Posterior keratoconus is an unusual abnormality of the cornea generally classified as one of the anterior chamber cleavage anomalies. It is characterized clinically by the presence of a circumscribed or generalized corneal thinning with posterior depression of the cornea and is considered distinct from keratoconus. Although patients with posterior keratoconus may have visual complaints clearly related to their abnormal corneas, the surface topography of these corneas has not been studied in detail. Keratometry and photokeratoscopy provide an incomplete picture of the surface geometry of poster

A new method is described for calculating the topography and curvature of an irregularly shaped cornea. Vertical planes of light are projected onto the cornea and points in x, y, z space are calculated from the light images on the corneal surface. A matrix of points is produced on the cornea and a mathematical surface is fitted to them. Using differential geometry theory, curvature values are calculated to find the corneal apex position, apex curvature and curvature change away from the apex. Typical results, in the form of a contour map, are shown for normal corneas and corneas with mild to s

Trabeculectomy is increasingly being performed earlier in the treatment of primary open-angle glaucoma (POAG). Surgically-induced alterations in corneal curvature resulting from filtering surgery may have a significant impact on patients, especially younger ones, whose vision may already be compromised by visual-field loss. Using data obtained by computer-assisted topographic analysis performed preoperatively and at 12 weeks postoperatively, we longitudinally studied the corneal surface changes induced by primary trabeculectomy in eight eyes undergoing surgery for POAG or normal-tension glauco

Recent advances in topographic analysis have provided powerful tools for detecting subtle, but clinically significant, alterations of corneal contour. This article compares keratometry, keratoscopy, and computer-assisted topographic analysis and provides specific examples of the sensitivity of computer-assisted systems in revealing topographic alterations that were not previously discernable. Quantitative descriptors of corneal topography such as the surface asymmetry index, the surface regularity index, and simulated keratometry value augment the information provided by color-coded topographi

OBJECTIVE: To evaluate the corneal topography and pachymetry in keratoconus.
METHODS: Thirty-five patients (47 eyes) with keratoconus were evaluated by Orbscan corneal topography system. The location, anterior and posterior surface elevation, curvature and pachymetry were analyzed with special reference to the central point of cornea, the apex, the highest point of posterior surface, the point with greatest curvature and the thinnest point. The correlation between the point of greatest curvature and the apex was also analyzed.
RESULTS: The mean elevation of the apex was (0.102 +/- 0.044) mm,

Keratometry provides useful information about the cornea's image-forming properties, such as corneal astigmatism, but is inaccurate on irregular corneas. Quantitative corneal topographic information is now obtainable on irregular corneas, but is difficult for the clinician to interpret. We developed a method to determine the spherical power, astigmatism, and topographic irregularity of a cornea by finding the best-fit spherocylinder that was closest to its measured topography. Keratometric measurements and two videokeratographs were gathered prospectively on 262 normal and abnormal corneas. Th

PURPOSE: To evaluate the effect of eyelid laxity on the structure of the cornea.
METHODS: This was a retrospective study performed in one institution. Patients with and without upper and lower eyelid laxity, which were determined using the snap-back test (SBT) and the upper eyelid distraction test (LDT), were evaluated for corneal structural changes with Scheimpflug imaging corneal tomography for the keratometric readings, and cor-neal spectral domain optical coherence tomography for the epithelial thickness (ET) measurements. The ET values were obtained using the statistics report for the su

Corneal topography analysis has become an indispensable tool in the evaluation and treatment of corneal disease. It is helping us understand the normal corneal shape and how it is altered in ectatic diseases, by contact lens wear, and by surgery. Topography analysis is also now being used in planning procedures to correct astigmatism and in the fitting of contact lenses.

OBJECTIVES: Systemic sclerosis (SSc) is a rare autoimmune disease associated with vasculopathy. Raynaud's phenomenon is a common symptom of SSc, and treatment guidelines support calcium channel blocker (CCB) and phosphodiesterase inhibitor-5 (PDE5i) as first- and second-line therapy, respectively. However, the impact of these drug classes on peripheral vascular function or haemodynamic outcomes in SSc is unknown.
METHODS: Flow-mediated dilation (FMD) was measured in patients with SSc (n=119) that were allocated into groups based on cardiovascular-acting medications usage within the previous 2

1. The incidence of keratoconus is about 1 in 2,000 in the general population, but is believed to be higher if subclinical forms are taken into account. 2. Corneal topography helps to detect keratoconus when other testing methods may have missed early signs of the disease. 3. At a time when refractive surgery is common, it is essential to identify corneal abnormalities prior to surgery.

Using a computer-assisted video-keratoscope, we determined the topographic findings of 14 eyes of 10 patients with keratoconus. None of these patients had worn contact lenses for at least one year prior to examination and were thus free of contact lens-induced topographical alterations. Two patterns commonly seen included inferior steepening more prominent temporally, and central steepening with a superimposed asymmetric bowtie pattern. When compared with normal eyes three quantitative parameters were significantly different (p less than 0.001): central corneal power, difference in corneal pow

A new method was developed to quantify corneal irregular astigmatism using computed corneal topography. Refractive powers on a mire ring projected on a toric plane (regular cornea) can be approximated to follow a sine curve. The discrepancy between the approximated sine curve and actual refractive powers was calculated and employed as a parameter of corneal irregular astigmatism. Artificial precision models of a sphere and a toric plane showed similar parameters and the least amount of irregular astigmatism (0.02-0.05D). Consecutive measurements on normal human corneas displayed a reproducible
