Bone densitometry in patients with multiple myeloma.

PURPOSE, PATIENTS, AND METHODS: We performed dual-energy x-ray absorptiometry in 10 selected patients with aggressive multiple myeloma in whom substantial tumor mass reduction was achieved after high-dose chemoradiotherapy followed by autologous blood stem cell transplantation. RESULTS: In most cases, bone mineral density (BMD) of the spine was initially low (Mean Z score: -2.69, SEM 0.76) and dramatically increased after treatment (mean increase 16.4%; 7.7% with 95% confidence interval 2.2 to 12.2, excluding one patient whose spine BMD increased by 94.8%). In contrast, skeletal roentgenogram

CT densitometry of the cochlear capsule in otosclerosis.

Cochlear otosclerosis can be recognized in most cases by high-definition axial and coronal CT images. However, routine technique does not allow a quantitative measurement of the changes. Reported here is a study of cochlear CT densitometry. The densitometric profile of the cochlear capsule was obtained in 10 ears with normal hearing and 50 ears in 27 patients with known clinical otosclerosis and progressive mixed-type hearing loss. Patients with hearing loss had decreased absorption of as much as 60% in comparison to the normal range. In 42 ears with abnormal densitometric profiles, the change

Influence of iris color on corneal densitometry.

PURPOSE: To determine whether iris pigmentation introduces a measurable bias in corneal densitometry (CD) values obtained via Scheimpflug imaging, and to develop an objective metric for iris color quantification. METHODS: This observational study included 91 eyes from 47 healthy adults. CD was assessed as mean pixel intensity (MPI) from 25 Scheimpflug images per eye. Brightness artefacts from the iris were quantified using automated image processing. Iris color was objectively characterized from slit-lamp photographs using an objective single-value metric (IrisColor) derived from normalized C

Influence of Room Illumination on Corneal Densitometry.

IMPORTANCE: Newborn screening (NBS) for lysosomal storage disorders (LSDs) is becoming an increasing concern in public health. However, the birth prevalence of these disorders is rarely reported in the Chinese population, and subclinical forms of diseases among patients identified by NBS have not been evaluated. OBJECTIVE: To evaluate the birth prevalence of the 6 LSDs in the Shanghai population and determine subclinical forms based on clinical, biochemical, and genetic characteristics. DESIGN, SETTING, AND PARTICIPANTS: This cohort study included 50 108 newborns recruited from 41 hospitals

Bone densitometry in a patient with hypophosphatemic osteomalacia.

A 60-year-old Caucasian woman with a 1-year history of pain at the ribs, spine, and pelvis consulted at our Institute in March 1999. She brought a bone densitometry performed using a Lunar DPX densitometer that showed bone mineral density (BMD) measurements in the osteoporotic range at both the lumbar spine and the femoral neck. As a child she had had bowed legs and had been treated with ultraviolet radiation. Results of the laboratory test performed at our institute showed normal total serum calcium, repeated low serum P levels, and a low renal phosphate threshold with elevated total and bone

[The analysis of the gastric mucosa by computerized densitometry].

An analytical method of histologic imaging of the gastric mucosa by computerized densitometry based on the extraction of components of an image on the basis of its luminous density is described. A group of 10 healthy controls, a group of 10 subjects with gastric ulcer and 10 with duodenal ulcer were included in the study. The method was particularly useful in the identification of atrophic and hypertrophic processes. A decrease was found in the density of parietal and chief cells in atrophic gastritis of the gastric body. In atrophic gastritis of the gastric antrum a decrease was observed in g

Bone scintigraphy and densitometry in children with osteopetrosis.

Bone scintigraphy and dual x-ray absorptiometry were performed in 18 children (8 males, 10 females) with clinical and radiologic diagnoses of osteopetrosis in order to demonstrate the scintigraphic features of this rare disorder and to measure the bone mineral density. Their mean age was 9 years (range, 3-16 years). Bone scintigraphy demonstrated characteristic features of a widened metaphysis of all long bones that showed increased tracer uptake, particularly in the distal femur and proximal tibia. Dual x-ray absorptiometry of the lumbar spine, three femoral sites, and total body showed a mar

Bone Mineral Densitometry Reporting: Pearls and Pitfalls.

