Moving Frailty Toward Clinical Practice: NIA Intramural Frailty Science Symposium Summary.

Frailty has long been an important concept in the practice of geriatric medicine and in gerontological research, but integration and implementation of frailty concepts into clinical practice in the United States has been slow. The National Institute on Aging (NIA) Intramural Research Program and the Johns Hopkins Older Americans Independence Center sponsored a symposium to identify potential barriers that impede the movement of frailty into clinical practice and to highlight opportunities to facilitate the further integration of frailty into clinical practice. Primary and subspecialty care pro

[Association between frailty awareness and frailty among community-dwelling older adults].

AIM: This study investigated the association between frailty awareness-defined as understanding the term "frailty" and its meaning-and actual frailty status as well as oral frailty among community-dwelling older adults. METHODS: This cross-sectional study was conducted among adults ≥65 years old residing in the community. Data were collected via a postal survey assessing frailty awareness, status (using a health assessment questionnaire for the national screening program for older adults in Japan), and oral frailty (using the Oral Frail Index-8). Frailty awareness was divided into three categ

Multiple frailty model for clustered interval-censored data with frailty selection.

Interval-censored time-to-event data often occur in studies of diseases where the symptoms of interest are not directly observable but require lab examinations for detection. Furthermore, the independence assumption among observations may not be valid if they are from clusters. Some methods have been developed for analysing clustered interval-censored data with a shared frailty to account for overall heterogeneity. In this paper, we propose a multiple frailty proportional hazards model, where we not only account for the baseline heterogeneity and effect variation across clusters for predictors

Frailty prevalence and factors associated with the Frailty Phenotype and Frailty Index: Findings from the North West Adelaide Health Study.

OBJECTIVE: To determine the prevalence of frailty and associated factors in the North West Adelaide Health Study (2004-2006) using the Frailty Phenotype (FP) and Frailty Index (FI). METHODS: Frailty was measured in 909 community-dwelling participants aged ≥65 years using the FP and FI. RESULTS: The FP classified 18% of participants as frail and the FI 48%. The measures were strongly correlated (r = 0.76, P < 0.001) and had a kappa agreement of 0.38 for frailty classification, with 37% of participants classified as non-frail by the FP being classified as frail by the FI. Being older, a curren

Frailty as a Predictor of Mortality in Dialysis: A Multicenter Validation of the Clinical Frailty Scale.

RATIONALE & OBJECTIVE: Frailty is highly prevalent in dialysis and is associated with poor outcomes, yet scalable assessment in routine care remains challenging. We evaluated the feasibility and prognostic performance of the clinical frailty scale (CFS) applied by dialysis nurses and compared it with the REIN predictive score (RPS), a comorbidity-based mortality tool routinely available in French dialysis care. STUDY DESIGN: A prospective multicenter observational cohort. SETTING & PARTICIPANTS: Adults receiving maintenance dialysis at 6 centers in northeastern France (n = 792 prevalent pati

The impact of BMI on frailty screening and the frailty phenotype in ventral hernia patients.

BACKGROUND: Ventral hernia repair (VHR) is a common procedure, but assessing surgical candidacy remains complex, particularly in the setting of obesity. Frailty is a well-established predictor of poor surgical outcomes and is traditionally associated with underweight phenotypes. However, obesity is increasingly recognized as a risk factor for frailty as well. Given the lack of consensus on the optimal frailty screening tool and the high prevalence of obesity in the VHR population, understanding how frailty presents in VHR patients with high BMI is essential. This study examines how obesity inf

Canine Model of Human Frailty: Adaptation of a Frailty Phenotype in Older Dogs.

Frailty is a clinical presentation resulting from age-related cumulative declines in several physiological systems. The aim of this study was to adapt the concept of frailty to the domestic dog, as a model for frailty research, by characterizing a 5-criterion frailty phenotype using objective measurement, and to investigate its independent association with death. A prospective cohort including 80 Labrador and Golden Retriever dogs aged 9 years or older was conducted between March 2015 and July 2020. An adapted frailty phenotype was defined according to the presence of 5 criteria (weakness, slo

Social frailty as social aspects of frailty: Research, practical activities, and prospects.

With the topical focus on the prevention of and countermeasures for frailty, scattered studies have subdivided its social elements and aspects as "social frailty." While the concepts and definition of "social frailty" are yet to be established, the purpose of using the term is to capture the attention of professionals using the social aspects of frailty to support older adults. The goal is to increase healthy life expectancy by recognizing that social factors are unlikely to pose a risk of functional decline in isolation, but may increase such a decline when combined with other factors. At fea

A Comparison of Different Frailty Scores and Impact of Frailty on Outcome in Patients With Cirrhosis.

