Robotic coronary artery bypass for aberrant right coronary artery stenosis.

Anomalous coronary arteries that course between the aorta and pulmonary artery are subject to compressive forces and can manifest angina, myocardial infarction and sudden death. The current report presents a young, female patient who presented with a short duration of severe, rapidly progressive angina despite optimal medical therapy. Combined computed tomography and myocardial perfusion scanning identified an anomalous dominant right coronary artery that appeared kinked at its origin between the aorta and main pulmonary artery. A robot-assisted right internal thoracic artery to right coronary

Iatrogenic left main coronary artery stenosis.

Iatrogenic left main coronary artery stenosis is a potentially life-threatening complication of cardiac valve replacement surgery due to injury by perfusion cannulas. This requires prompt clinical recognition and diagnosis by repeat coronary angiography, and treatment by early coronary artery bypass grafting. We present 3 patients who had normal coronary arteries prior to valve replacement surgery, and who developed severe left main coronary artery stenosis after surgery. Accelerating angina and refractory ventricular arrhythmia were presenting clinical manifestations. Coronary artery bypass g

[Coronary embolism revealing mitral valve stenosis].

We report a case of myocardial infarction with normal coronary arteriography in a 39-year-old woman presenting with tight mitral stenosis complicated by atrial extrasystoles and spontaneous atrial contrast at echocardiography. The diagnosis of coronary embolism was most probable. Coronary embolism is a rare disease, usually due to a blood clot or, less frequently, to a vegetation of endocarditis starting in a heart valve. The left network is usually involved, with typical myocardial necrosis. Coronary arteriography is sufficient to make the diagnosis. Preventive treatment is essential.

[Coronary occlusive disease and carotid stenosis].

In around 2.5% of patients with coronary artery diseases (CAOD), considerably occlusive changes in carotid arteries were also registered at the same time. In the period from March 1982 to February 15, 1993, at the University Clinic of Cardiovascular surgery in Novi Sad, a total of 5,701 patients underwent aortocoronary by-pass (ACBG). In 137 (2.4%) patients an additionally endarterectomy of carotid arteries (EAC) were also performed. Bad left ventricle function (EF < 30%) was registered in 30 pts (22%). Endarterectomy on one or more arteries had to be performed in 45% patients because diffuse

Syphilitic coronary ostial stenosis. Case reports.

Two young Coloured men with proven syphilitic coronary ostial stenosis had severe angina pectoris unresponsive to conventional medication. One underwent an aortic valve replacement for severe aortic insufficiency associated with subtotal ostial occlusion of the right coronary artery (RCA), which was corrected by an aortocoronary bypass graft; the left coronary artery (LCA) ostium was normal and patent. The other patient had total occlusion of the LCA ostium which resulted in an extensive transmural anteroseptal and anterolateral myocardial infarction; the RCA ostium was unaffected and the aort

A simplified coronary model for diagnosis of ischemia-causing coronary stenosis.

BACKGROUND AND OBJECTIVE: The functional assessment of the severity of coronary stenosis from coronary computed tomography angiography (CCTA)-derived fractional flow reserve (FFR) has recently attracted interest. However, existing algorithms run at high computational cost. Therefore, this study proposes a fast calculation method of FFR for the diagnosis of ischemia-causing coronary stenosis. METHODS: We combined CCTA and machine learning to develop a simplified single-vessel coronary model for rapid calculation of FFR. First, a zero-dimensional model of single-vessel coronary was established …

Coronary angioplasty of a severe coronary stenosis at the site of a myocardial bridge.

We report the case of a patient who underwent coronary angioplasty for a severe stenosis at the site of a myocardial bridge. Although favorable angiographic results and the disappearance of myocardial ischemia were obtained, a significant pressure gradient across the lesion and abnormal myocardial fractional flow reserve remained immediately following angioplasty. These results suggest that pressure measurements may be of limited value in this subset of patients.

Cardiovascular syphilis with coronary stenosis and aneurysm.

