Immunotoxicology of brown recluse spider (Loxosceles reclusa) venom.

A purified toxic protein from Loxosceles reclusa venom was assayed for its in vitro effects on the human immunological and blood clotting systems. The toxin caused inhibition of neutrophil chemotaxis, depletion of serum hemolytic complement, prolongation of the activated partial thromboplastin time and depletion of clotting factors XII, XI, IX and VIII by an average of 44% in human plasma. The prothrombin time of human plasma was also prolonged by 1.5-2.0 sec. No effect of the purified toxin was observed on microbicidal ability of neutrophils, the release of enzymes from neutrophils or the adh

Do brown recluse spider bites induce pyoderma gangrenosum?

Brown recluse spider bites are usually self-limited skin lesions that infrequently progress to bullae, ulceration, and scarring. We treated a patient with a documented brown recluse bite who had recurring lesions resembling pyoderma gangrenosum (PG) that persisted for months. Three other patients referred to Vanderbilt University because of probable brown recluse bite also had pyoderma for the first time after a suspected arthropod bite. The persistent and recurrent pyodermas in these four patients indicate that (1) brown recluse spider bites may not be self-limited but induce PG or PG-like le

Necrotizing fasciitis developing from a brown recluse spider bite.

A 20-year retrospective case series was analyzed to identify the brown recluse spider bite as a cause of necrotizing fasciitis. Data from 31 consecutive patients with necrotizing fasciitis were analyzed. Of the 31 patients with necrotizing fasciitis a brown recluse spider bite was found to be the initial cause in two patients. Both patients with spider bites delayed in obtaining medical treatment, and secondary infection of the necrotic tissue occurred. One patient was diagnosed by frozen section tissue biopsy, and the second patient was diagnosed by clinical examination. All patients in this

Brown recluse spider (Loxosceles reclusa) envenomation in small animals.

OBJECTIVE: To provide a comprehensive review of relevant literature regarding the brown recluse spider (BRS) and to define those criteria that must be satisfied before making a diagnosis of brown recluse envenomation. ETIOLOGY: The complex venom of the BRS contains sphingomyelinase D, which is capable of producing all the clinical signs in the human and some animal models. DIAGNOSIS: There is no current commercially available test. In humans there are many proposed guidelines to achieve a definitive diagnosis; however, there are no established guidelines for veterinary patients. THERAPY: Cu

Clinical presentation and outcome of brown recluse spider bite.

STUDY OBJECTIVE: To examine the clinical presentation and outcome of patients treated in the ED or toxicology clinic for suspected brown recluse spider bites. METHODS: We assembled a retrospective case of patients at a southeastern US university hospital. Our study group comprised 111 patients with suspected brown recluse spider bites treated during a 30-month period. Our main outcome measures were the need for skin grafting and the development of other complications. RESULTS: The mean age of our subjects was 34 +/- 17 years. Thirteen patients (12%) brought the spider to the hospital, 22 (20

Management of the brown recluse spider bite to the glans penis.

A significant number of people are bitten by the brown recluse spider (BRS) each year. Medical treatment regimens are sometimes unsatisfactory and surgical intervention is often necessary to debride the necrotic wound. This case study reports the treatment given to a 19-year-old active duty United States Army soldier who suffered a BRS bite to the glans penis. This patient received immediate medical attention and was started on intravenously administered diphenhydramine, methylprednisolone, calcium gluconate, and famotidine. Oral dapsone treatment was begun in the emergency room. Within 24 hou

Complications and outcomes of brown recluse spider bites in children.

Brown recluse spider bites may cause severe local and systemic morbidity, but data regarding morbidity in children are limited. This study reviewed inpatient medical records (n = 26; 10 years) with a discharge diagnosis of "spider bite" from a tertiary pediatric hospital. The majority (85%) of children had an inflammatory response accompanying necrosis, usually with signs of secondary cellulitis (77%). Hemolytic anemia (50%), rhabdomyolysis (27%), and acute renal failure (12%) were the most prevalent systemic effects. Hemolytic anemia was bimodal in distribution relative to the time-of-onset o

Brown recluse spider envenomation of the eyelid: an animal model.

