Delayed Diagnosis of Anastomotic Leak and Failure to Rescue After Colon Resection.

IMPORTANCE: Anastomotic leak remains a leading cause of morbidity and mortality following colon resection. There is increasing evidence to suggest that failure to rescue (FTR), defined as death after a complication, is the culmination of a series of cascading events, which may be exacerbated by delays in diagnosis. Timely identification and management of anastomotic leaks may represent a crucial strategy for reducing FTR after colon resection. OBJECTIVE: To determine whether delayed diagnosis of anastomotic leak is associated with FTR following colon resection. DESIGN, SETTING, AND PARTICIPA

Frailty Predicts Failure to Rescue Following Malignant Brain Tumor Resection: A National Surgical Quality Improvement Program Analysis of 14,721 Patients/ (2012-2020).

OBJECTIVE: Failure to rescue (FTR) is defined as mortality within 30 days following a major complication. While FTR has been studied in various brain tumor resections, its predictors in malignant brain tumor resection (mBTR) remain unexplored. This study aims to identify FTR predictors in mBTR resection patients using a frailty-driven model. METHODS: Patients undergoing craniotomy for mBTR were identified from the American College of Surgeons-National Surgical Quality Improvement Program database (2012-2020), with frailty measured by the Risk Analysis Index (RAI). RESULTS: Of 14,721 mBTR pat

Sex-based differences in mortality and failure to rescue after major gastrointestinal cancer resections: real-world evidence from a nationwide cohort.

BACKGROUND: Sex-based differences in outcomes after oncological surgery remain underexplored despite growing interest in personalized and equitable cancer care. Sex differences in in-hospital mortality and failure-to-rescue rates after major gastrointestinal cancer surgery were analysed using nationwide real-world data. METHODS: German Diagnosis Related Groups discharge data from 2010-2023 were analysed, including adult patients undergoing curative-intent resections for colorectal, gastric, pancreatic, and oesophageal cancers. Multivariable generalized linear models were applied to estimate a

Using the patient safety indicators to detect potential safety events among US veterans with psychotic disorders: clinical and research implications.

OBJECTIVE: Patients with psychotic disorders often experience poorer health outcomes, but whether they experience increased risks of medical errors/patient safety events is less clear. A single state-level US study found acute care inpatients with schizophrenia were at higher risk of incurring some of the Agency for Healthcare Research and Quality (AHRQ) patient safety indicators (PSIs). We examined a nationwide sample of US Veteran's Health Administration (VHA) acute care inpatients to determine the rates observed among patients with psychotic disorders for a larger set of PSIs. DESIGN: Retr

Nationwide Analysis of Failure to Rescue After Liver Transplantation.

INTRODUCTION: Failure to rescue (FTR) is mortality after a major complication. FTR may be an effective quality metric in liver transplantation (LT). However, there is a paucity of nationwide data on the rates and effects of FTR on outcomes. Our study aims to determine the nationwide rate of FTR and its impact on outcomes after LT. METHODS: We analyzed the 2015-2017 Nationwide Readmissions Database, including all patients with LT. Patients were stratified into terciles of average center mortality of < 1% for low (L), 1%-5.76% for intermediate (I), and > 5.76% for high (H). Postoperative compli

Failure to rescue after major gynecologic cancer surgery: A National Surgical Quality Improvement Program analysis.

OBJECTIVE: Failure to rescue (FTR), defined as mortality following a postoperative complication, is a more comprehensive quality metric than complication or mortality rates alone. This study evaluated the incidence of FTR and associated risk factors in major gynecologic oncology surgery. METHODS: The American College of Surgeons National Surgical Quality Improvement Program was queried for patients undergoing gynecologic oncology procedures between 2013 and 2019 and 2022-2024; data from 2020 to 2021 were excluded due to pandemic-related disruption. Primary outcome was FTR, defined as death wi

Avoidable 30-day mortality analysis and failure to rescue in dysvascular lower extremity amputees.

Background and purpose - An enhanced treatment program may decrease 30-day mortality below 20% after lower extremity amputations (LEA). The potential and limitations for further reduction are unknown. We analyzed postoperative causes of 30-day mortality, and assessed failure to rescue (FTR) rate in LEA patients who followed an enhanced treatment program. Patients and methods - Medical charts of 195 primary LEA procedures were reviewed independently by 3 of the authors, and deaths during hospitalization following amputation were classified according to consensus. Results - 31 patients died with

Area-based study identifies risk factors associated with missed antenatal corticosteroid prophylaxis in women delivering preterm infants.

AIM: All women delivering a preterm infant should receive antenatal corticosteroid prophylaxis, but many miss this opportunity. We determined the risk factors associated with missed prophylaxis in a geographically defined area of Italy. METHODS: We prospectively studied all mothers who delivered babies between 24 and 31 completed weeks of gestation, from 2009 to 2013, in all maternity units in Tuscany. RESULTS: Of 1232 mothers, 186 (15.1%) did not receive prophylaxis. The risk was higher in migrant mothers, with an adjusted risk ratio (RR) of 1.28 and 95% confidence interval (95% CI) of 1.04

Clinical Implications of Maternal Disparities Administrative Data Research.

Administrative data research on maternal racial disparities supports 2 broad clinical inferences. First, failure to rescue in terms of both death and severe maternal morbidity likely accounts for a significant proportion of maternal disparities. Second, risk for adverse outcomes by race is generally differential with risk for cardiovascular complications particularly high for non-Hispanic black women. These differentials suggest that underlying health conditions may represent an important contributor to overall disparities, and optimal longitudinal care utilization with nonobstetric specialist

Geriatric rescue after surgery (GRAS) score to predict failure-to-rescue in geriatric emergency general surgery patients.

BACKGROUND: Geriatric-patients(GP) undergoing emergency-general-surgery(EGS) are vulnerable to develop adverse-outcomes. Impact of patient-level-factors on Failure-to-Rescue(FTR) in EGS-GP remains unclear. Aim of our study was to determine factors associated with FTR(death from major-complication) and devise simple-bedside-score that predicts FTR in EGS-GP. METHODS: 3-year(2013-15) analysis of patients, age≥65y on acute-care-surgery-service and underwent EGS. Regression analysis used to analyze factors associated with FTR and natural-logarithm of significant odds-ratio used to calculate estim

Selection of complications to define failure to rescue as an optimal quality improvement metric.

BACKGROUND: Failure to rescue (FTR), defined as death after a surgical complication, is strongly impacted by systems-level care processes. The purpose of this study was to optimize the definition of FTR by developing the methodology for, and evaluating the subsequent impact of, adding complications to the Society of Thoracic Surgeons (STS) definition. METHODS: Patients undergoing coronary artery bypass grafting and/or valve operations from 2011 to 2024 in Michigan were included. Complications were considered for the FTR definition on the basis of the complication's association with mortality,

Impact of hospital volume on patient safety indicators and failure to rescue following open aortic aneurysm repair.

OBJECTIVE: Failure to rescue (FTR), a patient safety indicator (PSI) defined, codified, and adjudicated by the Agency for Healthcare Research and Quality, is classified as a preventable inpatient death following major complications. FTR has been reported to be a significant driver of postoperative mortality after open abdominal aortic aneurysm (OAAA) repair. The association between hospital volume (HV) and mortality is well known; however, the mechanisms responsible for these improved outcomes and relative contribution to observed interhospital variation is poorly understood. Similarly, HV inf