Cardiovascular manifestations of tertiary syphilis include aortitis, aortic root dilation, aneurysm formation, aortic regurgitation, and coronary ostial stenosis. Coronary ostial lesions have been detected in as many as 26% of patients with syphilitic aortitis. However nonostial coronary stenosis and coronary aneurysms in same patient is rarely described in cardiovascular syphilis.

Coronary atherosclerosis in subjects with mitral stenosis.

The coronary arteries were examined in 60 specimens from patients with mitral stenosis. In three, localized obstruction was nonatherosclerotic in nature (in one, arterial dysplasia; in two, embolic). In 18 of the remaining 57 cases (31.5%), significantly obstructive atherosclerosis in one or more segments of the coronary arterial system was found. This represented 46% of the males and 27% of the females. The incidence of involvement of three or more arteries by significantly obstructive atherosclerosis was 39%, while in a cited series of subjects with angina pectoris three or more vessels were

Artificial Intelligence-based Coronary Stenosis Quantification at Coronary CT Angiography versus Quantitative Coronary Angiography.

Purpose To evaluate the performance of a new artificial intelligence (AI)-based tool by comparing the quantified stenosis severity at coronary CT angiography (CCTA) with a reference standard derived from invasive quantitative coronary angiography (QCA). Materials and Methods This secondary, post hoc analysis included 120 participants (mean age, 59.7 years ± 10.8 [SD]; 73 [60.8%] men, 47 [39.2%] women) from three large clinical trials (AFFECTS, P3, REFINE) who underwent CCTA and invasive coronary angiography with QCA. Quantitative analysis of coronary stenosis severity at CCTA was performed usi…

Incidental Coronary Sinus Stenosis During Coronary Angiography: A Rare Angiographic Finding.

BACKGROUND: The coronary sinus (CS) is a major venous drainage pathway of the heart but is rarely the site of clinically significant pathology. When obstruction of the CS occurs, it can lead to serious hemodynamic consequences and a high risk of mortality if not promptly identified. CASE PRESENTATION: We report the case of a 60-year-old male with Type 2 diabetes mellitus on chronic hemodialysis, who presented with chest pain and dyspnea. While undergoing diagnostic coronary angiography for suspected ischemia, severe stenosis of the CS was incidentally observed. The patient had elevated left v

Reduced coronary flow reserve in hypercholesterolemic patients without overt coronary stenosis.

BACKGROUND: Reduced coronary flow reserve (CFR) in hypercholesterolemic patients without evidence of ischemia has been reported. However, it remains uncertain whether this abnormality occurs without overt coronary atherosclerosis. This study aimed to clarify whether CFR is impaired even in anatomically normal coronary arteries in hypercholesterolemic patients and to compare CFR between familial hypercholesterolemic (FH) patients and secondary hypercholesterolemic (SH) patients. METHODS AND RESULTS: Twenty-two patients with hypercholesterolemia (11 FH, 11 SH) and 11 control subjects were studi

[Correlations between Fractional Flow Reserve and Coronary Artery Stenosis Based on the Stenosis Position and Coronary Dominant Type].

For coronary artery diseases, imaging diagnosis is usually used to guide the treatment. However,it can only reflect the geometric characteristics of the disease but does not determine the hemodynamically significant stenosis. This study was aimed to investigate the relationship between angiographic and functional severity of coronary artery stenosis and to improve the diagnostic value of imaging.39 patients with 55 stenosis vessels were included in this study. The correlation between FFR and stenosis rate was analyzed with the medical statistical analysis method, and the influence of the position o

Isolated bilateral ostial coronary stenosis with proximal right coronary artery occlusion.

The incidence of coronary ostial stenosis in patients undergoing coronary arteriography has been found to range between 0.07 and 0.25%. A slightly higher incidence has been observed in patients with angiographically confirmed coronary artery disease: between 0.13 and 2.7%. Bilateral ostial stenosis is even less common. Although cardiovascular syphilis is no longer a prominent condition, it must be considered in the differential diagnosis since it carries a very high risk (50%) of cardiovascular complications if left untreated. Ostial coronary stenosis occurs in 26% of patients with syphilitic …
