False amblyopia prediction in strabismic patients by fixation preference testing correlates with contralateral ocular dominance.

BACKGROUND: Binocular fixation preference testing is commonly used to predict strabismic amblyopia in preverbal children. However, false positives can occur and can lead to unnecessary patching treatment. We explored potential causes for this phenomenon. METHODS: In this prospective cohort study (2013-2014) of consecutive verbal strabismic patients without incomitance or decreased vision other than strabismic amblyopia, binocular fixation preference testing was graded from 1 (strong preference) to 4 (free alteration), with grade 1 or 2 considered predictive of amblyopia and the examiner maske

Ocular lateral deviation with brief removal of visual fixation differentiates central from peripheral vestibular syndrome.

OBJECTIVE: Ocular lateral deviation (OLD) is a conjugate, ipsilesional, horizontal ocular deviation associated with brief (3-5 s) closing of the eyes, commonly linked to the lateral medullary syndrome (LMS). There is limited information regarding OLD in patients with the acute vestibular syndrome (AVS). In one case series 40 years ago OLD was suggested to be a central sign. Recently, horizontal ocular deviation on imaging (RadOLD) was frequently associated with anterior circulation stroke and horizontal gaze palsy. Similarly, RadOLD has been associated with posterior circulation stroke, e.g.,

Multifaceted Interactions of Stereoacuity, Inter-Ocular Suppression, and Fixation Eye Movement Abnormalities in Amblyopia and Strabismus.

PURPOSE: Amblyopic and strabismus subjects experience inter-ocular suppression, impaired stereoacuity, and increased fixation instability. The purpose of the study was to investigate factors affecting suppression and stereoacuity and examine their relationship to fixation eye movement (FEM) abnormalities. METHODS: We recruited 14 controls and 46 amblyopic subjects (anisometropic = 18, strabismic = 14, and mixed = 14) and 11 subjects with strabismus without amblyopia. We utilized the dichoptic motion coherence test to quantify suppression, and stereoacuity was assessed using the Titmus Fly tes

Inter-Ocular Fixation Instability of Amblyopia: Relationship to Visual Acuity, Strabismus, Nystagmus, Stereopsis, Vergence, and Age.

PURPOSE: Amblyopia damages visual sensory and ocular motor functions. One manifestation of the damage is abnormal fixational eye movements. Tiny fixation movements are normal; however, when these exceed a normal range, the behavior is labeled "fixation instability" (FI). Here we compare FI between normal and amblyopic subjects, and evaluate the relationship between FI and severity of amblyopia, strabismus angle, nystagmus, stereopsis, vergence, and subject age. METHODS: Fixation eye movements were recorded using infrared video-oculography from 47 controls (15.3 ± 12.2 years of age) and 104 am

The agreement between the Irvine 4 diopter prism test and assessment of ocular fixation in microtropia with identity.

Microtropia describes a primary ocular deviation of less than 10 prism diopters associated with harmonious anomalous retinal correspondence and reduced stereopsis. It is routinely accepted that children with microtropia are less likely to achieve equal vision following occlusion therapy than those with bifoveal fixation. The most commonly used methods of diagnosing a microtropia are the 4 diopter prism test (4∆PT) and assessment of ocular fixation. This study examines the agreement between the two tests. One hundred and twelve typically developing children without a manifest strabismus who wer

Anterior chamber and retropupillary iris-claw intar ocular lens fixation comparison of clinical outcomes: a systematic review.

BACKGROUND: In cataract surgery, optimal intraocular lens (IOL) placement is typically within the capsular bag. However, in the absence of sufficient capsular support, alternative techniques such as scleral-sutured IOLs, anterior chamber IOLs, and iris-fixated IOLs either in the anterior chamber or retropupillary are employed. The choice between these methods depends on factors like surgeon expertise, patient-specific anatomy, and clinical circumstances, with anterior chamber iris-claw IOLs offering a more straightforward approach and retropupillary techniques potentially providing additional

A novel technique of modified continuous blanket suture for amniotic membrane fixation in severe ocular surface diseases.

The purpose of this article is to demonstrate a novel technique using modified continuous blanket suture (MCBS) to fix the amniotic membrane (AM) in different severe ocular surface disease lesions. The MCBS techniques were used to fix the AMs of 5 representative patients with different ocular surface lesions related to severe ocular surface diseases. In all cases, stable adherence of the AM was maintained until the epithelialization of the ocular surface was completed. No early detachment, dissolution, or dislocation of the AM patch was observed. During follow-up, all patients acquired a smoot

[Reversible translimbal suture fixation of the ciliary body in treatment of ocular persistent hypotonia after surgical cyclodialysis].

UNLABELLED: After surgical or traumatical cyclodialysis dangerous hypotony can occur. In cases with peripheral anterior synechia the fixation of the ciliary body by reversible suture can bring about the desired pressure rise. PATIENTS: In nine eyes with persistent ocular hypotony after cyclodialysis 13 ciliary body fixations were performed. RESULTS: In four eyes the operation was successful. In two eyes a second operation was necessary. In two cases there was a high postoperative pressure rise. Cutting of the sutures led to a sudden decrease in pressure. In these eyes a second operation with

A Novel Sutureless Scleral-Fixated Lens That Eliminates Extra Ocular Haptic Manipulation: The CM-T Flex Lens.

The authors present their experience with an innovative model of scleral-fixated intraocular lens (SFIOL): the CM-T Flex intraocular lens (IOL). By its unique design, the CM-T Flex IOL simplifies the process of SFIOL fixation by limiting the process to simply exteriorizing and releasing the haptic. This retrospective cohort study included nine eyes with surgical aphakia. The CM-T Flex IOL was used in these nine aphakic eyes that had completed a 3-month follow-up. Improvement in visual acuity (0.34 ± 0.25 logMAR [range: 0.2 to 1] to 0.22 ± 0.32 logMAR [range: 0 to 0.77]) was recorded with no ma

The Neonatal Fc Receptor and Complement Fixation Facilitate Prophylactic Vaccine-Mediated Humoral Protection against Viral Infection in the Ocular Mucosa.

The capacity of licensed vaccines to protect the ocular surface against infection is limited. Common ocular pathogens, such as HSV-1, are increasingly recognized as major contributors to visual morbidity worldwide. Humoral immunity is an essential correlate of protection against HSV-1 pathogenesis and ocular pathology, yet the ability of Ab to protect against HSV-1 is deemed limited due to the slow IgG diffusion rate in the healthy cornea. We show that a live-attenuated HSV-1 vaccine elicits humoral immune responses that are unparalleled by a glycoprotein subunit vaccine vis-à-vis Ab persisten

Five-Year Follow-Up of an Eight-Year-Old Boy With a Scleral-Fixated Carlevale Intraocular Lens After Ocular Trauma.

Management of pediatric aphakia without capsular support remains challenging, particularly following ocular trauma. Long-term outcomes of sutureless scleral fixation of the Carlevale intraocular lens (IOL) in children remain limited. We present the surgical management and five-year follow-up of a child with traumatic aphakia treated with a Carlevale IOL. An eight-year-old boy sustained a penetrating ocular injury caused by a knife, resulting in a full-thickness corneal laceration, traumatic cataract, and inadequate capsular support. Following primary corneal repair and lens removal elsewhere,