Dual-energy X-ray absorptiometry (DXA) is the method of choice for assessing bone mineral density (BMD). Unfortunately, the performance and interpretation of DXA can be challenging and errors are common. In fact, it has been reported that up to 90% of BMD reports contain at least 1 error. Errors can be the result of technique or interpretative in nature or both and can result in inappropriate diagnosis and management. In this article, we review the various types of pitfalls frequently encountered by physicians interpreting DXA studies. Being aware of these pitfalls will help readers recognize

Accidental detection of Chondrocalcinosis through REMS densitometry.

Chondrocalcinosis, defined as the radiographic evidence of calcium pyrophosphate dihydrate (CPPD) crystal deposition within joint structures, often presents variably, ranging from asymptomatic forms to overt clinical manifestations. Radiofrequency Echographic Multi Spectrometry (REMS) is an advanced imaging modality traditionally employed for assessing bone mineral density (BMD), but it also exhibits potential in identifying incidental findings suggestive of crystal arthropathies such as chondrocalcinosis. This report presents a case of a 65-year-old female patient with a history of a vertebra

Postmenopausal osteoporosis: current status of bone densitometry.

Osteoporosis is the most common of all metabolic bone disorders characterized by loss of bone strength, due to modifications in bone turnover. It leads to bone fragility and increased fracture risk. Because of the increasing aging of the world population, the number of people affected by osteoporosis is continuously increasing. The WHO operational definition of osteoporosis, based on a measurement of bone mineral density (BMD) by dual-energy X-ray absorptiometry (DXA), identifies patients at greatest risk of fracture. However, in the population overall a greater total number of fractures occur

Clinical use of bone densitometry: clinical applications.

Osteoporosis represents a difficult problem for physicians because, although many diagnostic tests are available, interpreting their results is not straightforward. As a result, many patients, even those with clear indications such as long-term steroid therapy or vertebral fractures on radiography, do not get screened or treated. Current evidence-based guidelines recommend screening for all white women older than 65 years and not already receiving an osteoporosis treatment and for many nonwhite women. For postmenopausal women who are younger than 65 years and have strong risk factors for osteo

[Dual-photon bone densitometry in dialyzed patients].

In non-invasive diagnosis of renal osteodystrophy the levels of bone minerals and the extent of bone turnover are evaluated. The contents of bone minerals are assessed quantitatively by different modalities of bone densitometry, among which the most accurate one is double-energy bone densitometry. So far no standard examination method was defined nor the most suitable portion of the skeleton for densitometric examination. In order to find such an area and also to assess the prevalence of bone demineralization, its severity and regional differences the authors made a cross-sectional study of bo

Single photon bone densitometry in hemodialysis patients.

Renal osteodystrophy is a common finding in patients with renal insufficiency. The maximum of its intensity is found in hemodialysis patients. Bone densitometry is so far the best method for non-invasive assessment of the extent of the illness. Some densitometric studies in hemodialysis patients have already been published but their results differ in prevalence and intensity of renal osteodystrophy. They also demonstrated a slight relationship between intensity of renal osteodystrophy and duration of the dialysis treatment. Opinions vary on the relationship between bone mineral density and mar

Peripheral bone densitometry: an old friend revisited.

The earliest assessments of bone "mass" involved metacarpal morphometry that provided insight into age-related changes, the effects of low habitual dietary calcium intake, and the effects of estrogen deficiency and replacement. Single photon absorptiometry (SPA) made quantitative mass measurement possible but this was intellectually unsatisfactory since osteoporotic fractures are more of a concern at the spine and hip than at the wrist. Necessity forced the development of axial bone mass measurement (dual photon absorptiometry--DPA, dual energy xray absorptiometry--DXA, quantitative computed t

Bone densitometry: patients receiving prolonged steroid therapy.

Bone mass loss and osteoporosis are associated with various conditions, such as asymptomatic primary hyperparathyroidism, and treatments, such as prolonged steroid therapy. Bone densitometry is used to measure bone mass density to determine the degree of osteoporosis and to estimate fracture risk. Bone densitometers measure the radiation absorption by the skeleton to determine bone mass of the peripheral, axial, and total skeleton. Common techniques include single-photon absorptiometry (SPA) of the forearm and heel, dual-photon (DPA) and dual-energy x-ray absorptiometry (DXA) of the spine and
