Predictive factors of external ventricular drain placement in ruptured brain arteriovenous malformations with intraventricular hemorrhage.

BACKGROUND AD PURPOSE: Intraventricular (IV) hemorrhage is a common finding after brain arterio-venous malformation (BAVM) rupture and has been associated with poor prognosis. This report investigates previously published IV hemorrhage quantification scores as predictors of external ventricular drain (EVD) placement following BAVM rupture, along with other clinical and radiological parameters. METHODS: One hundred and twenty-nine patients with BAVM rupture and IV hemorrhage were included (133 ruptures), all admitted in our institution between January 1, 2003 and May 15, 2020. IV hemorrhage sc

Intraparenchymal vs extracranial ventricular drain intracranial pressure monitors in traumatic brain injury: less is more?

BACKGROUND: Management of severe traumatic brain injury has centered on continuous intracranial pressure (ICP) monitoring with intraparenchymal ICP monitors (IPM) or extracranial ventricular drains (EVD). Our hypothesis was that neurologic outcomes are unaffected by the type of ICP monitoring device. STUDY DESIGN: We reviewed 377 adult patients with traumatic brain injury requiring ICP monitoring. Primary outcome was Glasgow Outcome Score (GOS) 1 month after injury. Secondary outcomes included mortality, monitoring-related complications, and length of ICU and hospital stay. RESULTS: There we

Spontaneous thrombosis of the main draining vein revealing an unruptured brain arteriovenous malformation.

Haemorrhage is the most frequent revealing condition of brain arteriovenous malformations (bAVMs). We report a rare case of unruptured parietal bAVM revealed by spontaneous thrombosis of the main draining vein, responsible for a focal neurological deficit. The bAVM was embolized in emergency conditions; complete regression of the neurological symptoms was observed within five days after the embolization. Potential mechanisms of such spontaneous thrombosis of the bAVM's main drainage pathway as well as an exhaustive review of the literature concerning this rare revealing condition are presented

From brain drain to geopolitical talent competition: scientific diasporas and the future of Global South development.

Scientific diasporas have emerged as increasingly important actors in global science, technology, and innovation (STI) ecosystems. In the context of geopolitical fragmentation, technological competition, and intensifying global inequalities, talent mobility is no longer solely a migration issue but a strategic dimension of development, science diplomacy, and international influence. This policy review argues that Global South countries should move beyond traditional "brain drain" narratives and instead adopt long-term strategies that view scientific diasporas as transnational development partn

External Ventricular Drain Versus Intraparenchymal Pressure Monitor in Severe Traumatic Brain Injury: A TRACK-TBI Study.

BACKGROUND AND OBJECTIVES: The 2 most common intracranial pressure (ICP)-monitoring devices in traumatic brain injury (TBI) are external ventricular drains (EVDs) and intraparenchymal monitors (IPMs). EVDs and IPMs differ in functionality and debate remains as to whether device selection affects patient outcomes. We aimed to determine whether the use of EVDs or IPMs for ICP monitoring was associated with better outcomes using data from severe TBI patients prospectively enrolled in the 18-center US Transforming Research and Clinical Knowledge in TBI (TRACK-TBI) study. METHODS: We included pati

A case of rhinocerebral mucormycosis with brain abscess drained by endoscopic endonasal skull base surgery.

A 70-year-old Japanese man undergoing remission induction therapy for acute monocytic leukemia (AML-M5b) developed fever and headache, and was started on antibiotics and liposomal amphotericin B (L-AMB). There was no improvement, and computed tomography and contrast-enhanced magnetic resonance imaging revealed acute rhinosinusitis and brain abscess. Successful endoscopic endonasal surgery was performed at this point, providing drainage for the rhinosinusitis and abscess. Histopathological findings showed the mucormycosis.

B cells populating the multiple sclerosis brain mature in the draining cervical lymph nodes.

Multiple sclerosis (MS) is an inflammatory disease of the central nervous system (CNS) characterized by autoimmune-mediated demyelination and neurodegeneration. The CNS of patients with MS harbors expanded clones of antigen-experienced B cells that reside in distinct compartments including the meninges, cerebrospinal fluid (CSF), and parenchyma. It is not understood whether this immune infiltrate initiates its development in the CNS or in peripheral tissues. B cells in the CSF can exchange with those in peripheral blood, implying that CNS B cells may have access to lymphoid tissue that may be

Physician Brain Drain from Turkey: Push-Pull Dynamics, Return Intentions, and the Normative Challenge for Health Policy.

This mixed-methods study examines the push-pull factors influencing physician emigration from Turkey and assesses the feasibility of reverse brain drain, framing physician migration as a challenge for health governance rather than merely an individual career choice. Quantitative data from 1331 physicians (700 specialists, 631 interns) across 19 provinces and qualitative interviews with 32 emigrated physicians in four destination countries reveal that violence against healthcare workers (81.6%), low salary (74.3%), and challenging working conditions (59.8%) constitute the primary push factors.

Post-material values and brain drain: how value transformation shapes the migration mindset among highly educated South Asians?

