A Bridge Too Far? Towards Medical Therapy for Clinically Nonfunctioning Pituitary Tumors.

Clinically nonfunctioning pituitary tumors (CNFPTs) typically do not cause hormonal excess, progress insidiously, and are often large and invasive at presentation. Complete resection is frequently not attainable; radiotherapy (RT) may effectively limit growth but carries a significant risk of hypopituitarism. Medical therapy with dopamine D2 receptor agonists and/or somatostatin analogs has been explored in CNFPTs but have yielded inconsistent results, and there is an unmet need for novel efficacious and safe medical therapies. The authors used the PubMed database to identify and review articl

Successful Use of Intragastric Balloon Therapy as a Bridge to Heart Transplantation.

Obesity is an important limiting factor for heart transplantation (HT) in patients with congestive heart failure (CHF). Achieving substantial weight loss is challenging in this population due to activity limitations and fluid retention. Endoscopic bariatric therapies (EBTs) including intragastric balloons (IGB) are effective primary weight loss therapies. IGBs have also been successfully utilized as a bridge therapy prior to liver transplantation and, in one case report, prior to HT. Potential advantages of IGBs in this population include low bleeding risk and less invasiveness as compared to

Mechanical circulatory support as a bridge to transplant or for destination therapy.

Mechanical circulatory support (MCS) frequently is used to treat medically refractory end-stage heart failure. Initially designed to be a bridge to transplantation, MCS also has proven itself as a durable therapy for patients who are not transplant candidates. As outcomes for patients with MCS have improved, research interest in device development has flourished, with many new device types under investigation. In addition to improvement of MCS devices, investigational work continues to achieve appropriate patient selection and complication management.

Brief report: Ketamine-assisted "bridge therapy" for opioid tapering in complex cases.

BACKGROUND: Opioid use disorder (OUD) presents major challenges, especially when combined with chronic pain and psychiatric comorbidities. A 25-year-old woman with OUD, chronic pain, and major depressive disorder underwent an 8-week protocol of intravenous ketamine infusions (0.5 mg/kg weekly) as an adjunct to opioid tapering. RESULTS: Methadone dosage was reduced by 50%, with improved pain and craving control, reduced withdrawal symptoms, and greater psychotherapy engagement. DISCUSSION AND CONCLUSIONS: Ketamine-assisted "bridge therapy" treatment may support opioid tapering in complex case

Mechanical circulatory support as bridge therapy for heart transplant: case series report.

BACKGROUND: Mechanical circulatory support (MCS) represents an effective urgent therapy for patients with cardiac arrest or end-stage cardiac failure. However, its use in developing countries as a bridge therapy remains controversial due to costs and limited duration. This study presents five patients who underwent MSC as bridge therapy for heart transplantation in a developing country. CASE PRESENTATION: We present five patients who underwent MCS as bridge therapy for heart transplant between 2010 and 2015 at Fundación Cardioinfantil-Instituto de Cardiología. Four were male, median age was 3

Neuroversion: using electroconvulsive therapy as a bridge to deep brain stimulation implantation.

Parkinson's disease (PD) is a movement disorder with significant neuropsychiatric comorbidities. Electroconvulsive therapy (ECT) is effective in treating these neuropsychiatric symptoms; however, clinicians are reluctant to use ECT in patients with deep brain stimulation (DBS) implantations for fear of damaging the device, as well as potential cognitive side effects. Right unilateral ultra-brief pulse (RUL UBP) ECT has a more favorable cognitive side-effect profile yet has never been reported in PD patients with DBS implants. We present a case series of three patients with a history of PD that

Cuproptosis: Bridging targeted therapy and immunotherapy in cancer treatment.

Cancer therapy has entered an era dominated by molecularly targeted agents and immune checkpoint inhibitors (ICIs), yet therapeutic resistance and patient heterogeneity remain major barriers to durable benefit. Cuproptosis, a recently identified form of copper-dependent regulated cell death linked to mitochondrial metabolism, has emerged as a potentially relevant therapeutic concept. Distinct from apoptosis and ferroptosis, cuproptosis is triggered by the interaction of copper with lipoylated enzymes in the tricarboxylic acid (TCA) cycle, leading to proteotoxic stress and metabolic collapse. Garcinia indica: A Multifaceted Phytomedicine Bridging Nutrition and Therapy.

