Complications and costs of patellofemoral arthroplasty versus total knee arthroplasty.

BACKGROUND: Patellofemoral arthroplasty (PFA) is an alternative to total knee arthroplasty (TKA) for the treatment of patellofemoral arthritis. Although PFA may preserve native kinematics and accelerate recovery, it has been associated with higher revision rates. The purpose of this study is to compare complication rates and costs between PFA and TKA. METHODS: Using the PearlDiver database, 6,179 patients with isolated patellofemoral arthritis treated with PFA or TKA from 2010-2015 were retrospectively reviewed with 5-year follow up. PFA and TKA patients were matched by age, sex, and Elixhaus…

Resurfacing arthroplasty versus silicone arthroplasty for proximal interphalangeal joint osteoarthritis.

PURPOSE: To compare the outcomes of silicone proximal interphalangeal joint (PIPJ) arthroplasties to pyrolytic carbon implants in patients w … implant specific. The results of this series show promise for the pyrolytic carbon PIPJ resurfacing arthroplasty but did not clearly demonstrate superiority compared with the silicone implant.

Outcomes of Bilateral Shoulder Arthroplasties: A Comparison of Bilateral Total Shoulder Arthroplasties and Bilateral Reverse Shoulder Arthroplasties.

BACKGROUND: The purpose of this study was to compare outcomes of patients who underwent bilateral total shoulder arthroplasties (TSAs) for osteoarthritis (OA) versus bilateral reverse shoulder arthroplasties (RSAs) for cuff tear arthropathy (CTA). METHODS: Inclusion criteria were patients who underwent bilateral TSAs for OA or bilateral RSAs for CTA with at least 2 years of follow-up. Twenty-six TSA patients (52 shoulders) were matched 2 to 1 with 13 RSA patients (26 shoulders) by sex, age at first surgery, and time between surgeries. Outcomes measured were shoulder range of motion (ROM), com

Risk factors for revision after shoulder arthroplasty: 1,825 shoulder arthroplasties from the Norwegian Arthroplasty Register.

BACKGROUND AND PURPOSE: Previous studies on shoulder arthroplasty have usually described small patient populations, and few articles have addressed the survival of shoulder implants. We describe the results of shoulder replacement in the Norwegian population (of 4.7 million) during a 12-year period. Trends in the use of shoulder arthroplasty during the study period were also investigated. PATIENTS AND METHODS: 1,531 hemiprostheses (HPs), 69 total shoulder replacements (Neer type TSR), and 225 reversed total shoulder replacement (reversed TSR) operations were reported to the Norwegian Arthropl…

Advances in knee arthroplasty for younger patients: traditional knee arthroplasty is prologue, the future for knee arthroplasty is prescient.

Total knee arthroplasty (TKA) was a remarkable development in orthopedic surgery. Joint arthroplasty and arthroscopy were perhaps the greatest innovations in orthopedics in the 20th century and occurred without the advantages of today's technology. Initially, TKA was performed only on elderly patients and those with advanced rheumatoid arthritis because of concerns with long-term wear of polyethylene. Surgeons strongly discouraged this surgery for patients younger than age 60 years because both patients and many orthopedic surgeons believed that knee implants would last only for approximately …

Elbow Trauma Sequelae: Instability, Stiffness, Non-arthroplasty, and Arthroplasty Options.

Sequelae of elbow trauma are complicated to manage. Undiagnosed instability patterns are important to recognize to get the elbow into concentric alignment for further reconstructive efforts. Stiffness is also common after elbow trauma. Surgeons should be familiar with the different approaches to the elbow to safely address stiffness. Non-arthroplasty and arthroplasty salvage options are also important to understand because the indications are expanding and outcomes for these procedures are being better understood.

Conversion total hip arthroplasty: Primary or revision total hip arthroplasty.

Total hip arthroplasty (THA) is an increasingly common procedure among elderly individuals. Although conversion THA is currently bundled in a diagnosis related group (DRG) with primary THA, there is a lack of literature supporting this classification and it has yet to be identified whether conversion THA better resembles primary or revision THA. This editorial analyzed the intraoperative and postoperative factors and functional outcomes following conversion THA, primary THA, and revision THA to understand whether the characteristics of conversion THA resemble one procedure or the other, or are…

Conversion of failed modern unicompartmental arthroplasty to total knee arthroplasty.

Between January 1983 and January 1991, 29 patients (31 knees) with a failed Robert Brigham metal-backed knee arthroplasty (Johnson & Johnson, Raynham, MA) underwent revision to a total knee arthroplasty (TKA). Twenty-five patients had osteoarthritis, three avascular necrosis, and one rheumatoid arthritis. The average patient age was 72.3 years (range, 49-88 years), and the average weight was 179 lb. (range, 112-242 lb.). The interval between the primary and secondary index procedures averaged 62 months (range, 7-106 months), and mean postrevision follow-up period was 45 months (range, 24-104 m…

Total Wrist Arthroplasty.

