Although keratoplasties are key surgical procedures for treating corneal diseases, they may present late complications such as retrocorneal membranes. We present an unusual case following a penetrating keratoplasty (PKP) in a patient with a history of DMEK. This is the case of an 85-year-old woman with a past medical history of DMEK and 2 PKPs due to previous graft failures, who developed a retrocorneal membrane 6 months after the last PKP. A membranectomy was performed. Histological analysis confirmed the presence of retained Descemet's membrane and decreased endothelial cell density. Visual

The presence of Descemet membrane scrolls is an uncommon finding reported in syphilitic interstitial keratitis. We herein describe a novel surgical strategy for this entity; this report demonstrates that Descemet membrane endothelial keratoplasty surgery can be performed with excellent visual outcomes in the event of corneal edema as a late sequela of syphilitic keratitis.

Canaloplasty is a minimally invasive glaucoma procedure that aims to restore aqueous outflow through the Schlemm canal. Despite its excellent safety profile, complications can occur, including hyphema, creation of a cyclodialysis cleft, and incorrect passage of the catheter into the suprachoroidal space. Descemet membrane detachment (DMD) is an infrequent complication of canaloplasty and can be associated with significant visual decompensation, particularly when involving the central visual axis. Herein, we describe a case of a patient who underwent canaloplasty through an iTrack microcatheter

Descemet membrane detachment (DMD) is an uncommon but clinically significant complication of cataract surgery, particularly phacoemulsification. This review summarizes current evidence on the incidence, risk factors, diagnosis, and management of DMD associated with cataract surgery. A narrative literature review identified 28 relevant publications between 1995 and 2025 across PubMed Medline, Cochrane Library Database, and Scopus, including retrospective and prospective studies, review articles, and case reports. Most cases of DMD result from mechanical trauma at the clear corneal incision site

PURPOSE: To compare corneal densitometry in a consecutive series of 52 endothelial keratoplasties (DMEK/DSAEK) using a Scheimpflug-based device after six months of follow-up.
METHODS: Corneal densitometry (CD) values of 102 eyes were divided into three main groups: 33 DMEKs, 19 DSAEKs, and 50 healthy eyes without previous ocular surgery. The CD values were then analyzed and compared between the groups. We measured three main layers in depth and four different concentric zones at 1, 3, and 6 months postoperatively.
RESULTS: In the DMEK group, total CD significantly decreased from 38.02 ± 10.2

PURPOSE OF REVIEW: To review the current literature on Descemet membrane endothelial keratoplasty (DMEK) in complex eyes.
RECENT FINDINGS: DMEK surgery has become a standardized procedure in Fuchs endothelial dystrophy and simple bullous keratopathy. But eyes with more complex disease present unique intraoperative and postoperative challenges to the DMEK surgeon. Poor visualization during surgery, complex anterior segment anatomy, altered anterior chamber dynamics, glaucoma shunts, and congenital or iatrogenic missing or altered iris and lens make DMEK surgery extremely difficult to accomplis
