BACKGROUND: This study aims to quantitatively and qualitatively evaluate the activities of a Bioethics Unit (BU) 5 years since its implementation (2016-2020). The BU is a research unit providing empirical research on ethical issues related to clinical practice, clinical ethics consultation, and ethical education for health care professionals (HPS).
METHODS: We performed an explanatory, sequential, mixed-method, observational study, using the subsequent qualitative data to explain the initial quantitative findings. Quantitative data were collected from an internal database and analyzed by desc

Health care professionals and patients impacted by incarceration face unique medical, legal, and ethical issues. The frequency and nature of ethics consultations for these patients are underexplored. This study aimed to characterize the primary ethical issue and contextual features of ethics consultations involving patients impacted by incarceration. We conducted a qualitative concept content analysis of ethics consultations involving patients impacted by incarceration and calculated descriptive statistics of demographics to compare these patients with the broader population of patients impact

PurposeThe coronavirus disease 2019 (COVID-19) pandemic has raised a variety of ethical dilemmas for health care providers. Limited data are available on how a patient's concomitant cancer diagnosis affected ethical concerns raised during the early stages of the pandemic.MethodsWe performed a retrospective review of all COVID-related ethics consultations registered in a prospectively collected ethics database at a tertiary cancer center between March 14, 2020, and April 28, 2020. Primary and secondary ethical issues, as well as important contextual factors, were identified.ResultsTwenty-six cl

This paper argues that philosophical practice can complement existing medical ethics structures by offering a publicly accessible space for discourse and negotiation of basic concepts that are relevant to ethical decision making. The potential of collaboration becomes particularly evident by the example of assisted dying: it raises a wide range of philosophical questions which, however, tend to remain unarticulated where there is urgency for action and focus on ethical dilemma. In this paper, we first provide an analysis of the neglect of profound questions in ethics consultation and why these

Ethics consultation's (EC) role in supporting healthcare professionals, patients, and families confronted by eating disorders (ED) is under-described in the literature. We used qualitative conceptual content analysis to characterize the ethical issues and contextual features of EC involving patients with Anorexia Nervosa (AN), Bulimia Nervosa (BN), Binge-Eating Disorder (BED), Avoidant Restrictive Food Intake Disorder (ARFID), or Other Specified Feeding or Eating Disorder (OSFED) and compared adult vs. pediatric settings at a single institution from 2015 to 2024. We identified 40 consultations

Patients with serious illness hospitalized in the pediatric setting can receive support from Pediatric Palliative Care (PPC) and Ethics Consultation (EC). However, empirical data on dual PPC-EC involvement in pediatrics is limited. Therefore, we characterized ethical issues, contextual features, palliative care domains, and consultation timing for inpatients with dual PPC-EC involvement. We reviewed 135 EC from 2016 to 2023 at an academic children's hospital to identify cases with PPC involvement. PPC notes, demographics, and admission, consultation and discharge dates were extracted from the

BACKGROUND: Caring for patients with anorexia nervosa (AN) is associated with high levels of moral distress among healthcare professionals. The main moral conflict has been posited to be between applying coercion to prevent serious complications such as premature death and accepting treatment refusals. However, empirical evidence on this topic is scarce.
METHODS: We identified all 19 documentations of ethics consultations (ECs) in the context of AN from one clinical ethics support service in Switzerland. These documentations were coded with a sequential deductive-inductive approach and the co

Patients, families, and health care providers have a right to expect that ethics consultants can deal competently with the complex issues that they are asked to address. The Society for Health and Human Values-Society for Bioethics Consultation Task Force on Standards for Bioethics Consultation explored core competencies and related issues in ethics consultation. This position paper summarizes the content of the resulting Task Force Report, which included nine general conclusions: 1) U.S. societal context makes "ethics facilitation" an appropriate approach to ethics consultation; 2) ethics fac

