BACKGROUND: Wider step width and lower step-to-step variability are linked to improved gait stability and reduced fall risk. It is unclear if patients with spinocerebellar ataxia (SCA) can learn to adjust these aspects of gait to reduce fall risk.
OBJECTIVES: The aims were to examine the possibility of using wearable step width haptic biofeedback to enhance gait stability and reduce fall risk in individuals with SCA.
METHODS: Thirteen people with SCA type 3 performed step width training (single session) using real-time feedback.
RESULTS: Step width increased post-training (19.3 cm, interqua

INTRODUCTION: Gait assessments in a clinical setting may not accurately reflect mobility in everyday life. To better understand gait during daily life, we compared measures that discriminated Friedreich ataxia (FRDA) from healthy control (HC) subjects in prescribed clinic tests and free, daily-life monitoring.
METHODS: We recruited 9 people with FRDA (median age: 20, IQR [12, 48] years). A comparative healthy control (HC) subject cohort of 9 was sampled using propensity matching on age (median age: 18 [13, 22] years). Subjects wore 3 inertial sensors (one each foot and lower back) in the labo

We describe five patients presenting with high fever and isolated cerebellar gait ataxia. In all these patients, neurological examination revealed dysmetria, intention tremor and postural tremor during sustained posture, all restricted to the legs. Brain MRI was normal. In four of these patients, the recording of leg tremor during sustained postures showed a 3-Hz frequency. Cerebellar gait ataxia resolved within 3-10 days. We suggest that the ataxic gait was due to a reversible dysfunction of the spinocerebellar part of the anterior lobe.

Gait and balance difficulties pose significant clinical challenges in Parkinson's disease (PD). The impairment of physiological mechanisms responsible for maintaining natural orthostatism plays a central role in the pathophysiology of postural instability observed in PD. In addition to the well-known rigidity and abnormalities in muscles and joints, various brain regions involved in the regulation of posture, balance, and gait, such as the basal ganglia, cerebellum, and brainstem regions like the pontine peduncle nucleus, are affected in individuals with PD. The recognition of the cerebellum's

OBJECTIVES: With disease-modifying drugs on the horizon for degenerative ataxias, ecologically valid motor biomarkers are highly warranted. In this observational study, we aimed to unravel and validate markers of ataxic gait in real life by using wearable sensors.
METHODS: We assessed gait characteristics of 43 patients with degenerative cerebellar disease (Scale for the Assessment and Rating of Ataxia [SARA] 9.4 ± 3.9) compared with 35 controls by 3 body-worn inertial sensors in 3 conditions: (1) laboratory-based walking; (2) supervised free walking; (3) real-life walking during everyday liv

BACKGROUND: Studies on the natural history of rare, chronic diseases like spinocerebellar ataxia 3 (SCA3) are hard to be done, since patients enter the study with variable disease durations and are followed up at irregular intervals.
AIMS: Our purpose was to use all the available data to describe the progression of gait ataxia in a long-term cohort of patients with SCA3 through a markovian method.
MATERIALS AND METHODS: SCA3 patients were recruited between 1998 and 2005 and were invited to annual neurological follow-ups until 2007. Gait ataxia was described through a mean score graph and a m

BACKGROUND: Chronic lymphocytic inflammation with pontine perivascular enhancement responsive to steroids (CLIPPERS) syndrome is an immune-mediated, treatable and inflammatory CNS disease first reported by Pittock et al. (2010).
CASE PRESENTATION: We describe a 66-year-old man with previous history of diabetes, atrial fibrillation and hypertension, who was admitted to hospital with reduced general condition. He had experienced dizziness and unstable gait for a year, and had been periodically confused, especially in the previous month. MR imaging showed characteristic punctuate and curvilinear