Total wrist arthroplasty has undergone substantial evolution in indications, implant design, and expected functional outcomes over the past decade. Osteoarthritis has overtaken rheumatoid arthritis as the major underlying etiology for joint replacement. Patient expectations have also evolved with a trend toward motion-preserving procedures. Finally, implant design based on careful experimental research efforts has assisted in outcome optimization. This article reviews these points in greater depth and provides examples and some technical points for the interested hand surgeon to consider.

Total Wrist Arthroplasty.

This comprehensive review discusses wrist arthritis in young patients while discussing the evolution of wrist arthroplasty and its use in this population. Here, we emphasize the challenges of managing arthritis in a high-demand, younger population and review historical to modern implant designs, including their benefits, complications, and outcomes. Ultimately, while modern implants show improved outcomes, long-term success in younger patients remains uncertain, necessitating cautious patient selection and personalized treatment plans.

Stemless Shoulder Arthroplasty.

Stemless humeral components for shoulder arthroplasty represent the fourth generation of modern prosthetic shoulder implants. Because of their metaphyseal fixation, the implantation technique is rather straightforward and preserves the humeral canal from violation. Substantial benefits have been highlighted with such a design, including less perioperative morbidity, independence from the proximal humeral anatomy, preservation of bone stock, ease of potential revision surgeries, and limited risk of complex periprosthetic fractures. Initially conceived to better re-create the center of rotation …

Radial head arthroplasty.

In conclusion, radial head fractures with 3 or more fragments have a high incidence of complications when treated with ORIF including hardware failure, malunion, nonunion, and the need for re-operation. Radial head arthroplasty has demonstrated good success in the treatment of complex, comminuted radial head fractures which are not amenable to non-opeative treatment or ORIF. Success can be optimized by diligent surgical dissection, avoiding inadvertent nerve injury, placement of an appropriately sized implant, repair of associated injuries, and early protected motion.

[Knee arthroplasty revision].

The number of knee arthroplasties in Germany increases about 6.8% annually. Parallel to the increasing number of primary total knee arthroplasties (TKA) the frequency of TKA revision surgery also goes up and is a growing challenge for orthopedic surgeons. The main reason for revision arthroplasty is aseptic or septic mechanical loosening of the implant. Other reasons are persisting pain, instability or limited range of motion. The aim of preoperative evaluation by examination, x-ray control and serum tests is to understand the mechanism of implant failure. Besides the stable mechanical fixatio…

Total disk arthroplasty.

Spinal fusion remains the gold standard for surgical management of instability and mechanical low back or neck pain. However, even in carefully selected patients, successful clinical results can be difficult to achieve. Reasons for failure include pseudarthrosis and adjacent spine segment disease. The theoretic advantages of removing the painful disk while preserving motion have led to increasing interest in total disk arthroplasty. Although disk replacements have been implanted in Europe for decades, the procedure is relatively new in the United States. Recently, two artificial disks for symp…

Modular shoulder arthroplasty.

One hundred consecutive Biomet modular shoulder arthroplasties were studied prospectively and were evaluated with a minimum 2-year follow-up (average 41 months). Fifty-seven women and 43 men with an average age of 64 years were evaluated for pain, activities of daily living, range of motion, cost, and complications. Fourteen patients had undergone previous surgery to the shoulder. Seventy patients underwent total shoulder arthroplasty, and thirty underwent hemiarthroplasty. Pain and range of motion demonstrated statistically significant improvement. Eight activities of daily living were rated …

Total hip arthroplasty.

A review of 333 consecutive Charnley total hip arthroplasties four to seven years after operation showed that twenty-eight patients had died, and three deaths were directly related to the surgery. The survivors had an average improvement of 46.6 points in their evaluation scores to a level of 91.3 points. Four deep infections occurred for which removal of the prosthesis was required. Thirteen hips dislocated postoperatively, and five of them required additional surgery. Three were four sciatic-nerve injuries and all but one resolved. Wear of the acetabular component was evident in seventeen hi

[Rhizarthrosis: Resection Arthroplasty versus Resection Suspension Arthroplasty - Still the Gold Standard?].

Rhizarthrosis is one of the most common arthritic changes in the hand, which has led to the development of a large number of surgical methods in recent years. In addition to the traditional resection arthroplasty with tendon suspension or interposition, if needed, thumb saddle joint prostheses are being used more and more frequently. However, these are not an option for all patients such as those with severe osteoporosis, severe nickel allergy, insufficient height of the trapezium or STT osteoarthritis. This study examined whether the traditional surgical methods continue to have their "right …