Garcinia indica (Kokum), a tropical tree native to the Western Ghats of India, is an underutilized yet pharmacologically significant species in the family Clusiaceae. Traditionally used in food and folk medicine, Kokum has also attracted scientific interest due to its diverse therapeutic potential. The fruit of G. indica is rich in bioactive phytochemicals, including hydroxycitric acid (HCA), anthocyanins, and polyisoprenylated benzophenones such as garcinol, xanthochymol, and guttiferone isoforms. These compounds exhibit a wide range of biological activities, including antioxidant, anti-infla

Cinepathy: A translational framework bridging neurocinematics and clinical therapy.

BACKGROUND: Neurocinematics has described how the brain responds to cinema, though responses vary with mental state, prior experience, and cultural background, blurring therapeutic and personal effects. No standardized, neurobiologically grounded therapeutic framework has yet been derived from that literature. We introduce Cinepathy to address this gap. CONCEPTUAL BASIS: Cinepathy proposes that cinema modulates neural activity through six convergent mechanisms, proposed to operate in concert: sensory-emotional association, narrative involvement, embodied simulation, limbic activation, neuroho

Chloride Homeostasis in Neuronal Disorders: Bridging Measurement to Therapy.

Neuronal chloride (Cl-) homeostasis is fundamental for brain function, with disruptions increasingly recognized as pathogenic across neurological disorders. This review synthesizes evidence from preclinical models and clinical studies, integrating electrophysiological measurements, molecular analyses, imaging with genetically encoded sensors like ClopHensor, and behavioral assays. Key findings demonstrate that Cl- dysregulation follows distinct patterns: (1) in epilepsy, KCC2 downregulation converts GABAergic inhibition to excitation, promoting seizures; (2) in Alzheimer's disease (AD) models,

Complement system activation: bridging physiology, pathophysiology, and therapy.

The complement system is a set of over 50 proteins that constitutes an essential part of the innate immune system. Complement system activation involves an organized proteolytic cascade. Overactivation of complement system activation is the main pathogenic mechanism of several diseases and contributes to the manifestations of many other conditions. This review describes the normal complement system and the role for complement dysregulation in critical illnesses, notably sepsis and acute respiratory distress syndrome. Complement activation is involved in the immune system response to pathogens

NAION AND ANTICOAGULANTS: BRIDGING THE GAP WITH RATIONALE-BASED THERAPY.

BACKGROUND: NAION, a multifactorial disorder with an unclear mechanism has limited insight on its treatment option. Despite conflicting results in its relation to hypercoagulation, many metabolic risk factors associated with NAION are intertwined with hypercoagulation. We aim to investigate the impact of anticoagulant therapy in the vascular and visual function of NAION patients. METHODS: A prospective interventional study was conducted in the Neuro-Ophthalmology Division, Department of Ophthalmology at FKUI-RSCM Kirana from October 2020 to April 2022, involving two groups of NAION subjects:

Evaluation of Extracorporeal Membrane Oxygenation Therapy as a Bridging Method.

BACKGROUND: With the implementation of the new heart allocation system, heart transplantation teams are prompted to reevaluate management of patients requiring mechanical circulatory support. The purpose of our study is to compare the outcomes of patients supported with extracorporeal membrane oxygenation (ECMO) before transplantation. METHODS: The United Network for Organ Sharing database was queried for all adult patients (aged 18 years or more) who required support with ECMO before heart transplantation from 2001 to 2018. Patients were stratified into patients who did not require ECMO befo

Hybrid Extracorporeal Therapies as a Bridge to Pediatric Liver Transplantation.

OBJECTIVES: Standard intensive care treatment is inadequate to keep children with liver failure alive without catastrophic complications to ensure successful transplant, as accumulation of endogenous protein-bound toxins often lead to hepatic encephalopathy, hepatorenal syndrome, cardiovascular instability, and multiple organ failure. Given paucity of proven treatment modalities for liver failure, blood purification using different extracorporeal treatments as a bridge to transplantation is used, but studies evaluating the safety and efficacy of combination of these therapies, especially in pe

Preparing for CAR T cell therapy: patient selection, bridging therapies and lymphodepletion.

Chimeric antigen receptor (CAR) T cells have emerged as a potent therapeutic approach for patients with certain haematological cancers, with multiple CAR T cell products currently approved by the FDA for those with relapsed and/or refractory B cell malignancies. However, in order to derive the desired level of effectiveness, patients need to successfully receive the CAR T cell infusion in a timely fashion. This process entails apheresis of the patient's T cells, followed by CAR T cell manufacture. While awaiting infusion at an authorized treatment centre, patients may receive interim disease-d
