Comparison of the Forefoot Pressure-Relieving Effects of Foot Orthoses.

PURPOSE: Metatarsal pads are prescribed to decrease forefoot plantar pressure. However, the appropriate metatarsal pad placement location remains a debated issue. We aimed to identify the most effective insole design that reduces forefoot plantar pressure and determine the optimal position for metatarsal pad placement. MATERIALS AND METHODS: We recruited 16 healthy adult volunteers, and measured their plantar pressure data during walking while using an in-shoe system. The volunteers were randomly assigned to one of four insole conditions: 1/16-inch insole (i.e., control), 1/4-inch soft plasta

Evidence for foot orthoses for adults with flatfoot: a systematic review.

BACKGROUND: Flatfoot is characterised by the falling of the medial longitudinal arch, eversion of the hindfoot and abduction of the loaded forefoot. Furthermore, flatfoot leads to a variety of musculoskeletal symptoms in the lower extremity, such as knee or hip pain. The standard conservative treatment for flatfoot deformity is exercise therapy or treatment with foot orthoses. Foot orthoses are prescribed for various foot complaints. However, the evidence for the provision of foot orthoses is inconsistent. The aim of this systematic review is to synthesize the evidence of foot orthoses for adu

The effect of two types of foot orthoses on rearfoot mechanics.

Little experimental data exist regarding the comparative biomechanical of various foot orthoses. This study evaluated the comparative effect of biomechanical orthoses and over-the-counter arch supports on controlling rearfoot pronation. Twenty-four patients with forefoot varus deformity were studied while walking on a treadmill. Two-dimensional, videotape motion analysis was used for studying rearfoot mechanics with three experimental conditions: 1) shoes only, 2) shoes plus arch supports, and 3) shoes plus biomechanical orthoses. The variables studied were: maximum pronation, calcaneal eversi

Gait abnormalities in hemiplegia: their correction by ankle-foot orthoses.

Hemiparetic gait is characterized by slow speed and poorly coordinated movements. Because the values of gait parameters vary with changes in speed, the slow speed that is typical of hemiparetic gait necessitates applying controls for the influence of speed when comparing hemiparetic and able-bodied persons. Gait kinetics and kinematics were measured in seven hemiparetic and seven able-bodied adults to compare their gait patterns at similar speeds and to assess the effectiveness of ankle-foot orthoses which were double-stopped in 5 degrees of dorsiflexion or 5 degrees of plantarflexion. Hemipar

Biomechanical Evaluation and Strength Test of 3D-Printed Foot Orthoses.

Foot orthoses (FOs) are commonly used as interventions for individuals with flatfoot. Advances in technologies such as three-dimensional (3D) scanning and 3D printing have facilitated the fabrication of custom FOs. However, few studies have been conducted on the mechanical properties and biomechanical effects of 3D-printed FOs. The purposes of this study were to evaluate the mechanical properties of 3D-printed FOs and determine their biomechanical effects in individuals with flexible flatfoot. During mechanical testing, a total of 18 FO samples with three orientations (0°, 45°, and 90°) were f

Analysis of stiffness reduction in varying curvature ankle foot orthoses.

Ankle foot orthoses (AFO) are often used for patients who cannot generate a strong enough extension moment at the knee to allow functional gait. Orthotists often cut out portions of the AFO around the malleoli in order to improve comfort. There has been some question as to how this affects the stress distribution around the orthosis, the fatigue performance of the device, and the AFOs stiffness. To examine this, three orthoses were constructed with differing curvatures cut out of the malleolar regions. Photoelastic coatings were placed on the most stiff and least stiff orthoses, and the stress

Biomechanical Evaluation of Custom Foot Orthoses for Hallux Valgus Deformity.

The purpose of the present study was to compare the hallux valgus deformity pressure parameters seen in standard footwear (no orthosis) versus the pressure observed in the same footwear with the addition of 3 different length orthoses. The forefoot pressure at a hallux valgus deformity was recorded with pressure sensors placed on the plantar, medial, and dorsal surface of the first metatarsal head. The participants performed walking trials without an orthosis and with orthoses of 3 different lengths. The average pressure and maximum pressure of each area was recorded for each orthosis, and com

[Functional impact of carbon fiber springs in ankle-foot orthoses].

BACKGROUND: Patients with calf muscle insufficiency are often dependent on ankle-foot orthoses (AFO). The orthosis should improve walking and posture and should prevent structural deformities. AFOs are usually manufactured with a dorsiflexion stop. In the current study, orthoses with a dorsal carbon fiber spring were compared with the classic design. PATIENTS AND METHODS: Five patients with spina bifida, one with poliomyelitis, and one with a tibial nerve block took part in the study. All participants underwent a 3-D gait analysis including data collection of kinematics (VICON) and kinetics (

The effect of foot orthoses on standing foot posture and gait of young children with Down syndrome.

The purpose of this study was to determine if foot orthoses (FOs) immediately affected gait of children with Down Syndrome (DS) and excessively pronated feet. Subjects were 3--6 years old; 16 with DS and 10 without disabilities. Gait of children with DS wearing sneakers was compared with and without FOs and compared to gait of children without disabilities. Ankle moment, walking speed during stance phase, sneaker prints, center of pressure, and transverse plane foot angles were collected and compared. In standing, heel eversion was reduced when the children wore FOs (p=0.000). During gait the

Impact of foot orthoses hindfoot and forefoot wedges on foot and ankle kinematics: A cadaveric study.

INTRODUCTION: Foot orthoses (FOs) are widely prescribed to manage musculoskeletal and inflammatory disorders affecting the lower limbs by modifying foot and ankle biomechanics. While the addition of medial wedges to FOs is believed to enhance control of pronation-related movements, few studies have quantified their isolated kinematic effects under controlled experimental conditions. The use of cadaveric models allows for precise measurement of bone motion without soft tissue artifacts. Therefore, this study aimed to quantify the effects of 3D-printed FOs with and without a medial hindfoot-fore

Clinical Outcomes of Custom Foot Orthoses in Progressive Collapsing Foot Deformity: A Retrospective Cohort Analysis.

BACKGROUND: Progressive collapsing foot deformity (PCFD) is a painful and function-limiting condition most commonly affecting middle-aged and older adults. Although custom foot orthoses (CFOs) are routinely prescribed as a conservative intervention, evidence from large clinical cohorts remains limited. This study aimed to characterise patients with PCFD seeking orthotic care, examine demographic factors associated with pain improvement, evaluate clinical and self-reported outcomes following CFO use, and explore potential interaction effects of CFOs on clinical improvement across sex. METHODS:

Deformable foot orthoses shift the functional roles of foot and ankle joints during walking gait.

Carbon fiber deformable foot orthoses (DFOs) with customizable stiffness may benefit clinical populations by allowing an individual approach to enhancing gait energetics. Prior research suggests that increasing DFO stiffness can decrease energy loss while enhancing energy return at the metatarsophalangeal (MTP) joint, making it more spring-like. However, it remains unclear whether increasing DFO stiffness enhances the mechanics of the midtarsal and ankle joints or only the MTP joint, especially during walking. This study investigated whether walking with increasingly stiff DFOs impacted mechan