While economic reasons are predominantly considered as drivers of brain drain in the existing literature, the present study demonstrates that migration decisions are substantially shaped by structural misalignments between individuals' value orientations and the institutional, cultural, and social environments of their home countries. Drawing on 25 in-depth interviews and a focus group discussion, the study integrates Bourdieu's theorisations of habitus, field, and taste with Inglehart's theory of post-material value shift to understand how higher education cultivates durable dispositions orie

Association Between Early External Ventricular Drain Insertion and Functional Outcomes 6 Months Following Moderate-to-Severe Traumatic Brain Injury.

Traumatic brain injury (TBI) is a leading global cause of morbidity and mortality. Intracranial hypertension following moderate-to-severe TBI (m-sTBI) is a potentially modifiable secondary cerebral insult and one of the central therapeutic targets of contemporary neurocritical care. External ventricular drain (EVD) insertion is a common therapeutic intervention used to control intracranial hypertension and attenuate secondary brain injury. However, the optimal timing of EVD insertion in the setting of m-sTBI is uncertain and practice variation is widespread. Therefore, we aimed to assess if th

Impact of Drain Column Height setting on Intracranial Pressure Control and Outcomes After Severe Traumatic Brain Injury: A Retrospective Study.

BACKGROUND: External ventricular drainage (EVD) is commonly used to control intracranial pressure (ICP) in severe traumatic brain injury (TBI). Although drainage column height is assumed to influence cerebrospinal fluid (CSF) diversion and ICP control, its clinical impact remains uncertain. METHODS: We conducted a multicenter cohort study including patients with severe TBI requiring EVD placement across nine centers. Patients were categorized according to the initial drainage column height threshold (≤10 cmH2O vs. >10 cmH2O). The primary outcome was ICP control, assessed by the highest daily

The Incidence of Meningitis in Patients with Traumatic Brain Injury Undergoing External Ventricular Drain Placement: A Nationwide Inpatient Sample Analysis.

BACKGROUND/OBJECTIVE: Infection is the most common complication of external ventricular drain (EVD) placement. National trends in the annual incidence of meningitis among patients with traumatic brain injury (TBI) who have undergone EVD placement have not been reported. METHODS: The Nationwide Inpatient Sample was used to select adults with a primary diagnosis of TBI who underwent EVD placement between 2002 and 2011. Annual rates of meningitis among patients who underwent EVD placement were determined. We also calculated mortality rates and length of stay (LOS). Potential factors associated w

Harnessing peer mentorship as a tool to turn human resource for health brain drain into brain gain: a case study of a Nigerian peer-mentored research group.

BACKGROUND: Peer mentorship can be a potential tool to reduce the disparities in health research capacity between high- and low- and middle-income countries. This case study describes the potential of peer mentorship to tackle two critical issues: bridging health research capacity of doctors from low- and middle-income countries (LMICs) and the transformation of human resource for health brain drain into "brain gain". CASE PRESENTATION: In 2021, a virtual peer mentorship group was established by 16 alumni of the University of Benin College of Medical Sciences' 2008 graduating class, residing

Poroelastic Mechanical Properties of the Brain Tissue of Normal Pressure Hydrocephalus Patients During Lumbar Drain Treatment Using Intrinsic Actuation MR Elastography.

RATIONALE AND OBJECTIVES: Hydrocephalus (HC) is caused by accumulating cerebrospinal fluid resulting in enlarged ventricles and neurological symptoms. HC can be treated via a shunt in a subset of patients; identifying which individuals will respond through noninvasive imaging would avoid complications from unsuccessful treatments. This preliminary work is a longitudinal study applying MR Elastography (MRE) to HC patients with a focus on normal pressure hydrocephalus (NPH). MATERIALS AND METHODS: Twenty-two ventriculomegaly patients were imaged and subsequently received a lumbar drain placemen

Brain tumors induce immunoregulatory dendritic cells in draining lymph nodes that can be targeted by OX40 agonist treatment.

BACKGROUND: Primary and metastatic brain tumors have a poor prognosis, partly owing to the unique characteristics of the central nervous system (CNS) and tumor immune microenvironment (TIME). One distinct feature of the CNS TIME is the limited infiltration and activation of dendritic cells (DCs). The impact of CNS versus non-CNS TIME can be assessed by injecting tumor cells from the same model, either subcutaneously (peripherally) or into the brain. Subcutaneous tumors drain into the tumor-draining lymph nodes in the skin (TdLN-p), whereas brain tumors drain into the deep cervical TdLN (TdLN-c

Timing of external ventricular drain placement and its effects on in-hospital outcomes in traumatic brain injury: A propensity score-matched analysis.

OBJECTIVE: Patients with traumatic brain injury (TBI) often develop elevated intracranial pressure (ICP) and require emergency external ventricular drain (EVD) placement to decompress the cerebral ventricles. In patients who are initially stable, elevated ICP may develop later during hospitalization, necessitating delayed EVD placement. In these patients, it is unclear if early empiric placement can help reduce mortality. The purpose of this study is to compare the differences in in-hospital outcomes between delayed and early EVD placement. METHODS: In this retrospective cohort study, the Nat
