Rare Complications of Lumboperitoneal Shunts: Abdominal Cerebrospinal Fluid Pseudocysts in Adults.

Abdominal cerebrospinal fluid (CSF) pseudocysts are rare complications of shunt surgery, predominantly reported in pediatric patients undergoing a ventriculoperitoneal (VP) shunt. In contrast, their occurrence following lumboperitoneal (LP) shunt is exceptionally uncommon. We report the case of a 76-year-old woman who presented with recurrent symptoms, including gait disturbance and cognitive decline, approximately three years after undergoing LP shunt placement for idiopathic normal pressure hydrocephalus. Diagnostic imaging, including shuntography and abdominal computed tomography (CT), reve

Neuraxial anaesthesia in patients with cerebrospinal fluid shunts: A descriptive review.

Neuraxial anesthesia in patients with cerebrospinal fluid (CSF) shunt devices has traditionally been associated with a high risk of complications. In order to gather all available evidence, a structured search was conducted to include published studies involving users of these devices, undergoing any form of neuraxial technique for obstetric or surgical procedures unrelated to them. Effectiveness of the technique and perioperative complications were assessed. Only case series and case reports (n = 72) were identified. One patient was found to have insufficient anesthetic coverage, necessitatin

United States emergency department visits for children with cerebrospinal fluid shunts.

OBJECTIVE: CSF shunt placement is the primary therapy for hydrocephalus; however, shunt malfunctions remain common and lead to neurological deficits if missed. There is a lack of literature characterizing the epidemiology of children with possible shunt malfunctions presenting to United States emergency departments (EDs). METHODS: A retrospective study was conducted of the 2006-2017 National Emergency Department Sample. The data were queried using an exhaustive list of Current Procedural Terminology and International Classification of Diseases, Ninth Revision and International Classification

Challenges in cerebrospinal fluid shunting in patients with glioblastoma.

BACKGROUND: Cerebrospinal fluid (CSF) circulation disturbances may occur during the course of disease in patients with glioblastoma. Ventriculoperitoneal shunting has generally been recommended to improve symptoms in glioblastoma patients. Shunt implantation for patients with glioblastoma, however, presents as a complex situation and produces different problems to shunting in other contexts. Information on complications of shunting glioma patients has rarely been the subject of investigation. In this retrospective study, we analysed restropectively the course and outcome of glioblastoma-relate

[Risk factors for cerebrospinal fluid shunt infection].

BACKGROUND AND OBJECTIVE: Shunt-related infection is a major complication. The effort to face up to this problem has focused on therapy, analysis of risk factors and primary prevention. The aim of this study is to identify risk factors for cerebrospinal fluid shunt infection. MATERIAL AND METHOD: We retrospectively reviewed all patients who underwent shunt placement at our hospital during the period of study. Procedures followed for less than 90 days were excluded. One hundred and nineteen procedures were included in the final analysis. RESULTS: Infection rate per procedure was 8.4%. Previou

Cerebrospinal fluid shunt malfunctions: A reflective review.

PURPOSE: Pediatric hydrocephalus is a common and challenging condition. To date, the ventriculoperitoneal shunt (VPS) is still the main lifesaving treatment option. Nonetheless, it remains imperfect and is associated with multiple short- and long-term complications. This paper is a reflective review of the current state of the VPS, our knowledge gaps, and the future state of shunts in neurosurgical practice. METHODS AND RESULTS: The authors' reflections are based on a review of shunts and shunt-related literature. CONCLUSION: Overall, there is still an urgent need for the neurosurgical commu

Intraperitoneal cerebrospinal fluid pseudocyst with ventriculoperitoneal shunt.

Ventriculoperitoneal (VP) shunting is mostly used in the treatment of hydrocephalus and many complications have been reported with this method. Abdominal Pseudocyst (APC) are relatively uncommon but important complications in patients with VP shunts. We herein report the case of a 9-year-old boy with VP shunt who presented with abdominal distension, abdominal pain, malaise, and decrease of appetite. Abdominal pelvic computed tomography confirmed a diagnosis of APC. Laparotomy was done and VP shunt was placed into the other side of peritoneal cavity again. Also here, etiology, presentation, dia

Bacterial contamination in cerebrospinal fluid shunt surgery.

