Glioblastoma is the most aggressive and common malignant primary brain tumor among adults. Due to its aggressiveness, GBM entails a short median overall survival and fatal outcomes. Incidence notably increases with age, 50% of patients being diagnosed at the age of 65 or older. Given that cognitive loss is commonly confused with age-related cognitive impairment, delays in diagnosis are frequent. Furthermore, the elderly are a vulnerable group, commonly having poorer functional status, more comorbidities, and reduced tolerance to intensive treatments. Therefore, clinical and therapeutic implica

Glioblastoma is the most aggressive form of brain tumor in adults and is characterized by the presence of hypervascularization and necrosis, both caused by a hypoxic microenvironment. In this review, we highlight that hypoxia-induced factor 1 (HIF-1), the main factor activated by hypoxia, is an important driver of tumor progression in GB patients. HIF-1α is a transcription factor regulated by the presence or absence of O2. The expression of HIF-1 has been related to high-grade gliomas and aggressive tumor behavior. HIF-1 promotes tumor progression via the activation of angiogenesis, immunosupp

The occurrence of synchronous glioblastoma and meningioma in the absence of hereditary syndromes or radiotherapy is rare. A 63-year-old woman had two adjacent lesions in the left frontal lobe. The histopathology confirmed a fibroblastic meningioma and a glioblastoma.

Chronic stress has long been associated with poor cancer outcomes, yet the mechanisms linking systemic stress responses to glioblastoma progression remain poorly understood. In this issue of Cancer Cell, Yang et al. identify a stress-induced brain-bone marrow axis that rewires monocyte differentiation toward pro-tumorigenic macrophages, revealing unexpected therapeutic opportunities in glioblastoma.

Glioblastoma (GBM) remains associated with poor outcomes, with a median survival of 15-18 months despite maximal safe resection, radiotherapy, and temozolomide. Therapeutic development has increasingly centered on overcoming drug resistance, intratumoral diversity, and restricted delivery across the blood-brain barrier. Progress in targeted therapy includes evaluation of EGFR inhibitors, multitarget tyrosine kinase inhibitors, and FGFR-directed agents, while rare molecularly defined subsets such as BRAF V600E and NTRK fusions offer tumor-agnostic precision approaches. JAK/STAT inhibition, PARP

We have only recently begun to understand how cancer metabolism affects antitumor responses and immunotherapy outcomes. Certain immunometabolic targets have been actively pursued in other tumor types, however, glioblastoma research has been slow to exploit the therapeutic vulnerabilities of immunometabolism. In this review, we highlight the pathways that are most relevant to glioblastoma and focus on how these immunometabolic pathways influence tumor growth and immune suppression. We discuss hypoxia, glycolysis, tryptophan metabolism, arginine metabolism, 2-Hydroxyglutarate (2HG) metabolism, a

The discovery of the glymphatic system and meningeal lymphatic vessels revolutionized the understanding of brain waste clearance. These findings challenged the long-held notion of CNS immune privilege and provided insights into how the brain maintains homeostasis despite lacking conventional lymphatic drainage. However, the impact of pathological conditions, particularly CNS cancers, on the glymphatic system and vice versa remains underexplored. Glioblastoma (GBM) is the most common and aggressive primary adult CNS cancer. Its presence results in glymphatic dysfunction with significant clinica

OUTLOOK FOR THE MANAGEMENT OF GLIOBLASTOMA. Glioblastoma is the most common primary malignant tumour of the central nervous system. The standard treatment consists of surgical resection, followed by concomitant radiochemotherapy and then six courses of adjuvant temozolomide. Tumour-Treating Fields (TTF) extend the median survival to twenty months, at the cost of constant use of a brace and alopecia. These patients require close neuro-oncological monitoring, with clinico-radiological reassessment every two to three months. In the event of progression, there is no standardised treatment, and the