BACKGROUND: Hereditary sensory and autonomic neuropathy type III features marked ataxic gait that progressively worsens over time. We assessed whether proprioceptive disturbances can explain the ataxia.
METHODS: Proprioception at the knee joint was assessed using passive joint angle matching in 18 patients and 14 age-matched controls; 5 patients with cerebellar ataxia were also studied. Ataxia was quantified using the Brief Ataxia Rating Score, which ranged from 7 to 26 of 30.
RESULTS: Neuropathy patients performed poorly in judging joint position: mean absolute error was 8.7° ± 1.0°, and th

Deep brain stimulation (DBS) is an effective therapy for medication-refractory essential tremor, yet clinical outcomes remain highly variable and stimulation-induced side effects, particularly gait ataxia, frequently limit long-term benefit. In routine clinical practice, it is difficult to predict which stimulation settings will provide optimal tremor control while avoiding disabling motor complications. We analysed long-term tremor and gait outcomes in 73 patients from two DBS centres with chronic bilateral (sub)thalamic DBS, assessed before surgery and at follow-up after 12 months using blin

BACKGROUND: The search for valid preclinical biomarkers of cerebellar dysfunction is a key research goal for the upcoming era of early interventional approaches in spinocerebellar ataxias. This study aims to describe novel preclinical biomarkers of subtle gait and postural sway abnormalities in prodromal spinocerebellar ataxia type 2 (pre-SCA2).
METHODS: Thirty pre-SCA2 patients and their matched healthy controls underwent quantitative assessments of gait and postural sway using a wearable sensor-based system and semiquantitative evaluation of cerebellar features by SARA (Scale for the Assess

BACKGROUND: Pose estimation algorithms applied to two-dimensional videos evaluate gait disturbances; however, a few studies have used this method to evaluate ataxic gait.
OBJECTIVE: The aim was to assess whether a pose estimation algorithm can predict the severity of cerebellar ataxia by applying it to gait videos.
METHODS: We video-recorded 66 patients with degenerative cerebellar diseases performing the timed up-and-go test. Key points from the gait videos extracted by a pose estimation algorithm were input into a deep learning model to predict the Scale for the Assessment and Rating of At

Gait ataxia is one of the most common and impactful consequences of cerebellar dysfunction. Purkinje cells, the sole output neurons of the cerebellar cortex, are often involved in the underlying pathology, but their specific functions during locomotor control in health and disease remain obfuscated. We aimed to describe the effect of gradual adult-onset Purkinje cell degeneration on gaiting patterns in mice, and to determine whether two different mechanisms that both lead to Purkinje cell degeneration cause different patterns in the development of gait ataxia. Using the ErasmusLadder together

With disease-modifying drugs on the horizon for degenerative ataxias, ecologically valid, finely granulated, digital health measures are highly warranted to augment clinical and patient-reported outcome measures. Gait and balance disturbances most often present as the first signs of degenerative cerebellar ataxia and are the most reported disabling features in disease progression. Thus, digital gait and balance measures constitute promising and relevant performance outcomes for clinical trials. This narrative review with embedded consensus will describe evidence for the sensitivity of digital g

Cerebellar ataxic gait was compared with normals by a simple quantitative method. Strain gauges attached to the heel, the first and the fifth metatarsal areas of a shoe, were used and the gait was telemetrically recorded. A natural and one per second click sound-synchronized gait was analyzed. A contact sequence of the three areas of the sole, the stride time, the stance-phase time and the foot contact pattern were compared between patients and normals. All of these items disclosed different results in the two groups.

We estimated the severity of cerebellar ataxia by analyzing gait rhythm. We measured the step times in patients with pure cerebellar ataxia and healthy controls and then analyzed the distribution of the ratios of adjacent times. Gait rhythm displayed the best adaptation when expressed as the sum of the power law and lognormal distributions in both groups, and the groups could be distinguished by the exponent of the power law distribution, reflecting the fractal property of gait rhythm. Gait rhythm might reflect different features of impairment in patients with cerebellar ataxia, making it a us
