxysmal tachycardia and 13 with accelerated ectopic rhythms) were under observation. Accelerated rhythms were nearly always free of clinical symptoms and did not require an anti-arrhythmia treatment. Nonparoxysmal supraventricular and ventricular tachycardia was usually accompanied by aggravation of the patient's condition. Nonparoxysmal tachycardia turned to paroxysmal one or vi

Tetramisole is a new IK1 channel agonist and exerts IK1 -dependent cardioprotective effects in rats.

Cardiac ischemia, hypoxia, arrhythmias, and heart failure share the common electrophysiological changes featured by the elevation of intracellular Ca2+ (Ca2+ overload) and inhibition of the inward rectifier potassium (IK1 ) channel. IK1 channel agonists have been considered a new type of anti-arrhythmia and cardioprotective agents. We predicted using a drug repurposing strategy that tetramisole (Tet), a known anthelminthic agent, was a new IK1 channel agonist. The present study aimed to experimentally identify the above prediction and further demonstrate that Tet has cardioprotective effects.
