[Central laryngeal motor disorders following severe, closed craniocerebral trauma and in cerebrovascular diseases].

111 patients with voice disturbances after severe closed head trauma or cerebro-vascular accident were examined laryngologically. They presented no signs of peripheral nerve involvement. Their main symptoms of central motor dysfunction of the larynx were a reduced motility of the abductors and adductors, a shortening and thickening of the vocal folds, reduced fine control of vocal fold stiffness, and the presence of involuntary laryngeal movements. The frequency of these observations as well as their temporal relation to the onset of the disorder are reported. Possible pathophysiological subst

Mural cell dysfunction leads to altered cerebrovascular tau uptake following repetitive head trauma.

A pathological characteristic of repetitive traumatic brain injury (TBI) is the deposition of hyperphosphorylated and aggregated tau species in the brain and increased levels of extracellular monomeric tau are believed to play a role in the pathogenesis of neurodegenerative tauopathies. The pathways by which extracellular tau is eliminated from the brain, however, remains elusive. The purpose of this study was to examine tau uptake by cerebrovascular cells and the effect of TBI on these processes. We found monomeric tau interacts with brain vascular mural cells (pericytes and smooth muscle cel

Congenital spine deformities: a new screening indication for blunt cerebrovascular injuries after cervical trauma?

Blunt cerebrovascular injuries (BCVI) carry significant morbidity if not diagnosed and treated early. A high index of clinical suspicion is needed to recognize the injury patterns associated with this condition and to order the requisite imaging studies needed to diagnose it accurately. We report of BCVI associated with a congenital cervical spine malformation after blunt trauma. We recommend inclusion of cervical spine malformations to the current Eastern Association for the Surgery of Trauma screening criteria for BCVI and explain our rationale for the same.

Racial/ethnic disparities in morbidity and mortality among adults with cerebrovascular injury following blunt trauma.

OBJECTIVE: To assess the impact of race/ethnicity on adverse events (AEs), in-hospital mortality, and non-routine discharge among BCVI patients. METHODS: A retrospective cohort study was conducted using the American College of Surgeons Trauma Quality Programs Participant Use Files (2017-2023). Multivariable logistic regression analyzed associations between race/ethnicity and clinical outcomes. RESULTS: Among 44,898 BCVI patients, most were non-Hispanic White (NHW, 64.5%), followed by non-Hispanic Black (NHB, 14.9%), Hispanic Latino (HL, 10.5%), and other (10.1%). NHW patients were older and

Revascularization surgical options after carotid trauma: Case report from a cerebrovascular blood flow preservation overview.

An 18-year-old male underwent a tracheostomy that was complicated by an iatrogenic left-sided common carotid artery laceration, leading to loss of blood flow and a minor stroke in the anterior and medial territories of the left cerebral circulation. After the lesion was identified, carotid exploration was performed, followed by a vascular reconstruction using a synthetic graft to repair the damaged segment. This manuscript details the microsurgical techniques utilized and discusses the indications for this intervention, with a particular emphasis on stroke prevention.

Cervical seatbelt sign is not associated with blunt cerebrovascular injury in children: A review of the national trauma databank.

BACKGROUND: Blunt cerebrovascular injury (BCVI) is a rare consequence of blunt trauma. There appears to be benefit to an aggressive approach to screening for BCVI due to catastrophic sequelae of unrecognized injury. However, screening for BCVI carries extensive cost and oncologic risk to young patients. Foundational BCVI studies examined adults primarily, leaving question to the effectiveness of these criteria in children. We sought to evaluate BCVI screening criteria developed in primarily adult populations using a nationally representative pediatric dataset. METHODS: We queried the 2008-201

Blunt cerebrovascular trauma causing vertebral arteryd issection in combination with a laryngeal fracture: a case report.

