Carotid artery-cavernous sinus fistula.

Carotid artery-cavernous sinus fistula is a lesion most often associated with massive head trauma. The maxillofacial surgeon must be able to diagnose this major complication of head trauma, use proper diagnostic techniques, and initiate appropriate therapeutic regimens. The symptoms of pulsatile headache, bruit, and visual change accompany the physical findings of bruit, conjunctival engorgement, chemosis, proptosis, and ophthalmoplegia. A number of procedures have been devised to correct this difficult lesion, ranging from cervical ligation to balloon occlusion. Two cases of carotid artery-ca

HIV-associated cavernous sinus disease.

INTRODUCTION: The underlying diagnosis of cavernous sinus disease is difficult to confirm in HIV-coinfected patients owing to the lack of histological confirmation. In this retrospective case series, we highlight the challenges in confirming the diagnosis and managing these patients. RESULTS: The clinical, laboratory and radiological data of 23 HIV-infected patients with cavernous sinus disease were analysed. The mean age of patients was 38 years. The mean CD4+ count was 390 cells/μL. Clinically, patients presented with unilateral disease (65%), headache (48%), diplopia (30%) and blurred visi

Cavernous Sinus Hemangioma: Surgical Aspects.

Resection of lesions in the cavernous sinus remains a challenge to neurosurgeons due to its complex anatomy. This video (Video 1) presents the most relevant landmarks of the cavernous sinus region, helping to understand the surgical anatomy. A 55-year-old female presented with diplopia, partial ptosis, and facial dysesthesia in the left ophthalmic territory. Magnetic resonance imaging revealed a lesion in the left cavernous sinus. Microsurgical resection was performed through an approach that included a fronto-orbital craniotomy, an extradural clinoidectomy, and intradural resection of the les

Interdural cavernous sinus epidermoid cyst.

We report a patient with an uncommon interdural epidermoid tumor, located within the confines of dural layers of the lateral wall of the cavernous sinus. The tumor was resected by a basal subtemporal extradural-interdural approach. Following the surgery, the 45-year-old female patient recovered completely from her symptoms of atypical neuralgic facial pains.

Cavernous sinus thrombophlebitis in children.

Eight Thai patients less than 15 years of age who were diagnosed with cavernous sinus thrombophlebitis at Ramathibodi Hospital, Bangkok, Thailand over the past 30 years were reviewed retrospectively. The predisposing infections and causative microorganisms were similar to previous reports in children and adults. Despite severe neurologic dysfunction during admission, including blindness, there was neither death nor severe permanent deficit found in the majority of the patients. Only one patient experienced mild hemiparesis caused by cerebral infarction, which was secondary to this condition. E

Posttraumatic carotid-cavernous sinus fistula.

BACKGROUND AND OBJECTIVES: Posttraumatic carotid-cavernous sinus fistula is a rare complication of maxillofacial trauma and is seldom discussed in the literature. Motor vehicle accidents, falls, and other crush injuries contribute to the incidence of basilar skull fractures and the formation of fistulae. When injuries occur in the vessel wall, the carotid artery has the potential to fill the low-pressure cavernous sinus. The symptoms include chemosis, proptosis, pulsating exophthalmos, diplopia, ophthalmoplegia, orbital pain, audible bruits, and blindness. METHODS AND MATERIALS: The conventio

Radiosurgery of cavernous sinus tumors.

Stereotactic radiosurgery was applied to concentrate high dose of photon irradiation to tumors enclosed in the cavernous sinus. We have treated 10 meningiomas, five pituitary tumors, and three metastatic lesions in the past two years. Follow-up time ranges from 3 to 18 months with an average of 10 months. Tumor volume ranged from 0.92 to 53.31 cc with an average 10.79 cc. CT scans and MRIs were used to demonstrate the tumor relationship to cranial nerves and structures of the brain. One to 6 isocenters were used. Collimator size varied from 7 to 28 mm, and the dose to the margin of the tumor r

The anterior cavernous sinus space.

The anterior cavernous sinus space is the venous space anterior to the cavernous carotid artery. It is tetrahedral in shape with the anterior apex at the superior orbital fissure. The superomedial wall is formed by the base of the anterior clinoid process. The inferomedial wall is formed by the sphenoid bone. The lateral wall consists of two layers of dura with the oculomotor, trochlear, ophthalmic and abducens nerves in the inner layer. Medially, the posterior wall consists of the cavernous carotid artery. Laterally, it communicates with the lateral cavernous sinus space. Paraclinoid and caro

Traumatic carotid-cavernous sinus fistula.

Traumatic carotid-cavernous sinus fistula is an uncommon situation resulting from severe trauma, with less than 40 cases having been reported. It is usually associated with a skull base, frontal or mid-facial fracture, but it may also be a spontaneous phenomenon of congenital, infective or degenerative origin. The blood shunts from the internal carotid to the cavernous sinus resulting in pulsating exophthalmos, orbital headache, pain, orbital or frontal bruit, loss of visual acuity, diplopia, ophthalmoplegia and the differential diagnosis should include superior orbital fissure syndrome, orbit

Ischemic ("diabetic") cavernous sinus syndrome.

A 65-year-old diabetic woman presented with an acute cavernous sinus syndrome involving the right third, fifth, and sixth cranial nerves. The complete neuro-ophthalmic workup included high-resolution computed tomography, angiography, lumbar puncture, and laboratory studies, all of which were unrevealing. The cranial nerve palsies spontaneously resolved in 10 weeks, consistent with an ischemic etiology.

Traumatic carotid-cavernous sinus fistula.

Seventy-five percent of carotid-cavernous sinus fistulae result from high velocity head trauma. The most common symptom is a pulse synchronous bruit which usually becomes apparent when the patient regains his sensorium. The objective manifestations of such an arteriovenous fistula are characteristically localized to the ipsilateral orbit. A periorbital bruit and venous engorgement of the palpebral and bulbar conjunctivae are pathognomonic features. Bilateral carotid angiography confirms the diagnosis and determines which of several techniques might be used to obliterate the fistula. Occasional

Cavernous Lymphangioma in the Maxillary Sinus.

Lymphangioma in the maxillary sinus is rare. Herein, the authors report an unusual case of cavernous lymphangioma, in the maxillary sinus, with imaging examinations. Clinicians should be aware of the possibility of lymphangioma in the maxillary sinus, and be capable of distinguishing it, from other benign cystic lesions.

Cystic Chordoma of the Cavernous Sinus.

Chordomas are the tumors commonly involving base of skull which are predominantly solid in consistency. A cystic chordoma of the cavernous sinus is an extremely rare entity and has never been reported. We report a 26-year-old male presenting with gradually progressive right-sided sixth nerve palsy and headache. Computed tomography and magnetic resonance imaging of the brain showed a cystic lesion in the right cavernous sinus. The patient was successfully treated using a transnasal endoscopic transsphenoidal approach. Histopathological examination of the lesion revealed chordoma. Our case is th
