Selected pediatric emergencies in community practice.

In children, the signs and symptoms of serious infection often mimic those observed with minor, self-limiting diseases. One of the most important steps in making a diagnosis of an infection of the central nervous system is to suspect that an infection may be present. Acute epiglotitis predominantly affects children 2 to 7 years of age; delays in its diagnosis or treatment may result in death within a matter of hours. Pharmacists should be aware of the signs and symptoms of child abuse and neglect and understand the reporting procedures and requirements.

Septic bursitis: experience in a community practice.

We reviewed 47 episodes of septic bursitis occurring in a private community medical practice. Most patients were male (85%), and roughly half (49%) the cases were related to recreational or occupational trauma. About 72% of cases were located in the olecranon bursa, while the remaining cases were prepatellar. Prepatellar bursitis patients were more likely to be hospitalized. Staphylococcus aureus was isolated from 70% of bursal fluid aspirations; other etiologic organisms included gram negative bacteria and Mycobacterium marinum. The majority of patients were able to be treated as outpatients

The influence of spirituality on community nursing practice and patient experience.

Spirituality plays a significant role in end-of-life and palliative care, influenced by patients' religious and cultural backgrounds. Facing the reality that one's own life or that of a loved one is nearing its end can evoke feelings of hopelessness, anxiety and depression. Spiritual practices, such as invoking the intercession of saints or patrons, can provide comfort and hope, and a sense of meaning during this challenging and difficult time. Integrating these spiritual interventions into palliative care can support emotional wellbeing and enhance the holistic care of patients and their fami

Diagnosis and Treatment of Vaginal Discharge Syndromes in Community Practice Settings.

BACKGROUND: Although vaginal symptoms are common, diagnosis of bacterial vaginosis (BV), vulvovaginal candidiasis (VVC), and Trichomonas vaginalis (TV) is not standardized. Diagnostic approaches and appropriateness of treatment were evaluated for women with symptoms of vaginitis who were seeking care at community practice sites. METHODS: Three hundred three symptomatic women, across 8 University of Pittsburgh Medical Center-affiliated clinics, were evaluated per standard office-based practice. Four of 5 vaginal swabs (1 cryopreserved) were collected for a US Food and Drug Administration-autho

Monitoring complications of botulinum toxin in community practice: A pharmacological study.

Botulinum toxin is widely used for cosmetic wrinkle reduction, hyperhidrosis, chronic migraine and facial muscle disorders, but concerns persist regarding adverse effects related to injection technique, toxin diffusion and practitioner variability. Therefore, it is of interest to evaluate the complication rates of community-based botulinum toxin therapy and characterize the clinical presentations of its associated adverse effects. A prospective pharmacological observational study was conducted on 120 patients receiving botulinum toxin type A injections. Majority of complications were mild and

Performance characteristics of digital vs film screen mammography in community practice.

We compared the performance characteristics of 297 629 full field digital (FFDM) and 416 791 screen film mammograms (SFM). Sensitivity increased with age, decreased with breast density, and was lower for more aggressive and lobular tumors. While sensitivity did not differ significantly by modality, specificity was generally 1%-2% points higher for FFDM than for SFM across age and breast density categories. The lower recall rate for FFDM vs SFM in our study may partially explain performance differences by modality. In this large health care organization, modest gains in performance were achieve

Replantation and revascularization of the digits in a community microsurgical practice.

One-hundred and twenty-seven digits in Verdan zones I and II, and 24 thumbs in 106 patients were reviewed from 1979 to 1995. Patients were all followed for a minimum of 5 months to assess early results and complications, and to determine if a short course (minimum 3 days) of dextran 40 anticoagulation adversely affected digit survival. The total survival rate was 88 percent. Digits with only a single artery and vein repaired had a significantly higher rate of failure (p > .05). Index and small fingers also showed a trend toward lower survival. Vein grafts were used in 22 percent of cases. No v

The nature of an infectious disease practice in a community hospital.

Populations of patients in community and university-affiliated teaching hospitals differ, and therefore, problems encountered by infectious disease specialists in these two types of hospitals also differ. However, most infectious disease specialists are trained only in university hospitals. In order to characterize the nature of an infectious disease practice in a community hospital, the authors report data for the patients seen during a period of three years (July 1978-June 1981). Most of the 1,238 cases were referred by other physicians. General practitioners referred 35% of the cases, where

Community engagement to implement evidence-based practices in the HEALing communities study.

BACKGROUND: The implementation of evidence-based practices to reduce opioid overdose deaths within communities remains suboptimal. Community engagement can improve the uptake and sustainability of evidence-based practices. The HEALing Communities Study (HCS) aims to reduce opioid overdose deaths through the Communities That HEAL (CTH) intervention, a community-engaged, data-driven planning process that will be implemented in 67 communities across four states. METHODS: An iterative process was used in the development of the community engagement component of the CTH. The resulting community eng