The authors developed a rabbit model of the brown recluse (BR) spider envenomation of the human eyelid. The spider bite causes cutaneous necrosis and systemic toxicity in human eyelids, possibly leading to disseminated intravascular coagulation, hemolysis, and death. The treatment has been controversial. The animal model evaluated the effects of single- and combined-agent therapy in four phases: venom dose response, time course, therapeutic effectiveness (steroid vs. dapsone vs. antivenom), and optimal therapy (steroid and dapsone; steroid and antivenom; and dapsone and antivenom combination g

[A pustular form of loxoscelism (bite of the brown recluse spider)].

We report on a 42-year-old man who showed the typical clinical picture after being bitten by a brown recluse spider (Loxosceles reclusa). The patient developed a pustular eruption during the acute phase of the disease. To the best of our knowledge, this is the first report on pustular loxoscelism.

Persistent segmental cutaneous anesthesia after a brown recluse spider bite.

Patients with brown recluse spider bites commonly suffer from pain, muscular aching, and a variety of local dysesthesias during the acute and resolution phases of toxin-induced injury. This is our first well-documented observation of persistent cutaneous anesthesia caused by a spider bite. The anesthetic area conformed to the distribution of a specific nerve, the transverse cervical cutaneous nerve. The identification of the spider as a brown recluse and the location of the bite over the nerve's usual pathway strongly suggest that the venom was responsible for this complication.

Brown recluse spider bite. A literature review and case report.

A review of the literature on brown recluse spider bite is presented, including clinical presentation and treatment. Emphasis is placed on the treatment of local tissue necrosis. A case report of a severe necrotic ulcer secondary to a spider bit is presented. Tissue necrosis following a brown recluse spider bit can be debilitating, and healing may be prolonged for many months. Even with early treatment, prognosis often is poor. Future studies might help establish better treatment regimens.

Autoimmune haemolytic anaemia secondary to a brown recluse spider bite.

A female in her 30s from Missouri with no significant past medical history presented to the emergency department with worsening fatigue, haematuria and back pain following a brown recluse spider bite. On admission, laboratory findings were significant for anaemia, leukocytosis, hyperbilirubinaemia, elevated lactate dehydrogenase, low haptoglobin and a positive direct antiglobulin test. The working diagnosis was autoimmune haemolytic anaemia secondary to a brown recluse spider bite; a rare but serious haematologic complication associated with brown recluse spider bites. The patient required mul

Cutaneous-hemolytic loxoscelism following brown recluse spider envenomation: new understandings.

Background: Brown recluse spider (BRS) (Loxosceles reclusa) envenomation can cause local dermonecrotic lesions, constitutional symptoms, and potentially fatal hemolysis (i.e., cutaneous-hemolytic loxoscelism). As the incidence of hemolysis is low and the spider habitat is limited, little is known regarding the clinical course of cutaneous-hemolytic loxoscelism. Methods: We performed a retrospective observational study of patients following BRS envenomation over an eight-year period. Demographics, clinical course, laboratories, and interventions were assessed. Wilcoxon rank-sum tests and Pearson

Brown recluse spider envenomation: dermatologic application of hyperbaric oxygen therapy.

Envenomation from the brown recluse (Loxosceles recluse) spider commonly proceed on one of three clinical pathways. The majority of bites (90%) result in nothing more than a local reaction. They are essentially self-limiting, require little if any attention, and resolve spontaneously. A great majority of the remaining bites will produce necrotic ulcerations of various sizes and dimensions, with systemic sequela ranging from fever to hemolysis and kidney failure. Finally, and in the most rare cases, the patient will succumb a fatal systemic reaction. Current therapeutic options for these wounds

Dapsone or electric shock therapy of brown recluse spider envenomation?

STUDY OBJECTIVES: We tested the null hypothesis that no outcome differences exist among the treatment groups of animals with brown recluse spider envenomations. STUDY DESIGN: A prospective, placebo-controlled, experimental trial. Hartley guinea pigs were randomized into the following treatment groups: dapsone, Parali/azer stun gun, Guardian stun gun, and control. INTERVENTIONS: Brown recluse spider skin lesions were induced with intradermal injections of 30 micrograms spider venom and were treated beginning 16 hours after inoculation. Shock regimens consisted of four cross shocks of 1-second

Persistent painful plaque due to a brown recluse spider bite.

The bite of the arthropod Loxosceles is known to cause subdermal hemorrhage, dermal-epidermal separation, inflammatory infiltrates, as well as occlusion and necrosis of small arterioles. We report a case of a brown recluse spider bite that presented as a chronic painful skin plaque, with the unusual histologic findings of a cutaneous and deep subcutaneous hyalinizing panniculitis and myonecrosis.