Few institutions have published reviews concerning the case consultation history of their ethics committees, and policies used by ethics committees to address inappropriate treatment are infrequently reviewed. We sought to characterize the operation of our institution's ethics committee as a representative example of a volunteer ethics committee, and outline its use of a policy to address inappropriate treatment, the Conscientious Practice Policy (CPP). Patients were identified for retrospective review from the ethics consultation database. Patient demographics, medical admission information,

OBJECTIVE: To characterize clinical ethical dilemmas that arise in the care of international patients in the United States and to propose preventive ethics solutions.
PATIENTS AND METHODS: We conducted a retrospective chart review of ethics consultations placed on international patients at a quaternary medical center over a 19-year period (February 8, 2006, to April 5, 2024).
RESULTS: Forty-nine international patients met inclusion criteria. Most were adults (88%) with a primary diagnosis of malignancy (45%). Most patients came from countries in the Middle East (84%), spoke Arabic (73%), and

As part of a project to examine health care ethics consultation in Canada, we surveyed individuals who were considered by themselves or others to play a significant role in health care ethics consultation. Since one goal of the project was to examine the education and abilities necessary for consultants, we sought to determine the qualifications and skills currently possessed by persons considered to be ethics consultants. For the purposes of the questionnaire, 'health care ethics consultation' was defined broadly to include consultation on ethical issues in clinical care or in clinical resear

ContextClinical ethics and palliative care are distinct consultation services that are increasingly used in hospitals to support complex medical decisions, particularly at the end of life. Concurrent utilization of these services is poorly characterized despite documented benefits of collaboration.ObjectivesTo compare demographic, clinical, and utilization patterns among patients who received an inpatient clinical ethics consultation and did or did not receive a palliative care consultation.MethodsWe conducted an exploratory retrospective cohort study as part of a broader quality improvement i

In November 2018 the American Society for Bioethics and Humanities (ASBH) administered the first Healthcare Ethics Consultant Certification examination to 138 candidates, 136 of whom (98.5 percent) passed and were "certified" as "healthcare ethics consultants." I believe this certification process is both premature and inadequate. Certification for ethics consultants is premature because, as Kornfeld and Prager state repeatedly in their article in this issue of The Journal of Clinical Ethics, "The Clinician as Clinical Ethics Consultant: An Empirical Method of Study," there is a lack of "empir

INTRODUCTION: The Ethics committee (EC) are an essential part of the health institutions, however, in many occasions their response capacity is slow, being organs perceived as distant from the clinical practice. This means that they receive limited consultations, leaving many decisions to the sole discretion of the physician in charge.
MATERIAL AND METHODS: A prospective study was designed based on anonymous surveys for professionals, family members and patients attended at the Hospital Ramón y Cajal. The analysis included a description of demographic variables, information on knowledge of th

Gestational diabetes mellitus (GDM) sits at the intersection of maternal bodily integrity, fetal risk mitigation, and healthcare equity, and rigid guideline enforcement can trigger bedside conflicts and institutional friction. This article examines GDM management through the deliberative model of the Spanish physician and philosopher Diego Gracia, influential in Southern Europe and Latin America but less familiar in North America, which separates moral duties into Level I (Non-Maleficence and Justice) and Level II (Autonomy and Beneficence). We explain why we start from Gracia rather than from

INTRODUCTION: Ethics consultations are often needed at difficult junctures of medical care. However, data on the nature of how patient characteristics, including race/ethnicity, language, and diagnosis, affect ethics consult outcomes are lacking.
METHODS: We performed a retrospective cohort study of all patients who were seen by the Ethics Consult Service between 2017 and 2021 at a large tertiary academic center with the aim of determining whether patient demographic and clinical factors were associated with the timing of ethics consult requests and recommendations of the ethics team.

Surgical treatment is only one part of the management of the child with cleft lip and palate. This paper exclusively focuses on other important aspects of this management. This includes the information and psychological supports of the parents, whether the cleft lip is diagnosed prenatally or at birth, the practical aspects of the consultation within the team of the different specialists involved (surgeon, anesthetist, dentist, orthodontist, speech therapist, otorhinolaryngologist, geneticist, child psychologist). The school teacher must also be concerned at the beginning of the first school y
