The socket-shield technique: a step-by-step protocol after 12 years of experience.

The socket-shield technique, first published in 2010, has gained worldwide scientific and clinical acceptance. To address possible complications with this innovative approach in esthetic implant dentistry, we provide a comprehensive step-by-step protocol incorporating what we have learnt in the past decade. After initial decoronation of the tooth, the implant bed is prepared through the root of the tooth to be extracted. Following extraction of the palatal root fragments, the shield is prepared according to either a mechanical or biologic 'locking' principle. The mechanical 'locking' comprises

The effect of implant macrogeometry in immediate tooth replacement therapy: A case series.

The concept of immediate tooth replacement therapy (ITRT) in the esthetic zone has become a viable treatment modality and widely accepted in clinical practice. However, achieving adequate primary stability that enables immediate provisional restoration and desirable space for bone-grafting with conventional tapered body implant designs is challenging. The macro-hybrid implant design with a prosthetic angle correction and body-shift feature in a singular form provides not only greater primary stability of a wider diameter implant but also circumferential gap distance both facially and interprox

Timing of Implant Placement after Traumatic Dental Injury.

Dental implants are reliable to replace teeth lost because of traumatic dental injury. However, dental implants behave like ankylosed teeth and should not be placed in growing individuals because of the risk of infraposition. This risk may be reduced by ensuring arrested skeletal growth and ideal incisal support. The timing of implant placement may be immediate, early, conventional, or late and is determined by the extent of the trauma, remaining growth, and conditions of the hard and soft tissues. Timing should allow an observation period to properly evaluate the prognosis of concomitantly tr

Immediate implant placement and loading in the esthetic area when the buccal socket wall is significantly damaged.

OBJECTIVES: Recent clinical recommendations contraindicate immediate implant placement when the socket buccal bone plate is significantly damaged. The connective tissue graft (CTG) is increasingly being used in implant therapy and can replace periodontal defects lacking bone wall in periodontal regenerative surgery. Therefore, CTG could be used to allow immediate implant placement and loading even when the buccal socket wall is damaged, facilitating graft material stability. CLINICAL CONSIDERATIONS: In the first case, deep bone dehiscence was caused by a vertical root fracture. In the second

Avoiding Sinus Floor Elevation by Placing a Palatally Angled Implant: A Morphological Study Using Cross-Sectional Analysis Determined by CBCT.

BACKGROUNDS: Tooth loss in the posterior maxilla often necessitates dental implant placement, but the maxillary sinus anatomy poses challenges, especially during sinus floor elevation. Mesially angled implants are an alternative for total edentulism, but for single tooth deficiencies, palatally angled implants may offer a solution. This study evaluates the prevalence of avoiding sinus floor elevation by placing palatally angled implants in cases with a single missing tooth. METHODS: A retrospective study at Ahmet Keleşoğlu Faculty of Dentistry involved 100 participants with a single missing t

Preclinical Investigation of Artificial Intelligence-Assisted Implant Surgery Planning for Single Tooth Defects: A Case Series Study.

INTRODUCTION: Dental implant surgery has become a prevalent treatment option for patients with single tooth defects. However, the success of this surgery relies heavily on precise planning and execution. This study investigates the application of artificial intelligence (AI) in assisting the planning process of dental implant surgery for single tooth defects. Single tooth defects in the oral cavity pose a significant challenge in restorative dentistry. Dental implant restoration has emerged as an effective solution for rehabilitating such defects. However, the complexity of the procedure and t

A Modified Socket-Shield Technique: Simplifying the Root Sectioning Process.

Socket wall resorption leading to a loss of surrounding bone following tooth extraction has been documented in the dental literature. The use of various socket-shield techniques has been suggested as a solution to this issue. In these approaches, the tooth root is sectioned in two, and the coronal two-thirds of the buccal root is preserved in the socket. This allows the periodontium along with the bundle and buccal bone to remain intact, thus preventing or minimizing bone remodeling. According to the literature, this procedure is highly technique sensitive, especially when it comes to sectioni

Complication Management of a Socket Shield Case After 6 Years of Function.

Maintaining soft and hard tissues around dental implants after tooth extraction is one of the major challenges in implant dentistry. After tooth extraction, the subsequent loss of bone and soft tissue is inevitable due to the partial resorption of the buccal bone plate. The recently described socket shield technique addresses the problem by maintaining the buccal piece of the tooth in the extraction socket in order to preserve the buccal bone. As with every new technique, specific complications, like infection of the buccal piece of the tooth, can occur. Herein, the authors present a clinical

Conventional and Basal Implantology: A Narrative Review of Current Evidence, Clinical Decision-Making, and Future Perspectives.

Dental implantology has transformed tooth replacement by offering reliable, functional, and aesthetic solutions. Conventional implants rely on adequate alveolar bone and often require staged procedures with delayed loading. They come in various designs with surface treatments to enhance osseointegration. Basal implants, in contrast, anchor into the dense cortical bone, enabling immediate loading without bone grafting. These single-piece implants have polished surfaces to reduce infection and require minimal surgical intervention. While basal implants offer faster treatment and are suitable for