Infection is a major complication of cerebrospinal fluid (CSF) shunt treatment with a reported incidence of 8-27%. In the present study, comprising 64 patients operated under strictly defined preoperative and peroperative hygienic measures and without use of prophylactic antibiotics, the infection rate was 3.1%. Out of the 2 cases of shunt infection recorded, 1 was probably caused by peroperative contamination and 1 by postoperative, hematogenous spread. Bacterial cultures from the operation area before incision and during operation were negative in 95% and 78%, respectively, and when positive

Quantitative evaluation of cerebrospinal fluid shunt flow.

We describe a rigorous method for measuring the flow of cerebrospinal fluid (CSF) in shunt circuits implanted for the relief of obstructive hydrocephalus. Clearance of radioactivity for several calibrated flow rates was determined with a Harvard infusion pump by injecting the Rickham reservoir of a Rickham -Holter valve system with 100 microCi of Tc-99m as pertechnetate. The elliptical and the cylindrical Holter valves used as adjunct valves with the Rickham reservoir yielded two different regression lines when the clearances were plotted against flow rates. The experimental regression lines w

Cerebrospinal fluid ascites: a complication of a ventriculoperitoneal shunt.

A 1 year old Caucasian male born with an omphalocoele, malrotation of the large bowel, and Ladd's bands developed an E. coli wound infection and subsequent meningitis-ventriculitis which responded to antibiotic therapy. Aqueductal stenosis and obstructive hydrocephalus initially was treated with a ventriculoperitoneal shunt. After a routine diphtheria-pertussis-tetanus immunization, the child developed a CSF ascites which resolved following a ventriculoatrial shunt.

Cerebrospinal fluid shunting for hydrocephalus in the adult: factors related to shunt revision.

Cerebrospinal fluid shunting procedures are widely employed in the treatment of hydrocephalus and other disturbances of the dynamics of cerebrospinal fluid. In spite of its popularity, this operation frequently requires surgical revision. A retrospective analysis of a series of 356 adults who underwent the insertion of a cerebrospinal fluid shunt between January 1970 and December 1988 was performed. The incidence of revision was analyzed, and an attempt was made to identify possible causal factors. The overall incidence of surgical revisions was 28.65%; the number of revisions in the same pati

Variables Associated With Shunt Failure in Children With Cerebrospinal Fluid Diverting Shunts.

OBJECTIVES: We sought to identify clinical characteristics that would negatively predict shunt failure, thus potentially obviating the need for further diagnostic workup or extended periods of observation. We hypothesized that viral symptoms and a patient history of epilepsy or chronic headaches would be negative predictors of shunt failure. METHODS: Data were retrospectively collected for children 19 years or younger with a cerebrospinal fluid diverting shunt in their medical history or problem list who underwent neuroimaging during an ED visit from March 2008 to September 2016. Patients wer

Frequency and causes of shunt revisions in different cerebrospinal fluid shunt types.

Shunt complications and revisions are common in hydrocephalic patients treated with a ventriculo-atrial or a ventriculo-peritoneal shunt. The reported revision rate differs very much but the rate of revision is close to 50% in many papers. Data from 884 hydrocephalic patients treated with various shunt types in the period 1958-1989 were recorded retrospectively in order to evaluate the frequency of revision for various shunt types and secondly to analyse the specific reasons for the shunt revisions. The rate of shunt revision was 45%. The Pudenz shunt was revised more often (62%) than the rema

Cerebrospinal Fluid Shunts: An Updated Radiologic Review of Devices, Malfunctions, and Complications.