BACKGROUND & AIMS: There is no "gold standard" tool for the assessment of frailty in cirrhosis. This study compares Liver Frailty Index (LFI), Short Physical Performance Battery (SPPB), Fried Frailty Criteria (FFC), and Clinical Frailty Scale (CFS) for frailty assessment and ascertains its impact on predicting mortality and hospitalizations in a cohort of outpatients with cirrhosis. METHODS: 116 patients were enrolled in this prospective observational cohort study. Frailty assessment was done using LFI, SPPB, FFC, and CFS. All patients were followed up for 6 months. The primary outcome was th

Physical frailty predicts the development of social frailty: a prospective cohort study.

BACKGROUND: It has not been clarified whether physical frailty symptoms predict social. frailty. The purpose of this study was to elucidate the effect of physical frailty on social frailty, and to determine which domains of physical frailty predict the development of social frailty. METHODS: We employed a two-year prospective cohort study. A total of 342 socially robust community-dwelling older adults were recruited. We used a modified social frailty screening index consisting of four social domains including financial difficulties, living alone, social activity, and contact with neighbors. P

Does the frailty index applied to randomised controlled trials really measure frailty?

BACKGROUND: Cumulative deficit frailty indices from randomised controlled trials (RCT) are increasingly used to assess whether trial findings are applicable to people living with frailty. The aim of this paper was to examine the range and type of deficits included in these frailty indices and compare these to those from cohort studies. METHODS: We identified RCTs assessing treatment effect modification using the cumulative deficit frailty index, as well as cohort studies assessing mortality risk associated with frailty, from recent systematic reviews. We extracted the deficits included in the

Role of sarcopenia, obesity and sarcopenic obesity in frailty, frailty transitions and death.

BACKGROUND & AIMS: The role of sarcopenic obesity in the transitions of frailty has been largely neglected. The aim of this study is to assess the role of sarcopenic obesity with prevalent frailty, frailty transitions and mortality. METHODS: This is a cohort-based retrospective study including 1538 (74.73 years, 45.51% men) community-dwelling ≥ 65 years. Frailty status (baseline and at follow-up) was evaluated with the Frailty Phenotype (FP) and the Frailty Trait Scale-5 (FTS5). Comparison of the frailty phenotype and frailty index for identifying vulnerable companion dogs.

Frailty is a multidimensional syndrome of aging increasingly studied in companion dogs. Two main operational models have been developed: the frailty phenotype, based on five clinical criteria, and the frailty index, derived from the accumulation of health deficits. Despite their frequent use in human aging research, no comparative analysis of these models has been conducted in canine populations. Baseline data from 566 dogs aged six years and older in the CaniAge cohort were used to assess frailty using both Frailty Phenotype and Frailty Index. Prevalence of frailty was 6.36% according to the

Are Dysphagia Symptoms Associated With Physical Pre-frailty and Frailty in Older Adults?

ObjectiveTo investigate whether dysphagia symptoms are associated with the incidence of physical pre-frailty and frailty in older adults.MethodsA sample of 149 participants without pre-frailty or frailty at baseline was analyzed. Data were collected at baseline and at a 9-year follow-up. A multinomial regression model was run and controlled for socioeconomic, behavioral, and clinical characteristics.ResultsMany participants (41.6%) had dysphagia symptoms. Dysphagia symptoms were associated with physical pre-frailty (RR = 2.37, 95%CI 1.02-5.50) and frailty (RR = 3.64, 95%CI 1.01-13.21).Conclusi

Association of Oral Frailty with Physical Frailty and Malnutrition in Patients on Peritoneal Dialysis.

Background: Oral frailty is a state between normal oral function and oral hypofunction. Oral frailty progresses to oral hypofunction and dysphagia, which leads to malnutrition, and then to physical frailty and sarcopenia. Oral frailty is reported to be associated with physical frailty and malnutrition in hemodialysis patients, but there have been no reports on peritoneal dialysis (PD) patients. Methods: This prospective cohort study investigated the associations of oral frailty with physical frailty, sarcopenia, and malnutrition in patients on PD. Patients were divided into an oral frailty gro

Nutritional Management of Sarcopenia and Frailty-Shift from Metabolic Syndrome to Frailty.

Older adults have physical and metabolic characteristics, and there are many differences in nutritional outcomes from middle-aged adults. In addition, there are many factors that cause malnutrition peculiar to the older adults, which are not seen in middle-aged adults, and it is easy for them to lose weight and become malnourished. Therefore, nutritional management needs to take into account the age of each subject. Uniform nutritional management can even cause poor health outcomes. The concept of frailty, especially phenotype frailty, and sarcopenia, which have been advocated with the aging o

[Locomotive syndrome and frailty. Frailty in patients with fall & fall-related fracture].

Among geriatric syndromes, fall and fall-related fractures are one of the leading causes of the elderly's need for long-term care. Hip fractures are the typical cases. The underlying diseases of locomotive syndrome, such as sarcopenia, musculoskeletal ambulation disorder symptom complex, and osteoporosis, are closely associated with fall and fall-related fractures. From the stand point of frailty, sarcopenia, which is considered the major cause of aging-associated declines in function and reserve across multiple physiologic systems, plays a role in fall and fall-related fractures. The common a