The median survival in glioblastoma is just over a year, with no standard second-line therapy. Ipilimumab is an immune checkpoint inhibitor that activates the anti-tumour immune response by cytotoxic T-lymphocyte antigen-4 blockade. There is significant evidence supporting its role in the treatment of malignant melanoma, including in patients with brain metastases. The addition of the anti-angiogenesis agent, bevacizumab, seems to offer additional benefit and limit the immune-related side-effects of ipilimumab in melanoma. To date there have been no clinical trials investigating this combinati

Glioblastoma multiforme (GB) is the most common and most malignant primary intracranial tumor. Of the 577 patients who underwent surgery for newly diagnosed GB (World Health Organization grade IV) between January 1991 and March 2008 at our department, seven had infratentorial GB (iGB) (incidence 1.2%). Patients younger than 21years of age, as well as patients with gliomatosis cerebri, were excluded from the analysis. We concluded that iGB is rare in adults. Because of its rarity and the non-specific radiological features of iGB, it can easily be misdiagnosed as a brain metastasis, ependymoma o

Some cancers are involved in inherited genetic syndromes. These genetic diseases are suspected of being involved in approximately 1% of gliomas. Few data are available on glioblastomas and their characteristics among these diseases. Familial syndromes known to predispose individuals to glioblastoma are neurofibromatosis type 1, Li-Fraumeni's syndrome, tuberous sclerosis, and Turcot's syndrome. This review discusses glioblastomas related to these diseases and the current knowledge on the statistical, clinical, and molecular biology data. Non-syndromic glioma families are discussed: a better und

Angiogenesis, the formation of new blood vessels from the pre-existing ones, is a hallmark characteristic of glioblastoma, making it an appealing target for treatment development. Given potent anti-cancer efficacy of mefloquine, FDA-approved anti-malarial drug, there is increasing interest in repurposing mefloquine for treatment of cancers, including glioblastoma. In line with these efforts, our work is the first to demonstrate that mefloquine is also an inhibitor of glioblastoma angiogenesis. Using glioblastoma microvascular endothelial cell (GMEC) isolated from glioblastoma patients, we show

Glioblastoma is a rare tumor that is responsible for 4% of all cancer deaths and is thus considered one of the deadliest human tumors. The new biologic drugs that have met success in many solid and hematologic malignancies constantly failed in glioblastoma, even though it harbors the same molecular alterations successfully hit in other tumors. The last advancement in glioblastoma therapy was in 2005, when it was shown that a subset of patients has a survival benefit from a multimodal combined therapy that includes irradiation and chemotherapy with the alkylating agent temozolomide. The patient

Turcot syndrome (TS) is an extremely rare genetic disorder characterized by the concurrent occurrence of primary brain tumors and colorectal cancer. The prognosis for patients with TS is typically poor. A 57-year-old man with TS who developed recurrent glioblastoma and had a family history of colon cancer is reported. In 2022, the patient underwent robot-assisted stereotactic surgery for the resection of a central nervous system (CNS) tumor. Molecular genetic analysis identified microsatellite instability in the DNA mismatch repair (MMR) gene, confirming the diagnosis of TS. Additional mutatio

Despite the development of various novel therapies, glioblastoma (GBM) remains a devastating disease, with a median survival of less than 15 months. Recently, targeted radionuclide therapy has shown significant progress in treating solid tumors, with the approval of Lutathera for neuroendocrine tumors and Pluvicto for prostate cancer by the US Food and Drug Administration (FDA) and the European Medicines Agency (EMA). This achievement has shed light on the potential of targeted radionuclide therapy for other solid tumors, including GBM. This review presents the current status of targeted radio

Outcomes for glioblastoma (GBM) remain poor despite standard-of-care treatments including surgical resection, radiation, and chemotherapy. Intratumoral heterogeneity contributes to treatment resistance and poor prognosis, thus demanding novel therapeutic approaches. Drug repositioning studies on antiretroviral therapy (ART) have shown promising potent antineoplastic effects in multiple cancers; however, its efficacy in GBM remains unclear. To better understand the pleiotropic anticancer effects of ART on GBM, we conducted a comprehensive drug repurposing analysis of ART in GBM to highlight its