INTRODUCTION: The diagnosis and therapy of blunt cerebrovascular injuries has become a focus since improved imaging technology allows adequate description of the injury. Although it represents a rare injury the long-term complications can be fatal but mostly prevented by adequate treatment. CASE PRESENTATION: A 33-year-old Caucasian man fell down a 7-meter scarp after losing control of his quad bike in a remote area. Since endotracheal intubation was unsuccessfully attempted due to the severe cervical swelling as well as oral bleeding an emergency tracheotomy was performed on scene. He was he

Repetitive head trauma and apoE4 induce chronic cerebrovascular alterations that impair tau elimination from the brain.

Repetitive mild traumatic brain injuries (r-mTBI) sustained in the military or contact sports have been associated with the accumulation of extracellular tau in the brain, which may contribute to the pathogenesis of neurodegenerative tauopathies. The expression of the apolipoprotein E4 (apoE4) isoform has been associated with higher levels of tau in the brain, and worse clinical outcomes after r-mTBI, though the influence of apoE genotype on extracellular tau dynamics in the brain is poorly understood. We recently demonstrated that extracellular tau can be eliminated across blood-brain barrier

Antithrombotic choice in blunt cerebrovascular injuries: Experience at a tertiary trauma center, systematic review, and meta-analysis.

BACKGROUND: Blunt cerebrovascular injuries (BCVIs) may occur following trauma and lead to ischemic stroke if untreated. Antithrombotic therapy decreases this risk; however, the optimal agent has yet to be determined in this population. The aim of this study was to compare the risk-benefit profile of antiplatelet (AP) versus anticoagulant (AC) therapy in rates of ischemic stroke and hemorrhagic complications in BCVI patients. METHODS: We performed a retrospective review of BCVI patients at our tertiary care Trauma hospital from 2010 to 2015, and a systematic review and meta-analysis of the lit

The necessity of CT/CTA scans on pediatric cerebrovascular injury after blunt trauma - a single center study.

BACKGROUND: Blunt cerebrovascular injury (BCVI) is a rare but potentially devastating complication of pediatric blunt trauma. Delayed or missed diagnosis may result in ischemic stroke, permanent neurological deficits, or death. To avoid missed injuries, many trauma centers apply liberal computed tomography angiography (CTA) screening strategies, although this may expose children to substantial and potentially unnecessary radiation. Several adult- and pediatric-derived screening tools have been proposed, but their applicability in pediatric trauma populations remains controversial. This study a

Fall downs should not fall out: Blunt cerebrovascular injury in geriatric patients after low-energy trauma is common.

BACKGROUND: There are limited data examining the impact of screening for blunt cerebrovascular injury (BCVI) in the geriatric population sustaining falls. We hypothesize that BCVI screening in this cohort would rarely identify injuries that would change management. METHODS: A retrospective study (2012-2016) identified patients 65 years or older with Abbreviated Injury Scores for the head and neck region or face region of 1 or greater after falls of 5 ft or less. Patients who met the expanded Denver criteria for BCVI screening were included for analysis. Outcomes were change in management (def

Presence of intraluminal thrombus in grade 2 blunt cerebrovascular injuries does not increase risk of stroke in trauma patients.

INTRODUCTION: As screening protocols for blunt cerebrovascular injuries (BCVI) have improved, the incidence of these injuries has increased among trauma patients. Grade 2 BCVIs represent a heterogeneous group of vascular injuries and include injuries with thrombus. We hypothesize that the presence of intraluminal thrombus in patients with grade 2 BCVI is associated with a higher incidence of stroke compared to those without thrombus. METHODS: We conducted a single-center retrospective review of trauma patients diagnosed with BCVI at a Level I Trauma Center from November 2015 to October 2023.

Diagnostic yield and clinical predictors of blunt cerebrovascular injury: A CTA utilization review at a level I trauma center.

PURPOSE: Blunt cerebrovascular injury (BCVI) is a potentially devastating complication of high-energy trauma that can lead to ischemic stroke if undetected. Computed tomography angiography (CTA) is the primary screening modality, though optimal patient selection criteria remain debated. Rising CTA utilization at our institution prompted a quality improvement review to evaluate the diagnostic yield and identify clinical predictors of BCVI. METHODS: A retrospective review was conducted of trauma patients who underwent CTA of the head and neck at a Level I trauma center between January and Decem