Cerebrospinal fluid (CSF) shunt devices are essential for managing hydrocephalus by diverting excess CSF from the ventricles and thereby reducing symptoms such as headaches, vision problems, and cognitive deficits. A typical shunt system includes a ventricular catheter, reservoir, valve, and distal catheter. These components are usually made of silastic and radiopaque materials for durability and imaging. The systems also include enhancements like programmable valves, anti-siphon devices, and gravitational units to improve function. Shunts are categorized by outflow destination (ventriculoperi

Ventricular sepsis and abdominally related complications in children with cerebrospinal fluid shunts.

In a retrospective study of 300 children who underwent placement or revision of cerebrospinal fluid (CSF)-peritoneal shunts during a 10-year period, 15 (5%) developed shunt-related abdominal complications with ventricular sepsis and two developed acute perforated appendicitis. Abdominal complications and associated shunt infections suggested two potential modes of development: (1) descent of contaminated CSF from an infected shunt into the abdomen (CSF ascites--four patients, CSF pseudocysts--four patients, and shunt-induced abscess/peritonitis--five patients); and (2) ascent of bacteria into

Solid noninfectious growing masses over cerebrospinal fluid shunts: report of 3 cases.

The authors describe 3 children who presented with progressively enlarging skin-covered solid masses over the shunt catheter in the neck/clavicular region. The authors reviewed the clinical, laboratory, pathological, radiographic, and follow-up data for all 3 patients and reviewed the literature on the subject. The patients had no clinical evidence of an infectious process. Surgical exploration revealed that masses were surrounding and encasing the shunt tubing to which they were strongly attached. Pathological studies of the tissues demonstrated varying degrees of exuberant chronically inflam

Paediatric ventriculoperitoneal shunt obstruction and cerebrospinal fluid protein.

BACKGROUND: Ventriculoperitoneal shunt (VPS) insertion is a life-saving intervention for hydrocephalus. Device failure is extremely common, and carries immense morbidity for patients, their families, and the healthcare system. Predicting shunt obstruction remains a substantial challenge. Clinically, cerebrospinal fluid protein (CSFp) is suspected to contribute to VPS obstruction, this is widely debated, and definitive evidence is lacking. We assess the relationship of CSFp in predicting VPS failure in children. METHODS: A retrospective review of VPS procedures at the Queensland Children's Hos

Cerebrospinal Fluid Shunt Infections: A Multicenter Pediatric Study.

BACKGROUND: Infections complicate 5%-10% of cerebrospinal fluid (CSF) shunts. We aimed to describe the characteristics and contemporary pathogens of shunt infections in children in Canada and the United States. METHODS: Descriptive case series at tertiary care hospitals in Canada (N = 8) and the United States (N = 3) of children up to 18 years of age with CSF shunt infections from July 1, 2013, through June 30, 2019. RESULTS: There were 154 children (43% female, median age 2.7 years, 50% premature) with ≥1 CSF shunt infections. Median time between shunt placement and infection was 54 days (i

Feasibility of laparoscopic cholecystectomy in patients with cerebrospinal fluid shunt.

INTRODUCTION: Previous reports of laparoscopic surgery in patients with cerebrospinal fluid (CSF) shunts for intracranial hypertension described shunt-related complications. Thus, the shunts have been considered a contraindication for laparoscopic procedures. However, with the implementation of recent improvements in surgical techniques, perioperative management, and shunt technology, laparoscopic surgery may now be safe in cases with shunts. The aim of the present study was to examine the safety of such procedures based on our own experiences with laparoscopic surgery in patients with CSF shu

Gonococcal infection in cerebrospinal fluid and the presence of a ventriculoperitoneal shunt.

BACKGROUND: Neisseria gonorrhoeae is one of the most common organisms associated with pelvic disease in a woman of reproductive age. CASE: We present an unusual case of cerebrospinal fluid infection with N. gonorrhoeae in a woman with a ventriculoperitoneal shunt who complained of abdominal pain. Her shunt was removed and after adequate antibiotic therapy, it was re-inserted. CONCLUSION: Sexually active women, especially those with ventriculoperitoneal shunts, should be encouraged to use a barrier method of contraception, and should have a pelvic examination as part of their evaluation when
